Your benefits could help your family pay for: Outstanding debt Burial expenses Medical bills Your children s education Daily expenses To estimate your insurance needs, you ll need to consider your unique circumstances. Use our online calculator at www.standard.com/life/needs. 3

How Much Your Costs Because this insurance is offered through National Hospitality Services LLC, you ll have access to competitive group rates, which may be more affordable than those available through individual insurance. You ll also have the convenience of having your premium deducted directly from your paycheck. How much your premium costs depends on a number of factors, such as your age and the benefit amount. Use this formula to calculate your premium payment: 1000 = x = Enter the amount of coverage you are requesting (see benefit amounts in the About This section). Enter your rate from the rate table. This amount is an estimate of how much you would pay each month. If you buy coverage for your spouse, your monthly rate is shown in the table below. Use the same formula to calculate the premium that you used for yourself, but use your spouse s age and your spouse s rate. If you buy Life coverage for your child(ren), your monthly rate is $0.180 per $1,000, no matter how many children you re covering. If you elect AD&D insurance with your Dependents Life insurance for your child(ren), your child(ren) s monthly AD&D rate is $0.02 per $1,000 added to the above rate.
