The Pleasant Valley Recreation and Park District Senior Center invites you to participate in the 12th annual 50 Plus Expo on May 1, 2018 from 9:30am to 1pm, at the Community Center, 1605 E. Burnley St., in Camarillo. Our goal is to reach out to the increasing population of 50-Plus adults who should be thinking about the services you can provide. This event helps provide services, activities, and programs to seniors at the Pleasant Valley Senior Center. Advertising opportunities in the event brochure are offered as well. Sponsorship opportunities are also being offered for your consideration. Registration: You may download the exhibitor registration forms, or complete them on your computer, using the following link: http://pvrpd.org/recreation/special_events/50_plus_expo.asp Mail the forms with your check to 50 Plus Expo, 1605 E. Burnley Street, Camarillo, CA 93010. Payment by credit card is also possible; a $10 service fee is required. Please scan the completed form and email it to dcleric@pvrpd.org, or mail the completed forms with your check to the Pleasant Valley Senior Center at 1605 E. Burnley Street, Camarillo. Both the completed forms and payment of the designated sum made out to PVRPD are required for the committee to assign table space. Please have your registration forms to us before March 30. Those postmarked after March 30 must include a $25 late fee. We have three distinct display areas, some with and some without electrical service. Booth setup starts at 8:30am. Agreement, Waiver and Release I understand that a portion of this event is outdoors and may be subject to inclement weather (rain, wind, etc.) Pleasant Valley Recreation and Park District (PVRPD) will close the event only under extreme conditions. Any fee paid is non-refundable and will be retained by PVRPD to cover staff and publicity costs. It is the responsibility of the applicant to decide to set-up or not to set-up their booth. I have carefully read the description of the activity/event for which I/we are applying and in consideration for being permitted by the Pleasant Valley Recreation and Park District (PVRPD) to participate in the activity/ event, I hereby waive, release, and discharge any and all claims for damages for personal injury, death or property damage which I may have, or which may hereafter occur to me, as a result of participation in this event. This release is intended to release in advance the Pleasant Valley Recreation and Park District, its officers, employees, volunteers and agents from any and all liability arising from or connected in any way with my participation in this activity/event, even though that liability may arise as the result of negligence or carelessness on the part of the Pleasant Valley Recreation and Park District, its officers, employees, volunteers and agents. It is understood that this activity/event involves an element of risk and danger of accident and knowing those risks, I hereby assume those risks. It is further agreed that this waiver release and assumption of risk is to be binding on my heirs and assigns. I agree to indemnify and to hold the Pleasant Valley Recreation and Park District, its officers, employees, volunteers and agents free and harmless from any liability, loss, damage, cost or expense which may be incurred as a result of injury, death or property damage that I sustain while participating in this activity/event. CONSENT AND RELEASE FOR MAKING AND USE OF VIDEO/PHOTOGRAPHY FOR PRINT OR INTERNET: In consideration for being a participant in an activity/event offered by PVRPD, I hereby consent and grant to PVRPD the right, without fees, to make and use video recordings and still photographs of me, my minor children, dependents in my care and any product I may be selling at the activity/event in which I am a participant, in any manner or form and for any lawful purpose at any time. I waive any right I may have to review or approve the finished product, or the use to which it may be applied. I release and discharge PVRPD, its employees, volunteers an agents from any liability to me, my children and/or dependents in my care by virtue of any represnetation that may occur in making, editing or use of said video recording or still photography.
