PERSONAL LINES ACCOUNT BILLING PLAN 1. INTRODUCTION EMC s Account Billing Plan (ABP) is designed to be a flexible payment plan similar to revolving charge accounts offered by many national credit card companies. All policies currently in Personal Lines accounts will automatically have combined billings. A down payment of only one month premium is required. The policyholder can pay the minimum amount or the entire term premium, or anything in between. Any activity to a policy within the account will cause the minimum to be adjusted. There is a $5.00 transaction fee applied per account unless payment is made by Electronic Funds Transfer. Any number of Personal Lines policies, including Life insurance policies with EMC National Life Company can be included together with one convenient bill under this plan. Policies do not need to have the same terms or effective dates. As a direct bill plan, the policyholders receive a Billing Account Notice directly from EMC in a separate mailing from the Declarations Page. This Notice indicates which policies are included in the Account Billing Plan, any transactions applicable, the amount due and the due date. To assist agents in monitoring clients accounts, EMC will send a daily Past Due List which is a notification of any insured who has failed to make his/her premium payment by the date indicated on the Billing Account Notice. 2. GETTING STARTED WITH ABP All policies in the same Personal Lines account are automatically combined into one billing account. In addition, agents may request in writing to combine multiple Personal Lines accounts and even EMC National Life policies onto one Billing Account. For example, the following will automatically be combined onto one bill: Billing Account Account Name Personal Policy Account Number 12X-34-56 John Doe Jr. 12X-34-56 Policy Numbers 12A-34-56 12H-34-56 Homeowners If the insured requests, other Personal Policy accounts or EMC National Life policies can be added to the same billing account. 3. FULL PAY ACCOUNTS Full payment will be required if: A. Notice of insufficient funds (INSF) received; or B. Habitually late or insufficient payment of prior billing notices, or C. The policy is escrow billed. 4. MONTHLY PAY ACCOUNTS The minimum due will be: A. Account balance if less than $25 plus transaction fee. B. The sum of current account balance divided by the number of months left in the term, and C. fee is applied to all accounts except those paid by Electronic Funds Transfer. 5. NEW BUSINESS If direct bill is indicated as the preferred billing method, only a one month down payment will be required for all new business applications. If no money is included with the application (no cash), the new business will be issued on an agency bill basis. The policy will convert to direct bill at the first transaction or renewal. (Exception: if the application is processed at least 30 days prior to the effective date of coverage, a bill for the down payment will be sent to the insured or escrow provider at the agent s request.) If the new business is issued after the effective date, the down payment required will be two months premium plus one months premium for each billing missed. 6. COMMISSION All commission is paid after sufficient payment is applied to the billing account, regardless of payment method selected. 7. POLICY CHANGES Policy change premium is applied to the remaining account balance for the current policy term. If the policy change creates an account credit balance, a refund check will be issued. If a policy is amended and it results in a minimal premium adjustment of $5.00 or less, the amount may be waived, except that the actual return premium shall be returned at the request of the insured. 8. BILLING INFORMATION Billing Account Notices are issued at least 20 days before the due date. These notices are mailed directly from EMC and are a separate mailing from the Declarations Page. The effective date of the initial policy issued is the account billing due date; however, the insured may select to change this due date by up to 7 days. This will then become the new due date for all policies in the account. PP-2

Late Payments Payments received after the due date: 1. Late fee of $10.00 applied (subject to individual statute); and 2. Fee is added to next billing notice; and 3. Subsequent funds received for billing notice will first be applied to the outstanding late fee; and 4. EMC sends a daily "Past Due List" to the agent which is notification of any insured who has failed to make his/her premium payment by the due date. C. Insufficient Funds A charge of $25.00 (subject to individual statute) will be made for each transaction returned as "insufficient funds" (NSF). PP-3
