1 Account applications for new SunWise Elite TFSA contracts All clients who wish to open a TFSA must complete a new SunWise Elite (Including SunWise Elite Plus) Individual Variable Annuity Contract Application Form. The form used will depend upon how the contract will be held. The original client signed application must be delivered to the CI office.
