Subscriptions to the child s Stocks & Shares Junior ISA The maximum anyone can subscribe to a Junior ISA in the 2017/18 tax year is 4,128. Please see the Junior ISA Key Facts document for more information. If Direct Debits are being made by a third party (e.g. a parent) that person must complete and sign the Direct Debit Instruction form. Please make sure it is you or the parent/legal guardian who submits all the documentation to us, as it is you or the parent/legal guardian who takes responsibility for you or the child s Junior ISA and the subscriptions made to it. You can make additional subscriptions by cheque at any time during the year, subject to the limit stated above. If you have decided that you wish to invest the subscriptions stated below, please complete the investment instructions in section 7. However, it is not compulsory to complete these sections to open the Account. If you have not yet decided which investments you would like to make, you may leave this section blank and your cash will be placed on deposit until we receive further instructions from you. If you wait until you have your Personal ID and Password and make your investment instructions online, our online dealing fees will apply. Please note if you fail to pay charges related to your Junior ISA we reserve the right to disinvest in order to pay any outstanding charges. If your chosen investment is not available in the Junior ISA or if your instruction is unclear, we will allocate your funds to the Deposit Account. Notes 1. Investment: Enter the full name and class of each investment you wish to make. This is important as there may be different classes available. Please also quote the MEX Code which you can find in the relevant investment choice list available within the Forms & Documents section at www.alliancetrustsavings.co.uk To allocate your cash to your Deposit Account or Online Dealing Account, enter deposit or online. 2. Payment Amount: Lump Sum please enter the amount you wish to allocate to each investment. Direct Debits please indicate the percentage you wish to allocate to each investment and ensure they add up to 100%. 3. Income options: Any income received from each investment will automatically be used to purchase more of the same investment when the Minimum Purchase Level (see below) is reached, unless you wish to: Use it to purchase another investment enter name and MEX code Direct it to the Deposit Account enter deposit Direct it to the Online Dealing Account enter online 4. How we will place your order: We will carry out your investment instructions once the Account is opened. We may buy your investments for you along with other client instructions as part of the same deal. We will take all reasonable steps to ensure that the purchase is dealt on the best terms generally available in the market for transactions of a similar size and nature at the time of execution, as described in our Order Handling Policy. This may sometimes work to your advantage or disadvantage. Minimum Purchase Levels When cash in the Security Account reaches the Minimum Purchase Level, we will automatically purchase more of the same investments for you. The default level is 100 but if you wish to select a different amount please tick below. The level you select will apply to all investments in this Junior ISA. You can change the levels at any time by completing a Junior ISA Instruction form. Minimum Purchase Levels: 150 300 500 Purchasing funds If you wish to invest in a fund (an OEIC, ETF or unit trust) you must confirm that you have read an up-to-date version of the Key Investor Information Document (KIID) for each fund in which you want to invest, as well as the Supplementary Information Document (SID) and in addition, confirm that you are not a US Person or applying on behalf of a US Person. Copies of the fund KIIDs and SIDs can be found in the Investment Selector/Investment Information section of our website at www.alliancetrustsavings.co.uk or are available from the fund provider directly. 7. Declaration by Alliance Trust Savings We declare to you that the following statements are true: The Stocks & Shares Junior ISA and the rights under it will be governed by the details on the application as accepted by us, and by our Terms and Conditions documents referred to in it (the Governing Documents ) as amended by us from time to time We will hold cash subscriptions, Stocks & Shares ISA eligible investments, interest, dividends and any other rights or proceeds in respect of those investments, and any other cash in the Stocks & Shares Junior ISA Under the Data Protection Act 1998, we are a data controller and will process data about you and the named child in performance of our duties as an ISA manager We are an ISA manager authorised by Her Majesty s Revenue & Customs We are authorised by the Prudential Regulation Authority and regulated by the Financial Conduct Authority and the Prudential Regulation Authority. 