From our main webpage http://www.fajua.org/ click on FAJUA Producers, then select the link for FAJUA Online Binding Forms & Applications The log in screen will be displayed as shown below. Screen includes several document links that are available prior to logging in, including quoting instructions, change request forms and other supporting documents. Next complete the Policy Information section including the effective dates, and other questions. Fleet Policy- Only answer Yes to the fleet policy question if there are more than 5 vehicles. Enter the mailing address information and complete the Business Information section. At the bottom of any page you can also select the More Actions dropdown box. Here you will be save your work on the current page. Next, confirm the information that prefilled into the Locations section of the quote. Open up the drop down button for Territory and Town Code to select the value under the dropdown. Click on the Add button to list this address. Notice on the top left, you have 2 tabs which are now blue. Your quote is now saved when you hit the continue button on the previous screen and now to can toggle to any screen which has been completed already. On the Vehicles page, select Click Here to add to enter in your first vehicle. At the top of the page, confirm the garaging location and all information for the first vehicle which will be added. Under Vehicle Type select Special Vehicle under the drop down menu. Page 7 of 20 As of 11/12/2015
Next select Low Speed Vehicle under Special Vehicle Type section and enter in the coverage choices for the liability coverage. Physical damage coverage is not available for Low speed vehicles through the FAJUA. Page 8 of 20 As of 11/12/2015
Hit Add when you re at the end. If you have other vehicles, select the Click here to add to move onto the next one. When all the vehicles are added, select Continue. The FAJUA Low Speed vehicle policy will give you the ability to endorse the policy with Hired Auto coverage that will extend coverage to a substitute vehicle. Don t make any changes to the Premium Modification screen and hit Continue to move to the Driver screen. Next, complete the Drivers section to add your operators. Hit the highlighted link to add the first driver s information. Page 10 of 20 As of 11/12/2015
Then enter the information for the first operator and click on Save when completed. Do not enter in any information under the No of Years Continually Licensed section because the system will calculate this for you. Select Add at the end, then you can move onto any other drivers. If there aren t any more drivers to add, click on Continue to move to the next screen. Complete the General Questionnaire which will ask additional Underwriting questions and hit Continue. Page 12 of 20 As of 11/12/2015
If the Insured has any Additional Interests, it can be added on this page. Select the Click here to add to make this entry. Contracts with any Additional Interest Will need to be submitted to us with the application. Next, you ll need to reference the prior carrier information. Next, add in any chargeable violations for the operators on the policy. Did conviction arise as a result of a crash- If the answer to this question is Yes, then it should be added under loss history. Click on Add for this to be included on the quote. Page 14 of 20 As of 11/12/2015
Select Click here to add to include any more violations. On the far right, you can edit or delete violations as well. Next, include any chargeable accidents under the Loss History tab. Select Click here to add to enter. Page 15 of 20 As of 11/12/2015
Select Add when all the required fields have been completed then add any more if they should be included. The next tab on our portal is for attaching files but this function is currently unavailable. Please include any additional comments or information that may be helpful to Underwriting. If you don t have enough room on the Applicant name field you can enter in any additional named insured s here. Also enter in the reason the customer is changing carriers. The portal will then provide you with the proposal request or the application. This is provided to give you the opportunity to review your work before it is finalized and submitted to FAJUA. Complete all the fields about the payment being make for this policy and hit Continue. If this will be a Request for Proposal, enter in $0 for the amount. The system will move you to the confirmation screen. Please hit Continue again. Page 18 of 20 As of 11/12/2015
The portal will then provide you with the finalized proposal request or the application. The paperwork will need to be completed and signed by the insured. The original signed proposal request or the application will need to be mailed, along with any required supplemental documents. If the risk has over 5 vehicles, the system will provide a Request for Proposal instead. The rating will be concluded about 30 days after the date that all the necessary information is received. Do not mail us payment if you are sending us the proposal request for a quote. Required documents- Original signed application or Request for proposal.
