Updated: October, 2017 How to Select Your Benefits for Open Enrollment 2018 Overview: The following Step-Action table will walk you through the process of selecting your benefit plans for 2018. Open Enrollment for 2018 benefits starts October 16 and ends on November 13, 2017 Step Action 1 Click the Oracle Icon on your computer desk top. To select benefits from your home computer: In your web browser type www.tampagov.net Once on the Tampagov homepage, type Employees in the green search field. Click the link Guide for City Employees Click the link Oracle EBS Employee Self Service (Payroll, Benefits and Personal Information). You may need to scroll to view the link. Go to Step 2 below. 2 To login to the Oracle System: 1. Type your Oracle User Name in the User Name box. Be sure to type a period between your first and last name. Example: james.smith. 2. Enter your password. (If you can t remember your password contact the Help Desk at 274-8067 for assistance). 3. Click the button
3 From the Oracle Applications home Page, click the link under the Main Menu. 4 Click the link.
5 1. Read the Employee Benefits Medical Disclaimer. Note: If you select Decline, you will not be able to elect your benefits. Enrolling in these plans authorizes the City of Tampa to provide the vendors with your personal information such as social security number, address of record, home phone number and date of birth for you and your dependents enrolled. By submitting your benefit choices, you are authorizing the City of Tampa to make deductions from your bi-weekly pay for the cost of the benefit elections you have chosen. You understand that you cannot change your elections, with the exception of deferred compensation, during the plan year except within 30 days of a qualifying change of status event as defined by the Internal Revenue Service (IRS). You certify that all information entered is true and correct to the best of your knowledge, and that all dependents meet eligibility requirements. You understand and agree that any misstatements may result in a denial of benefits and/or termination of coverage/membership. You agree to be bound by the group contract terms and conditions for the benefits you enroll. Any person who knowingly and with the intent to injure, defraud or deceive any insurer, files a statement of claim or an application containing any false, incomplete or misleading information is guilty of a felony of the third degree and subject to disciplinary action. As required by State and/or Federal laws, statutes, or regulations, legal notices that are detailed in the City of Tampa Benefits Booklet are incorporated here by reference. 2. Click Printable Page should you desire to have a copy of the disclaimer for your records. 3. Click to continue your benefits selections. 4. Click button to go to the next page. Tip: Next is also the Save the information of this page button throughout the sections. 6 You will arrive on the Dependents and Beneficiaries page. 1. Review the information to make sure the list of dependents and beneficiaries, their social security numbers and birthdates are correct. 2. If you want to change the information, click the pencil. 3. Click Next to go to the next page.
7 To add another person, click button. 8 Type in the name and relationship information, and then click the Tip: * Indicates required field. button. 9 1. Click box if the person you added has the same address as yours. 2. Finish adding addresses for the new beneficiaries or dependents as well as the Miscellaneous Information. 3. 11 The top menu bar will guide you through the enrollment process: As you move through the process, the yellow ball will move to the right. 12 The choices listed below may vary based on family member information. Please add any dependents you want to cover or beneficiaries that you want to designate that have not been added into the system. You will need to enter the social security number and birthdate for each. Tip: If the dependents/beneficiaries did not have a social security number in the current system, they will not appear in the list. 13 Your current enrollment will display with a checkmark in the box. If you are waiving the Medical Plan, be sure to select the Waive Medical Coverage The 2018 cost for your election will be listed in the Pay Period Before Tax Deduction column to help you make your decisions. box. 14 Select your Dental Plan or select Waive Dental Coverage. New enrollees in the Humana DHMO need to contact Humana in January, 2018 to select a primary care dentist. 15 Advantica Vision Plan. This is an elective vision plan, available through Advantica and separate from the basic vision coverage available under the UnitedHealthcare health plan. Select the option that is most beneficial to your family or waive the coverage. 16 Select Medical Flexible Spending Account (FSA) Tip: The federal law limiting annual contributions in the Medical FSA to $2,600 per year does not affect the Dependent Care FSA limit of $5,000. 17 Select Dependent Care Flexible Spending Account (FSA) Tip: The federal law limiting annual contributions in the Dependent Care FSA is $5,000. 18 Your Life and Accidental Death and Dismemberment insurance is provided at no cost to you. The coverage amount is based upon your annual salary ($37,500 minimum coverage or one times your salary to a maximum of $150,000.)
19 Select a Long Term Disability option. Tip: All full-time employees automatically receive the City-paid base plan that replaces 30% of income up to a maximum of $10,000 per month. You may increase your coverage by one level during Open Enrollment. 20 Select a Deferred Compensation option, enter the monthly amount you would like to elect, then scroll down and click the button to find out the cost per pay period. PreTax Plan: After Tax Plan: 21 Designation of beneficiaries to receive unused accruals and unpaid earned wages. 22
23 Verify your Dependent Selections. Click the next button to save and move to the next page. 24 Determine the percentage amount of your Group Life, Last Paycheck, and Pension Plan to assign to your beneficiaries. You may need to scroll down the page to see all screens. Continued on Next Page.

27 During the Open Enrollment period, October 16, 2017 to November 13, 2017, you can change or update your benefits as needed. If you make any changes, be sure to click the button to the final page and then click the or save a new confirmation statement for your records! Confirmation Statement button to print 28 Additional elective benefits are offered through Employee Family Protection (EFP). You may read about them on the Employee Benefits page under Insurance Plans. Short Term Disability Whole Life Insurance Legal Services Accident Insurance Critical Illness with Cancer Insurance If you choose to elect one of these elective benefits, make an appointment with an EFP Benefits Counselor by calling (844)231-1623 or go to www.efpnow.com/cotappointments. Employees must meet with an EFP Benefits Counselor to enroll, change, or cancel an elective benefit.
