LEAVING MOTOROLA SOLUTIONS CHECKLIST FOR U.S. EMPLOYEES WHO ARE LEAVING MSI NOVEMBER 2017 THIS CHECKLIST HIGHLIGHTS U.S. BENEFIT AND OTHER ITEMS THAT YOU SHOULD BE AWARE OF WHEN YOUR EMPLOYMENT WITH MOTOROLA SOLUTIONS ENDS. If you leave MSI mid-month, the full month s benefits will be deducted from your last paycheck for coverage through the end of the month. You may elect to continue the coverage you have in effect as of your employment termination date for up to 18 months through COBRA. You may also want to review the health care options at www.healthcare.gov. You ll receive information about your 401(k) options from Voya within 14 days. You ll receive COBRA (and retiree health care, if applicable) information from the Benefits Service Center within 14 days of Motorola Solutions providing the Center with notification of your employment termination date. If you have a 401(k) loan, contact Voya. Elect COBRA coverage within 60 days from the receipt of the COBRA information by calling the Benefits Service Center. Send in your first COBRA payment within 45 days of your election, payable to Motorola Solutions to: COBRA - Motorola Solutions P.O. Box 0742 Carol Stream, IL 60132-0742. Initiate your request for post-termination loan repayments within 30 days of your last day worked. You must start receiving payments from your 401(k) account by April 1 of the year following your age 70 ½ birthday. BENEFIT YOUR OPTIONS PROCESS DEADLINE CONTACT INFORMATION Health Savings Account (HSA) Your HSA is portable and carries forward into the future to be used for qualified medical expenses at any time. A $2.50 monthly service fee will be charged to maintain your account effective the first month following your Employment Termination Date if your HSA balance is less than $3,000. When your employment ends, any amounts left in your HSA will remain in the account. You are responsible for ensuring you have not over-contributed for the plan year. See page 1 for information regarding health care benefits Claims must be postmarked or faxed to Your Spending Account by March 31 of the year after your termination of employment. Contact the Benefits Service Center at (800) 585-5100 If you have already spent your entire year s worth of FSA funds, you are not required to pay FSA dollars back to Motorola Solutions even though you may not have contributed the full amount elected. Critical Illness, Hospital Indemnity, Accident and Identity Theft Protection You may continue your coverage via direct bill. You ll receive information from Allstate and or InfoArmor on how to continue coverage. Contact Allstate and/or InfoArmor within 60 days after your employment ends to set up direct billing. Dependent Care Account (DCA) You may request reimbursement of eligible expenses incurred through Dec. 31 of the year your employment ends. You may check your balance and request reimbursement using the Your Spending Account website. DCA claims must be postmarked or faxed to Your Spending Account by March 31 of the year after your termination of employment. About one week after your employment ends, you may call the Benefits Service Center to request information on your options. Only certain lump sum payments may be requested online. BENEFIT YOUR OPTIONS PROCESS DEADLINE CONTACT INFORMATION Basic and Supplemental Life Insurance and Dependent Life Insurance Long-Term Care Insurance Adoption Assistance You and/or your dependents may apply to convert your coverage to an individual policy. Contact CNA to continue your coverage via direct bill. Contact LifeSecure to continue your coverage. You may request reimbursement of eligible adoption assistance expenses (up to $8,000 per child). You ll receive information from the Benefits Service Center within 14 days after your employment termination date about your right to convert coverage. You can also contact MetLife in advance of your last day of employment for information. Contact your provider. You may request reimbursement using the Your Spending Account. You must apply for conversion with MetLife and pay the first premium within 31 days after the end of the month in which your employment ends. Contact CNA within 31 days after your employment ends to set up direct billing. Your reimbursement request must be received within 31 days of your employment termination date. HERE ARE OTHER ITEMS YOU MAY NEED TO ADDRESS BEFORE LEAVING MOTOROLA SOLUTIONS: OTHER ITEMS YOUR OPTIONS PROCESS DEADLINE CONTACT INFORMATION Unused Paid Time Off (PTO) Equipment return Update your address Corporate credit card and expense reimbursement Annual Incentive Plan (AIP) Record all used PTO hours in etime. You will be paid for earned, unused PTO hours within two pay periods after your last day on payroll. You must repay the amount of any negative PTO balance. You have to return company-owned equipment. Maintain your address records after you leave MSI. Process your outstanding credit card balances before your exit date. Payment is your responsibility. Amex/Citi will turn over any balance that is 180 days past due to a collection agency. Eligibility: Your eligibility for a payment under AIP is determined by your separation reason. For additional information, see the Leaving MSI FAQs. Go