MU Cre 14 Medicatin Recnciliatin This guide will prvide instructins n hw t cnfigure the necessary setup and preferences t enable Enterprise EHR t wrk fr Cre Measure 14: The Eligible Prfessinal (EP) wh receives a patient frm anther setting f care, prvider f care, r believes an encunter is relevant shuld perfrm medicatin recnciliatin. Final Rule Requirements The Eligible Prfessinal (EP) wh perfrms medicatin recnciliatin fr mre than 50% f transitins f care (TOC) in which the patient is transitined int the care f the EP. Cre Objective: Menu Objective: Numeratr: The number f transitins f care in the denminatr where medicatin recnciliatin was perfrmed. Denminatr: The number f transitins f care during the EHR reprting perid fr which the EP was the receiving part f the transitin. Exclusin: Any EP wh was nt the recipient f any transitins f care during the EHR reprting perid. If this preference is set t N fr the system, then the TOC and SOC clumn is nt viewable and the prvider is unable t select the checkbx. This checkbx allws the user t dcument frm the Daily Schedule if the appintment is cnsidered an inbund TOC and if a SOC was received fr the patient frm the transitining prvider r facility. New Patient r Cnsult) yu can administratively flag thse t default t TOC 1. Navigate t TWAdmin Dictinaries Appintment Type.

This setting is nly available fr any encunter types that the rganizatin has created (nt the system delivered encunter types).
Set the Shw Recncile ptin as required in the Encunter Type t highlight when t perfrm Medicatin and Allergy Recnciliatin. When checked, the recncile butt n will be highlighted yellw fr the type f encunter. This ptin is disabled fr enfrced Encunter Types, including appintments and is nly available t change fr any encunter types that the rganizatin has created. Wrkflw Cnsideratins If needed, indicate the encunter as TOC by clicking n the Transitin f Care checkbx within the Encunter Summary r clicking the TC checkbx n the Daily r Prvider Schedule. Ensure that users understand when t click this checkbx. Identify which rle wuld click the TOC checkbx.
Navigate t the patient s Clinical Desktp. Click n the Meds cmpnent n the Clinical Desktp. Review the medicatin list with the patient. If needed, add any missing medicatins that the patient reprts utilizing Recrd withut Ordering. Click the Med Recnciliatin buttn. If the Medicatin Recnciliatin is cmpleted fr the encunter, a v10 r v11 nte assciated t the encunter will cite the date/time infrmatin int that nte s Current Medicatin sectin. Fr TOC encunters, dcument whether the Summary f Care dcument is received using the Summary f Care Received checkbx n the Encunter Summary r n the Daily/Prvider Schedule. Fr existing patients, a TOC encunter with n summary f care dcument is nt cunted in denminatr.
Hw EEHR Calculates the MU2 Reprt Numeratr: Fr each unique Transitin f Care Encunter in the denminatr, cunt unce: Denminatr: Medicatin Recnciliatin tk place (determined by Recnciled Date ) during the TOC Encunter. The system will cunt nce, each Transitin f Care Encunters seen by the eligible prfessinal (EP) during the EHR reprting perid fr existing patients where a Summary f Care dcument was prvided by the transitining prvider. This is indicated by a Summary f Care CCDA dcument in the patient s ChartViewer r the encunter has the Summary f Care Received flag checked AND The system will cunt nce, each Transitin f Care Encunters seen by the eligible prfessinal (EP) during the EHR reprting perid fr new patients. Exclusin: The system will cunt nce, each Transitin f Care Encunters seen by the eligible prfessinal (EP) during the EHR reprting perid. If the number is zer, the prvider will be excluded frm this bjective. Indicatrs: When the numeratr divided by denminatr is 50% r greater When the numeratr divided by denminatr is between 40% and 50% When the numeratr divided by denminatr is less than 40%
Calculatin Flw Diagram General Cnsideratins Medicatin recnciliatin is described as the prcess f identifying the mst accurate list f all medicatins that the patient is taking, including name, dsage, frequency and rute, by cmparing the medical recrd t an external list f medicatins btained frm a patient, hspital r ther prvider. The EP, per CMS, is nt required t d the actual med recnciliatin. If it is perfrmed by ther clinical staff, that is acceptable. Transitin f care is clarified as the mvement f a patient frm ne setting f care (hspital, ambulatry primary care practice, ambulatry, specialty care practice, lng-term care, hme health, rehabilitatin facility) t anther. We als clarify that the receiving eligible hspital r EP wuld cnduct the medicatin recnciliatin. At a minimum, transitins f care include first encunters with a new patient and encunters with existing patients where a summary f care recrd (f any type) is prvided t the receiving prvider. The summary f care recrd can be prvided either by the patient r by the referring/transiting prvider r institutin. A transitin f care encunter is determined by the Transitin f Care checkbx n the appintment type, if the encunter type is Appintment, frm the Encunter Type dictinary. If this value is checked n the schedule r in the Encunter Summary windw, the encunter will be included in the denminatr. If this value is unchecked n the schedule r in the Encunter Summary, the encunter will nt be included in the denminatr.
The default value fr the Transitin f Care prperty fr each encunter is determined using the Transitin f Care field in the encunter type dictin ary, based n the encunter type f the encunter being created. There is ne imprtant exceptin t this. If the encunter type is Appintment then the appintment type assciated with that encunter is used t determine the default value fr Transitin f Care prperty. This is dne based n the Transitin f Care field in the appintment type dictinary. Currently all appintments will indicate the user must recncile medicatins fr all appintment types. Hwever, Meaningful Use reprting will nly cnsider this required fr appintment types flagged as transitins f care. An existing patient is defined as a patient that has a previus valid encunter with the prvider.
