The Company agrees to pay at Harare, the benefits which may become payable, as stated in the Schedule to the person entitled to receive the proceeds subject to conditions specified on item 4 of the General Privileges and Conditions. It is hereby declared that this Policy contract is subject to the accompanying conditions and privileges and every subsequent endorsement, amendment or change of the terms and conditions (if any) by the Company. No variation, amendment or any change of the terms and conditions of this Policy contract in any manner whatsoever shall be valid and binding on the company unless reduced to writing and signed by the Managing Director. DIASPORA FUNERAL CASH PLAN GENERAL PRIVILEGES AND CONDITIONS 1. BENEFITS PAYABLE The Sum assured is payable on the death of an assured member 2.1 PAYMENT OF BENEFITS Benefits are payable upon satisfactory proof being supplied to the Operations Director of the Company and all the conditions listed below having been complied with: 2.1.1 Death in Urban Areas An original burial order, with a letter from the hospital/clinic stating the date, place and cause of death. A certified copy of the Death Certificate within a month from the date of notification. The Company will not pay the next claim under the policy if the certified copy death certificate of the initial claim is not submitted. 2.1.2 Death in Rural Areas An original letter from the Headman or Chief, stating the full name of the deceased, date of birth, identity number date of death and cause of death. A certified copy of the Death Certificate within a month from the date of notification 2.1.3 Accidental Death A written confirmation from the Police A certified copy of the Death Certificate within a month from the date of notification In all cases, the Company must have received all premiums due. 2.14 Waiting Period If death of an Insured life due to causes other than accident occurs within twelve months from the date of first premium payment, a benefit equal to the monthly premiums received less levies in respect of that benefit will be paid. 3. PREMIUMS Premiums are paid monthly, quarterly, half yearly or yearly in advance. 3.1 PREMIUM PAYING TERM The premium paying term is 20 years, after which cover continues for life for free. 3.2 DEATH OF PRINCIPAL MEMBER On the death of the principal member, other family members remain covered with no premium payments required, provided the policy has been running for a continuous period of 16 Years. In the event of death of the Principal within the 16 years, the family members can continue to pay premiums on the existing policy for the balance of the 16 years after which premiums cease. 4. PERIOD OF GRACE One calendar month grace period shall be allowed for the payment of annual premiums. Where premiums are paid on a frequency other than annually, a thirty (30) day grace period shall be allowed. Failure to pay due premiums within the grace period entitles the Company to cancel the Policy without further notice. 5. LUMP SUM INJECTIONS 6. LAPSE Lump sum premium injections are not allowable under this Policy. In the event of non-payment of premiums within the grace period at any time during the currency of this Policy, the Policy shall lapse. All premiums paid shall be forfeited to the Company and any benefits under this Policy shall be lost.
7. PAID-UP STATUS This Policy cannot be made Paid-up at any time during the Policy term. 8. REINSTATEMENT Should this Policy lapse, it may be reinstated upon request, on such terms as may be fixed from time to time by the Company. Such reinstatement on lapsed Policies can only be done within a twelve (12) calendar months period from the lapse date. 9. SURRENDER 10. LOAN This Policy cannot be surrendered partially or wholly at any time during the Policy term. Loan benefit is not allowable under this Policy. 11. CESSION No benefit under this Policy shall be capable of assignment, alienation or being pledged as security. Any act or attempt to cede, assign, alienate or pledge the Policy as security shall be null and void notwithstanding any representation or warranty to the contrary. 12. PAYMENTS All payments (including plan benefits) to or by the Company shall be made at/from its office in Harare or other branches of the Company in United States Dollars (US$s). 13 POLICY EXCLUSIONS Any claim arising as a consequence of war, invasion, act of foreign enemy, hostilities or war like operations( whether war has been declared or not), mutiny, riot civil commotion, civil war, rebellion, insurrections, conspiracy or siege
a) Accidental Death: This shall be construed to mean that death is caused during the currency of this policy before the expiry date of cover, directly or independently of all other b) causes, by bodily injury resulting from external, violent and accidental means as long as the accident was not directly or indirectly occasioned or accelerated by any of the following causes: Dear Policyholder IMPORTANT This flyer is meant to assist you our valued client and claimant, of our claim requirements, settlement period, and procedure and payment methods. A. CLAIMANT The Principal Life shall be the Principal Claimant during his lifetime and all payments shall be paid to him. In the death or absence of the Principal Life, any of the covered lives who is a major shall claim. B. MANDATORY CLAIM DOCUMENTS Urban Areas A death certificate, burial order or equivalent document from the country of death Rural Areas A letter from the Herdman or Chief, authenticated by the Police, stating the full name of the deceased, date of birth, identity number, and date of death Accidental Death (Urban/Rural) NOTE: A police report for all accidental deaths within the waiting period (Note: after the waiting period, the police report ceases to be mandatory). 1. A certified copy of the death certificate is required for all claims notified after burial. 2. Where ALL the required documents are not submitted the claim WILL NOT be processed. 3. The Company shall not honour claims on normal deaths occurring during the waiting period. C. CLAIM NOTIFICATION All claims should be notified through the nearest Zimnat Life office. All valid claims shall be settled within 24 hours subject to submission of the above requirements. 1. Nuclear explosion or radio activity; his /her taking of alcohol; drugs; fumes; or gas, suicide; attempted suicide or self injury committed intentionally or while mentally disordered; any violation of the criminal law by the life assured; his/her willful exposure to unnecessary danger, except in the tempt to save human life; or bodily or mental infirmity or disease of any kind. 2. His or her active participation in mountaineering, racing on horseback, on wheels or in motor powered boats, or activities, which do not form part of his normal occupation duties. 3. His/her active participation in parachute jumping or skydiving, or in aviation, other than as a passenger traveling solely for the purpose of transport in a registered passenger aircraft on a regular route between licensed airfields, flown by a duly licensed pilot and owned and operated by a duly licensed airline or air transport company. 4. War, invasion, enemy action, hostilities (whether war be declared or not) or civil commotion, or the act of any person acting on behalf of any organization with activities directed towards the overthrow by force of the Government de jure or to the influencing of it by terrorism or violence. b) Normal Death: This refers to death caused by any other causes other than accident as stated in a) above. c) Policy Term: This is the selected duration of the policy. d) Company: Company is used in the terms and conditions to mean Zimnat Life company. e) Principal Life Assured: This is the proposer or the owner of the policy or simply the policyholder. f) Urban Areas: These are areas in towns and major cities of Zimbabwe, the term includes growth points where there is a police post plus death and birth registration office to enable people to process death claim certificates and the burial orders. g) Rural Areas: This refers to the remote areas in Zimbabwe where one can not easily get the police and a Registrar of Death and Birth s office to enable the processing of a death claim and the burial order. This is the area where the Chief and the Headman can certify a death as explained in the terms and conditions. D. SUICIDE CLAUSE In the event of the death of the life assured by suicide (whether sane or insane) before the policy has been in force for twenty-four months from the date of commencement of this insurance, this policy shall be void and all premiums paid shall be forfeited. The same condition shall be imposed from the date of reinstatement in the event of the policy lapsing, as if the date of reinstatement was the date of commencement.
