Are You a Parent or Guardian of a Person with ASD? Or Do You Care For a Person with ASD? If so, would you be protected by insurance in the event of a member of the public holding you responsible for bodily injury or damage to their property caused by a person with ASD in your care? In today s climate, the possibility of being sued for negligence is increasing, with legal costs, expenses and awards continuing to escalate. You may find that your household contents insurance provides this cover. Where this is not the case, we are pleased to be able to offer cover designed exclusively for members of the National Autistic Society to protect them against these eventualities. This insurance facility has been negotiated for the National Autistic Society members by Aon Ltd and is underwritten by Liberty Mutual Insurance Europe Limited ( Liberty ). Personal Liability Insurance Summary of Cover This information is a summary of the cover provided by the Liberty Personal Liability Insurance facility for members of the National Autistic Society. The facility provides personal liability insurance for parents, guardians of or persons caring for people with ASD. The summary highlights the main features and benefits and significant exclusions of the cover. A significant exclusion is something that may affect your decision as to whether the cover is suitable for you; it may also be unusual compared to products offered by other companies. These exclusions are marked with an X under significant exclusions and limitations throughout. In addition there are Conditions that apply to the cover detailed in the certificate. For example you have a duty to take reasonable steps to prevent any circumstances where liability may occur. It is important to note that the summary does not contain all the terms and conditions that may apply, this information can be found in the certificate and policy document which is issued to you when you take out this cover. A copy is attached. MAIN FEATURES, BENEFITS AND EXCLUSIONS Features and/or Benefits Personal liability for accidental bodily injury to third parties or damage to their property arising out of your care control and supervision of persons with autism within the United Kingdom, Channel Islands, and the Isle of Man and for up to 60 days in any one period of insurance anywhere in the world Exclusions and Limitations Limit 1,000,000 any one claim Legal costs and expenses are payable in addition (except for any claim arising in USA/Canada) X Excludes terrorism related incidents X Excludes fines or penalties of any kind X Excludes any personal liability otherwise insured (you may be covered under your household contents insurance) Excess Excludes the first 100 of claims for third party property damage How long does the insurance cover me for? The insurance is annually renewable on the 1st November. The first period of insurance may not be for 12 months; the certificate will detail the period of cover provided. You will receive notice from us that your insurance is approaching renewal at least 21 days before the due date. What if I need to make a claim? If after insuring with us and receiving the full written cover details, you subsequently change your mind, you have 14 days to write to Liberty confirming that you do not wish to continue. No charge will be made and any premium you have already paid will be refunded. If you do not cancel the insurance within the 14-day cooling-off period mentioned above, you are committed to pay the premium. However, you can still cancel the insurance providing you give Liberty notice in writing. As long as you have not made a claim, you will receive a refund of the part of your premium which covers the cancelled period, providing this exceeds 10. If you have made a claim then the full annual premium is due. Liberty have the right to cancel the cover by giving you 7 days notice in writing sent by recorded delivery to your last known address. If Liberty cancel the insurance they will refund the part of your premium which covers the cancelled period. What is the cost? The annual charge is 30.00 (inclusive of Insurance Premium Tax) per policy. Annual Renewal Date: 1st November each year. If you join after 1st May, the cost is 20.00 (inclusive of Insurance Premium Tax) per policy. NB: The premium remains the same in the event of more than one person with ASD being cared for. 2 5

INSURER COMPLAINTS PROCEDURE Liberty is dedicated to providing you with a high quality service and want to maintain this at all times. Liberty values the opportunity to investigate any concerns you may have about any aspect of our service and are committed to handling all complaints fairly, thoroughly and promptly. Who to contact Many concerns can be resolved straight away. Therefore, in the first instance please get in touch with National Autistic Society as they will generally be able to provide you with an immediate response to your satisfaction. Where a complaint cannot be resolved straight away Liberty aims to resolve your concern as soon as possible and keep you informed of progress while enquiries continue. If you remain dissatisfied once you have received Liberty s response to your complaint Liberty will escalate your complaint and conduct a separate review. This will be concluded with the issue of Liberty s final decision in writing N.B. Full details of Liberty Mutual Insurance Europe Limited including contact details are stated in the Schedule of this Policy The Financial Ombudsman Service (FOS) If following receipt of our final response letter you remain dissatisfied you may be able to ask the FOS to formally review your case. The FOS can help with most complaints if you are: a. a private individual b. a business with an annual turnover of less than 1,000,000 c. a charity with an annual turnover of less than 1,000,000 d. a trustee of a trust with a net asset value of less than 1,000,000 You must contact the FOS within six months of our final response. If you are unsure whether the FOS will look at your complaint please contact them directly for further information. You are entitled to contact the FOS at any stage of your complaint. Please note that the National Autistic Society will need to be in receipt of your completed application form via post or email (membership@nas.org.uk) before taking payment. You shall include, as relevant, individual insured customers (including prospective customers) such as sole traders, a company s owners, partners and/or any individual who may be covered by an insurance policy with Liberty. In summary, this includes but is not limited to managing and handling your insurance queries, underwriting and claims handling. We may use an anonymised form of it for various purposes. We may check your information against public sources (such as the electoral roll and court judgments bankruptcy or repossession and other insurance industry databases) and this may involve carrying out bankruptcy/judgment debt checks, fraud prevention, anti-money laundering and counter terrorism financing checks, subject to data protection law and with your consent if required. We may directly or indirectly share your information with other insurers, the police and other law-enforcement agencies, the Claims and Underwriting Exchange Register and the Motor Insurance Anti-Fraud and Theft Register, government agencies and/or regulatory authorities, as applicable. In addition, we may pass your information and claims history to agents, service providers, other Liberty Mutual Group companies, regulatory organisations, and to certain other third parties. By going ahead with a Liberty insurance quote or policy or by giving us your personal information including sensitive personal information (as more particularly described in your Policy) you will be explicitly agreeing to us, our agents, other insurers and the other persons described in this notice (and/or in your Policy) processing it for all relevant purposes. Before you provide us with any information about others, you should make sure they are aware of how we will use their information and have agreed to this.