8. Client declaration and signature I apply to open and subscribe to a Stocks & Shares Junior ISA Account for the named child in the application for the tax year in which the application is submitted and each subsequent year until further notice. I confirm that I have been given the opportunity to review the Terms and Conditions and documents referred to in it (the Governing Documents ). I confirm I understand and will tell or have told any subscriber known to me to the Junior ISA established from this application that all subscriptions made to that Junior ISA and investment return thereon belong to the named child, and the Junior ISA applied for will be held by the named child immediately following the opening of the Account. I confirm that I understand that you will not normally contact them about the Junior ISA until they are a registered contact, or have reached 18 years of age. I confirm that I understand if for any reason the transfer from a Child Trust Fund is not successful, any Junior ISA that has been opened on a provisional basis to accept the transfer will be invalid, and any subscriptions to the provisional Junior ISA will be returned. I declare that in relation to each Stocks & Shares Junior ISA application effected by me in this application process is made by me and I am either over 16 and under 18 and will hold the account for me, or over 18 and I am the parent of or have parental responsibility for the child named in the application process. I declare that (a) The application is made to open a Stocks & Shares Junior ISA Account for the child named in the application; (b) I understand that if the named child holds a Child Trust Fund, it must be transferred in full as part of this application; (c) The child who will hold the account opened because of the application is (i) Resident in the United Kingdom, and/or (ii) A person who has general earnings from overseas Crown employment subject to United Kingdom tax within the meaning given by section 28 of Income Tax (Earnings and Pensions) Act 2003, and/or (iii) Married to or in a civil partnership with a person mentioned in paragraph (ii), or (iv) A dependant of a person mentioned in paragraph (ii); (d) I will be the registered contact in relation to the Account until such time as I may give up that position and another take over; (e) I have not subscribed, and will not subscribe, to any other Stocks & Shares account that is a Junior ISA account held by the same named child, and am not aware of any other Stocks & Shares account held by the named child that is a Junior ISA account; (f) As far as I am aware, (after properly examining my affairs) I have not subscribed amounts to the Account (or any other account) that, when aggregated with other subscriptions, exceed the overall subscription limit in regulation 4ZB(1) of the ISA Regulations (subscriptions to accounts opened in accordance with this regulation) in relation to the named child who will hold the account opened because of the application in the year in which the Junior ISA application is made; and (g) I will not knowingly subscribe amounts to the Account that, when aggregated with other subscriptions, exceed the overall subscription limit in regulation 4ZB(1) of the ISA Regulations in each successive year following the year in which the Junior ISA application is made. I declare that: I and the named child will be bound by the Governing Documents as they apply to Stocks & Shares Junior ISA the information given by me in this application is true and correct to the best of my knowledge and belief I and the named child are not a US person, and I and the named child undertake to inform you without delay of any changes to the particulars given in this application and any other changes which may affect the ability to hold or subscribe to a Stocks & Shares Junior ISA or a Stocks & Shares ISA. I and the named child will inform you if my circumstances change so that any one or more of the declarations made no longer apply to me or the named child Any cash held in the Stocks & Shares Junior ISA will be held only for the purpose of investment in qualifying investments for the purposes of the ISA regulations I have told or will tell the named child about this Junior ISA I authorise you (on behalf of the child who holds the account where appropriate) (a) To hold the subscriptions, account investments, interest, dividends and any other rights or proceeds in respect of those investments and cash (b) To make on behalf of the named child any claim to relief from tax in respect of account investments (c) To make a written record of this application and declaration if appropriate (d) Create additional Accounts for me or the named child as appropriate for the delivery of Services or receiving payments using the details contained in this application, as updated by me as appropriate (e) On my authenticated request, subject to the Governing Documents, transfer or pay to the named child, as the case may be, investments, interest, dividends, rights or proceeds in respect of those investments or any cash (f) If I do not already have a Personal ID and Password, issue me with one on acceptance of this application so that I may use the Services that need my authentication (g) Act on instructions given under that Personal ID and Password or replacement authentication for any of your Services that need prior authentication. I am aware why my and the named child s information and data is collected by you, the length of time it will be retained and that it may be passed to others, all as described in the Governing Documents. You are authorised to: Contact me by email, telephone or post to provide me with information on your products, services and events and those from companies within the Alliance Trust group, and Transfer my information to other companies within the Alliance Trust group and I agree that they may contact me too until I write or email your Client Services Team requesting you or them to stop. By submitting this application, I consent to the use and processing of my and the named child s information and data as set out in the Governing Documents. It is a serious offence to make any false statements and to do so can lead to prosecution, civil action, deductions from the Junior ISA Account or closure of your Account.
