This product is a life insurance policy that accelerates the death benefit on account of chronic illness, and is not a health insurance policy providing long term care insurance subject to the minimum requirements of New York Law, does not qualify for the New York State Long Term Care Partnership program and is not a Medicare supplement policy. For Guarantee Advantage UL and the Legacy Advantage SUL, the policy s five-year no-lapse guarantee remains in force provided required premiums are paid and certain other requirements described in the policy are met. If the guarantee terminates, the policy will stay in force for as long as its cash surrender value is sufficient to pay the monthly deductions when due. If the cash surrender value is insufficient to pay the monthly deductions and the required premium is not received by the end of the grace period provided in the policy, the policy will lapse and terminate without any cash surrender value. For MetLife Provider UL, the Coverage Continuation Benefit (CCB) guarantees that the policy will remain in force, as long as the required premiums are paid, until the end of the selected guarantee period, but no later than age 95.
