Lending Electronic Modules COM, CONCU, FCM, GSM, HEM, HIM, LEM, MHM, MVM, REMCU, TPM LENDING MULTISTATE FORMS UCA Universal Credit Application Minor revisions are being made to the UCA to revise the pronouns in 2 sentences to improve clarity. In section 1. Type of Application - Individual Credit, the word "my" is being replaced with "your". In section 2. Type of Requested Credit - Proceeds of Credit to Be Used for, the word "my" will be replaced with "your". The first five asset fields on Page 2 will be updated to remove the hard-coded captions and to update the field tags to make them more generic - allowing the creditor and consumer to populate the asset fields in a more consistent manner. The consent authorization for contacting the consumer by telephone will be updated to clarify the language and extend the authorization to any telephone numbers that the consumer subsequently provides to the creditor. We are also correcting two field names that were reversed on page 1 section 2. We are changing field 116 from Application.Secured.Yes to Application.Secured.No and field 117 from: Application.Secured.No to: Application.Secured.Yes. LENDING STATE-SPECIFIC FORMS Arizona MV-1-AZ Title and Registration Application The Motor Vehicle Division of Arizona has updated their Title and Registration Application (form 96-0236) with a revision date of R09/12. We are making changes to our form MV-1-AZ to match the states changes. They have changed "Legal Status (if "or", sign here)" to "Legal Status (If "Or" is checked here or above, owner must sign here)." Other minor changes will be made to the document. Idaho New VMP4041ID Discharge of Deed of Trust We discontinued the SAT-DT-ID and replaced it with the VMP4041ID. The VMP4041ID evidences full payment of a loan under a Deed of Trust. Code 24-4.5-2-203.5(5) for retail contract late charge maximum dollar amounts. This year, the maximum late charge amount for retail sales contracts increased from $17.50 to $18.00 effective July 1, 2012. This authority applies to both motor vehicle and goods and services retail sales. Our motor vehicle retail contracts with LF and LFA in the form names hardcode the amount of the late charge. The dollar amount in the Late Charge provision (in the Fed Box) is being revised in these forms to use the new $18.00 maximum figure. As an enhancement, we are also revising the first bulleted paragraph in the Remedies section of the LF and LFA motor vehicle retail contracts to strengthen and clarify the creditor s right to require immediate payment on default. No other changes or enhancements are being made. The changes will not add or remove any completion fields. These changes are not legally required (creditors can charge less than the maximum amount allowed for late charges), but we strongly encourage customers to timely update to the new revision. RSSIGSLFLZIN - Retail Sales Simple Interest Goods and Services Late Fee RSSIGSLFAZIN - Retail Sales Simple Interest Goods and Services Late Fee with Arbitration On even numbered years, the Indiana Department of Financial Institutions publishes dollar amount adjustments to Ind. Kentucky MV-1-KY- Application for Kentucky Certificate of Title or Registration The state of Kentucky has updated their Application for Kentucky Certificate of Title or Registration (form TC 96-182) with a revision date of 11/2011. We are updating our forms MV-1-KY to match the states changes. In the "JOINT OWNERSHIP" section, they have added to the "S.S. #" field. It now reads "S.S #, KyDL#, or Govt. issued #." A field will also be added for "PHONE NO." Other minor changes will be made to the document. Maryland VMP1034MD Affidavit of Refinancing Exemption WKFS will be adding to our Eforms Library. the VMP1034MD to meet compliance requirements. All mortgage lenders must provide this form at closing to refinancing borrowers who qualify for taxexemption under Section 12-108(g) (3) of the Tax-Property Article of the Annotated Code of Maryland. Borrower swears under oath that he/she is original mortgagor, resides in the mortgaged property and the amount of the refinanced loan. MD Code provides that a mortgage or deed of trust is not subject to recordation tax if the property being refinanced is still the principal residence of the mortgagor (or Trustee of an Inter Vivos Trust) and the new mortgage amount is not greater than the current unpaid principal balance. To qualify for this exemption, a signed affidavit may be attached to the security instrument stating such. Minnesota RE-MTG-MN Real Estate Mortgage The following paragraph is being added to comply with Section 47.20 subd. 8 (2). 