The CNS Benefits Prgram and certain benefit plans, as described in this Summary Plan Descriptin (SPD), cmmnly referred t as the Bk f Benefits, are intended t cnfrm t all applicable legal requirements including, but nt limited t, the Emplyee Retirement Incme Security Act f 1974 (ERISA), the Internal Revenue Cde f 1986 (the Cde ), and all regulatins issued thereunder. It is further intended that the Prgram meet the requirements f a Cafeteria Plan under Cde Sectin 125 and Treasury Regulatins thereunder. If the emplyee elects qualified benefits under the Prgram, thse benefits may be eligible fr exclusin frm the emplyee s incme under Cde Sectin 125(a). Dependents Yu may chse t cver yur Eligible Dependents fr medical (including prescriptin drug), dental, visin, life insurance, and special accident insurance cverage. Yur Eligible Dependents may als use the emplyee assistance prgram. Yu are bligated t submit prf f dependent status fr yur Spuse and Children. Dcumentatin includes a birth certificate, Scial Security card, marriage certificate, and ther dcuments required t prve eligibility. Medical (including prescriptin drugs), dental, and visin cverage may be cntinued fr an unmarried Child wh is permanently and ttally disabled and incapable f self-supprt due t a physical r mental handicap that began befre he r she reached the age at which cverage wuld therwise be terminated. Yu must submit prf f the Child s disability t the claims administratr within 31 days after attaining the maximum age. Additinal prf f the Child s cntinuing disability will be required peridically. Special accident insurance cverage (ATLC, IGUA, PGU and USW bargaining unit emplyees nly) may be cntinued indefinitely fr an unmarried Child wh is incapable f self-supprt due t a physical r mental handicap that began befre he r she reached age 19.
