Employee Self-Service (ESS) Online Health Care Enrollment due to a Qualifying Life Event The following presentation will give you instructions on completing your online health care elections changes using the district s self-service portal. From the School District website, select Staff from the menu bar. Then select Employee Self Service from the dropdown menu bar. 2
This is the ESS home screen. Here you will want to select Log In from the menu bar. 3
Once logged in, you will want to select Benefits from the menu bar. 4
Select Report/View Life Events. 5
Select the type of qualifying event and effective date of the event. Then select Submit. 6
You will need to submit supporting documentation to HR. Once it is received, HR will approve your life event request and you may make election changes. Please call 452-2000 x11382 or x11311 if you have questions about what is needed as supporting documentation. 7
Here are the elections you may make: Health Insurance This is the medical plan you may choose. You may only waive if you are a part-time employee or a full-time employee with other coverage. Full-time employees are required to submit a waiver form to HR if not electing the School District Health Insurance. Dental Vision and Audio (DVA) This election may be made alone or with a health insurance plan. You must make an election (you may make an election to waive this coverage). Optional Benefits The medical and dependent care FSA may be elected with any health insurance or if health insurance is waived. This election may be declined. The HSA may only be elected with Plan B Health Insurance (may not elect a medical FSA in the same calendar year) This election may be declined. The HRA may only be elected with Plan C Health Insurance. This election may be declined. Select Make New Election or Decline Benefit for each section 8
Here is a snap shot of your health insurance elections. Select a radio button to make your election Please note the tier choice when making your election: Employee Only Employee + Spouse Employee +Child(ren) Employee + Family Part Time Plan B Employee & Children Also note the employment status option for part-time employees Part Time Plan C Employee & Spouse *If you are a grand-fathered employee, you will also see Plan A as an option. 9
If electing a choice requiring a dependent, select Add coverage or Add new dependent. Current dependents will show in the drop down box. You must choose or add each dependent you want coverage on. 10
Fill in dependent information. SSN is necessary for tax reporting purposes (1095C reporting). Select OK when complete. *You will be able to enter additional dependents by selecting Add new dependent from previous screen. 11
Select Continue when finished making an election and adding dependents. 12
Select a radio button for your Dental, Vision, and Audio (DVA) coverage. Note tier levels Note the employment status option for part-time employees DVA is for those electing Plan A or waiving health insurance. There is a cost associated with this election. DVA0 With Plan B or C Employee & Family Part Time DVA0 Employee & Spouse DVA0 is for those electing Plan B or C health insurance coverage. There is no cost associated with this election. If adding a dependent already added to your health election, select Add Coverage and choose the dependent s name If adding a dependent not added to your health election, select Add new dependent 13

The Health Reimbursement Arrangement (HRA) is shown here. This is an employer only contribution for those electing Plan C. The amount contributed is prorated quarterly based on the date of hire. *Part time employees will receive ½ of the entitlement. Either select or decline this benefit Select Continue when finished 16
You will only see your pay period election displayed on the benefits screen. We do not have the annual election amount turned on to display as our deductions are taken out in 19 pay periods and the system can only display the deductions for 26 pay periods. 17
If you do not see the option to continue, you may have forgotten to make an election. You will not be able to submit your elections until all sections have an election or been declined. 18
Once you have reviewed your elections and submit, you will see a confirmation message. You may make changes up to your 30 day deadline or until your elections have been approved by the administrator. 19