26. BORROWER'S COPY. Lender shall furnish to borrower a conformed copy of the evidence of debt and this Security Instrument at the time of execution or within a reasonable time after recordation. This authority applies to goods and services retail sales - AS LONG AS the retail contract is assigned to a financial institution under Minn. Stat. 47.59. The amount of the late charge is hardcoded in the goods and services (gs) retail contracts with LF and LFA in the form names. They also allow the flexibility for dealers to assign the contracts to financial institutions. In the LF and LFA forms, the maximum charge referenced in the Late Charge provision (in the Fed Box) is being changed from $7.28 to $7.80. 5

Missouri BTA-1-MO Application for Missouri Boat/Vessel or Outboard Motor Title and Registration The Missouri Department of Revenue has updated their Application for Missouri Boat/Vessel or Outboard Motor Title and Registration form DOR-93 with a revision date of 08-2012. We are updating form BTA-1-MO to match the states changes. In the Unit section, a field will be added for Model Year - New Outboard Motor Only. Checkboxes will be added for Commercial Passenger, Commercial Fishing, and Commercial Other. EFS-3-ADD-MT - Effective Financing Statement Amendment Addendum The state of Montana has revised their Effective Financing Statement Amendment Addendum form with a revision date of 8/2/2012. We are updating EFS-3-ADD-MT to match the states changes. When referring to "Debtor's Signature" it now states "Debtor's Signature, Authorization, or Authentication." In the additional/changed or deleted farm products table they have added more fields. Other minor changes have been made to the document. 7

EFS-3-LAZ-MT - Effective Financing Statement Amendment The state of Montana has updated their Effective Financing Statement Amendment form with a revision date of 8/2/2012. We are updating EFS-3-LAZ-MT to match the states changes. When referring to "Debtor's Signature" it now states "Debtor's Signature, Authorization, or Authentication." In the Amendment Types section, they have added a $5.00 fee to sections Continuation, Full Assignment, Partial Release, and Partial Assignment. Other minor changes have been made throughout the document. EFS-3-INS-MT - Instructions for Completing EFS Amendment The state of Montana has updated their Instructions for Completing EFS Amendment form with a revision date of 8/2/2012. We are updating EFS-3-INS-MT to match the states changes. The EFS Amendment Fee section has been changed to "See each amendment type for the specific filing fee." Other minor changes will be made to match the states form. New VMP4041MT Discharge of Deed of Trust We discontinued the SAT-DT-MT and replaced it with the VMP4041MT. The VMP4041MT evidences full payment of a loan under a Deed of Trust. Discontinued SAT-DT-MT - Satisfaction of Deed of Trust The SAT-DT-MT was discontinued and replaced with the VMP4041MT New York BTA-1-NY - Boat Registration/Title Application The New York State Department of Motor Vehicles has updated their Boat Registration/Title Application (form MV-82B) with a revision date of 8/11. We are updating our BTA-1-NY to match the states changes. In section 3, they have added an additional field to "OWNER CLIENT ID NO. (from Driver License)." E-FORMS End-User Product Update MV-1-OK - Title Application The Oklahoma Tax Commission has updated their Form 701-6. Wolters Kluwer Financial Services will mirror their changes on our MV-1-OK. The form has changed from a two page portrait layout to a one page landscape layout. The state also added a hardcoded barcode at the top left of the document. Utah MV-1-UT - Application for Title The Division of Motor Vehicles has updated their Application for Title (TC-656) with a revision date of 10/12. We are updating our forms MV-1-UT to match the states changes. A checkbox has been added for Non-repairable title. Other minor changes will be made to the document. Washington BTA-1-WA - Vessel Title Application The Washington State Department of Licensing has updated their Vessel Title Application (form TD-420-289) with a revision date of 7/12.
