Submit your completed and original application form DO T FAX or EMAIL 2. Submit the Payment Remittance form and the non-refundable application fee of $250.00 + HST 3. Submit a copy of the following documents: Issued Articles of Incorporation Issued Articles of Amendment (if any) Issued Articles of Continuance (if any) Issued Articles of Amalgamation (if any) TE: The articles must restrict both the business of the corporation and the ownership of shares of the corporation in accordance with the Business Corporations Act and the Law Society Act. The Law Society of Upper Canada may investigate or verify any information supplied in this application, and may require further information from you before the application is approved. Omissions or inaccuracies in responses may delay processing. If the space provided for any answer is insufficient, attach a separate sheet that is signed and dated by the applicant and staple it to the application.
