Plans are underway for the marching band to perform in Walt Disney World! In this packet you will find the payment schedule, trip itinerary, student contract, and chaperone interest form. With all of our past trips, we have taken the total trip cost and split them into 5 payments over a 7 month period. You will need to strictly adhere to the payment schedule in order to keep each student s trip on schedule and as planned. As always, we offer a reduced rate for chaperones. I know many parents have given me a verbal commitment on chaperoning, but to be fair, I must ask that we take chaperones on a first-come, first-served basis determined by the return of your Chaperone Interest Form (attached). At this time we are looking at needing 8-10 chaperones for the trip. Please read the Chaperone Interest Form and Policies. Beginning in July we will offer several fundraising opportunities so that every student can defray some of the costs associated with this trip. We will outline the fundraising opportunities for this trip at the May 22nd Marching Band Rehearsal. Students can also use any student credits earned from past years as well. For Students, please return the attached participation form with your non-refundable down payment of $175 with the signed trip contract to Mr. Irwin, Mrs. Giotta, or Mrs. Laux by July 27, 2018. All checks are payable to PLIMPA. You may also mail these to the PLIMPA PO Box (PLIMPA,, Canton, OH, 44711-8748). Credit card payments can also be made on the Payments tab at GOHSBAND.COM. Updated student credit amounts will be posted in the band room in May. You may use student credits for any payments. Please indicate this on the payment forms. All five payments must be made by the deadlines listed below. PAYMENT SCHEDULE Student Payment Schedule $875.00 Total (all students are in 4-person rooms) Chaperone Double Room Payment Schedule $875.00 Total Chaperone prices are based on two-persons per room. Student credits cannot be used for chaperones

If paying by Credit Card, please indicate that above and turn-in this form. If using Student Credits to pay all/part of your deposit, please indicate amount: $ Reservation/Trip Policy When reading the following trip policy, please understand that this trip is completely optional for students and chaperones, therefore we will strictly adhere to the following protocol: A non-refundable payment of $175 is due with this reservation form by July 27, 2018. All prices for students are based upon being housed in quads (4 per room). Room sign-up will occur in December 2018. Every participant must make the required payments on time. Should there be a delay, Mr. Irwin must be made aware IN WRITING before the payment is due. Without your written notification of a late payment prior to the payment deadline, your spot will be released to someone on the waiting list and you will forfeit any payments made per the protocol detailed in the Individual Cancellation Policy below. At no time can students be more than 1 payment behind schedule and all payments must be made by February 1, 2019. PLIMPA is contractually obligated to make payments to outside vendors by certain deadlines, therefore you must abide by the following payment schedule: Individual Cancellation Policy: Any student or chaperone may cancel their trip prior to January 3, 2019 and receive a refund of any payments made MINUS the $175 non-refundable down payment plus out-of-pocket expenses as determined by our tour company. You must notify Mr. Irwin in writing prior to January 3, 2019. Cancellations after January 3, 2019 will not receive a refund of any kind and will incur a additional $50 cancellation fee. Any student finding themselves in serious disciplinary action throughout the school year may find their trip privileges revoked. This decision will be made after consultation with GlenOak HS administrators. The refund protocol detailed in the Individual Cancellation Policy will apply should the student find themselves in these circumstances. Discipline: All Plain Local Schools/GlenOak High School codes of conduct policies apply on this trip. Any student in violation of this code may be sent home immediately at the cost of his/her family. We reserve the right to request a parent to pick-up the child should staff and administrators determine the disciplinary infraction is serious enough for immediate removal. Upon return, the student will be dealt with by the Plain Local Schools administration. Friday, April 5, 2019 Cocoa Beach, Florida 7:00-9:00 AM $ Breakfast on your own en route at stop TBD. 12:00 PM Arrive at Cocoa Beach Boxed Lunch Provided Students will have a couple hours at the beach before checking into hotel. 3:00-4:00 PM Check-in: Students will stay at the Doubletree by Hilton Oceanfront Cocoa Beach. 2019 Florida Spring Band Trip Chaperone Interest Form Chaperone Name: Email address: Band Have you chaperoned band students before? Yes No Dear interested chaperone(s), Thank you very much for your interest in chaperoning band students on our planned trip to Florida in April 2019. Because of the responsibility associated with chaperoning all band students, not just your own, we must carefully review all chaperone applications. Applications will be considered in the order in which they are received with the deadline to submit this application being May 25, 2018. We do give more consideration to those parents who have chaperoned before. We will contact you by June 4, 2018 to let you know whether you will be chaperoning the trip or if you have been placed on a waiting list pending trip openings. Again, do not send a payment with this application. Please understand that chaperoning is an important job and this is not a vacation. You will need to follow our itinerary and assist throughout the trip with the needs of the band. For the safety of all students and the success of this trip, we will strictly adhere to the Chaperone Reservation/Trip Policy below. Please take a moment and review this policy. All chaperones must pass a FBI/BCI criminal background check per Plain Local Schools policy. All prices for chaperones are based upon being housed in doubles (2 per room). Room sign-up will occur in December. Because of your chaperoning responsibilities during the trip, we have reduced the cost of your trip to reflect the value we place on your commitment to helping our students. The cost of a Double Room is considerably higher than the Quad price the students are given. If you would like a single room, please let Mr. Irwin know. The cost for a single room is $1100.00 and will change the scheduled payment amounts. We rely on the professionalism of our parent chaperones, all payments must be made on time. If chaperone payments are late, we will release you from participating in the trip. Should this be necessary, we will follow the protocol in the Individual Cancellation Policy below regarding the refunding of any payments.
