This policy indemnifies you as the employer against pecuniary loss sustained as a result of fraud or dishonest act of your employee(s). 2. What are the covers/benefits provided? This policy covers you against direct pecuniary loss sustained by any act of fraud or dishonesty committed by the employees:- a) during the Period of Insurance stated in the policy. b) during the uninterrupted continuance of employment of such employee. c) in connection with the occupation and duties of such employees and d) discovered during the aforesaid Period of Insurance or within 6 months thereafter or within 6 months after the death, dismissal or retirement of such employee whichever shall happen first. Duration of cover is usually for one year. You need to renew your insurance policy annually. 3. How much premium do I have to pay? What are some of the key terms and conditions that I should be aware of? a) Duty Of Disclosure Non-Consumer Insurance Contract Pursuant to Paragraph 4(1) of Schedule 9 of the Financial Services Act 2013, if you are applying for this Insurance for purposes related to your trade, business or profession, you have a duty to disclose any matter that you know to be relevant to our decision in accepting the risks and determining the rates and terms to be applied and any matter a reasonable person in the circumstances could be expected to know to be relevant, otherwise it may result in avoidance of your contract of insurance, refusal or reduction of your claim(s), change of terms or termination of your contract of insurance. This duty of disclosure shall continue until the time the contract is entered into, varied or renewed. You also have a duty to tell us immediately if at any time after your contract of insurance has been entered into, varied or renewed with us, any of the information given in the Proposal Form is inaccurate or has changed. b) Premium Warranty Premium must be paid within 60 days from the inception date of the cover, otherwise the cover will automatically be cancelled. c) Limit Of Guarantee Please ensure that the limit of guarantee is adequate. d) Claims Should you become aware of any circumstances likely to give rise to a claim, you shall immediately give notice to us stating if known the whereabouts of the employee and particulars of the acts or defaults discovered. e) Renewal Depending on the circumstances of the loss or claims made during the term of policy, we may review the policy terms and conditions upon renewal or decline renewal. f) Payment Mode Payment can be made by cash, credit card or GIRO to Berjaya Sompo Insurance Berhad. Insist on a receipt of the premium paid as proof of payment for future reference. Note: This list is non-exhaustive. Please refer to the policy contract for the full list of the terms and conditions under this policy. 6. What are the major exclusions under this policy? This policy does not cover:- a) Unexplained losses or shortages discovered during an inventory or stock taking. b) Loss due to unidentifiable employees. c) Non-notification to us on the change in the nature of your business and duties of your employees. You may cancel your policy at any time by giving written notice to us. Upon cancellation, any refund premium would be based on the conditions as stated in the policy contract. 9. Pastikan anda juga membaca terma dan syarat am.) 1. Apakah produk ini? Polisi ini mengganti rugi anda sebagai majikan terhadap kerugian kewangan yang dialami akibat penipuan atau perbuatan tidak jujur pekerja atau pekerja-pekerja anda. 2. Apakah perlindungan / faedah yang disediakan? Polisi ini melindungi anda terhadap kerugian kewangan langsung yang ditanggung oleh mana-mana tindakan penipuan atau ketidakjujuran yang dilakukan oleh kakitangan:- a) dalam Tempoh Insurans seperti yang dinyatakan di dalam polisi. b) semasa tempoh berterusan tanpa gangguan penggajian pekerja itu. c) berkaitan dengan pekerjaan dan tugas pekerja itu dan d) ditemui semasa tempoh yang disebut terdahulu Insurans atau dalam masa 6 bulan selepas itu atau dalam tempoh 6 bulan selepas kematian, pemberhentian atau persaraan pekerja itu yang mana berlaku terlebih dahulu. Tempoh perlindungan biasanya selama setahun. Anda dikehendaki membaharui polisi insurans anda setiap tahun. 3. Berapakah premium yang saya perlu bayar? Premium yang anda perlu bayar mungkin berbeza bergantung kepada had jaminan, jumlah pekerja, pekerjaan atau jenis perniagaan, pengalaman tuntutan dan lain-lain. Apakah terma dan syarat utama yang saya harus ketahui? a) Kewajipan Pendedahan Kontrak Insurans Komersial Menurut Perenggan 4(1) Jadual 9 Akta Perkhidmatan Kewangan 2013, jika anda memohon insurans ini untuk tujuan yang berkaitan dengan perdagangan, perniagaan atau profesion anda, anda berkewajipan untuk mendedahkan apa-apa perkara yang anda tahu akan mempengaruhi keputusan kami dalam menerima risiko dan menentukan kadar dan terma yang dikenakan, dan apa-apa perkara yang munasabah yang boleh dijangka, jika tidak ia boleh menyebabkan pembatalan kontrak insurans, keengganan atau pengurangan ganti rugi, perubahan terma atau penamatan kontrak insurans anda. Kewajipan pendedahan untuk Kontrak Insurans Pengguna dan Kontrak Insurans Komersial hendaklah diteruskan sehingga kontrak insurans anda dimeterai, diubah atau diperbaharui dengan kami. Anda juga mempunyai kewajipan untuk memberitahu kami dengan serta-merta jika pada bilabila masa selepas kontrak insurans anda ditandatangani, diubah atau diperbaharui dengan kami, apaapa maklumat yang dinyatakan dalam Borang Cadangan ini tidak tepat atau telah berubah. b) Waranti Premium Premium mesti dibayar dalam masa 60 hari dari tarikh bermulanya perlindungan, jika tidak perlindungan secara automatik akan dibatalkan. Sila ambil perhatian bahawa anda masih bertanggungjawab untuk membayar perkadaran premium bagi 60 hari c) Had Jaminan Sila pastikan bahawa had jaminan adalah mencukupi. d) Tuntutan Sekiranya anda menyedari sebarang keadaan yang mungkin menimbulkan tuntutan, anda hendaklah dengan segera memberikan notis kepada kami menyatakan jika diketahui di mana beradanya pekerja dan butir-butir mengenai perbuatan atau kemungkiran ditemui. e) Pembaharuan Bergantung kepada keadaan kerugian atau tuntutan dibuat semasa tempoh polisi, kami boleh mengkaji semula terma-terma dan syarat-syarat polisi pada masa pembaharuan atau menolak pembaharuan. f) Kaedah Pembayaran Bayaran boleh dibuat melalui Tunai, Kad Kredit atau GIRO kepada Berjaya Sompo Insurance Berhad. Sentiasa meminta resit untuk premium yang dibayar sebagai bukti pembayaran untuk rujukan masa depan. Nota: Senarai ini adalah tidak lengkap.