to etime system to update your PTO. You and your manager should determine what equipment needs to be returned and how you ll turn it in. Your address must be updated in all systems each time you change address. If not, MSI will have no way to contact you if necessary. Submit your final expense statement through iexpense and pay any personal expenses. If there is an AIP payout for the year in which your employment ends and you are eligible to receive this payment, your AIP award will be prorated for the time you served with the company. Prior to your last day. Return equipment on or before your last day worked. Immediately upon changing address. Pay all balances by your last day on payroll. If you plan on submitting expenses later than this date, work with your manager to make arrangements. AIP is paid into the bank account you have on record, generally in late March/early April of the following year. OTHER ITEMS YOUR OPTIONS PROCESS DEADLINE CONTACT INFORMATION Sales Incentive Plan (SIP) Employment verification Equity (stock options and restricted stock units RSUs) Employee Stock Purchase Plan (ESPP) Mobile phone If a new employer needs to verify your Motorola Solutions employment, you have several options. Review the appropriate separation schedule or refer to your award agreement(s) in your E*TRADE account prior to your last day at MSI to understand what happens to your stock options and/or RSUs based on your termination reason. If you are a participant in the ESPP, review the information on the ESPP website to confirm what you should do before you leave. Obtain your manager s approval if you want to keep your mobile phone number. Contact the HR team. US Employees can also obtain verification via The Work Number. Access your E*TRADE account within 5 business days of leaving for details. If your employment ends during an offering period, you will be withdrawn from the ESPP. Any accumulated payroll deductions for the current offering period will be returned to you, without interest through payroll. If your employment termination date is the last day of an offering period, your contributions for the current offering period will be used to purchase shares. Cancel the current account, change to a personal account in your name, or transfer it to another employee. You can access information in your E*TRADE account at any time after you leave MSI. Update your email address to ensure you continue to receive communications after you leave. Information on the MSI HR portal is only accessible through your last day worked. Your ESPP account will remain active at E*TRADE until it s closed (e.g. if you sell your shares/transfer your account to another broker or the funds or shares held in the account are distributed to your estate). There aren t any fees to retain your ESPP account at E*TRADE. View the ESPP Participation guide for details. The request to move your mobile phone account to a personal account (or transfer to another employee) must be completed at least five business days before your last day worked. Access your prior pay statements through ADP. Change your ADP portal email address to your personal email address to ensure future access to paystubs and W-2 Forms. You may keep your account(s) open. You ll continue to be eligible for EAP benefits for 30 days following your termination and/or retirement. During this 30-day period, an initial session can be scheduled at any time. You can continue participation in the Education Assistance Program for the current quarter or semester. If your separation is voluntary, complete the online exit survey. Contact the IT Help Desk with questions on how keep access to personal information. Access ipay using your ADP login ID. If you don t subscribe to ipay as an active employee, visit the ipay website for registration instructions to create your login ID and password using registration code Motorola-ipay. If you start sessions either before your termination/retirement or within the 30-day period following your termination or retirement, you re allowed the usual number of sessions (up to five). Your dependents who lose eligibility will also be granted the 30-day time period to initiate services. Click on the link, enter your employee ID, and you ll be taken to the survey. This exit survey should only take about 10 minutes to complete. Prior to last day worked. Prior pay statements and Form W2 are available for at least three years. An initial session can be scheduled within 30 days following your termination and/or retirement. Reimbursement decisions will be made as usual, according to policy. This survey will be available to you for 30 days and can be accessed online using the address provided. OTHER ITEMS YOUR OPTIONS PROCESS DEADLINE CONTACT INFORMATION Wellness center If you re not eligible for coverage under the Post-Employment Health Benefits Plan (Retiree Health Plan), your access to our onsite Wellness Centers ends on your last day of employment. If you re currently a member of the wellness center and you meet the eligibility requirements for Retiree Health Plan benefits (i.e. Rule of 75) as of your Employment Termination Date, you may continue your membership by paying the applicable rate. If you re not a member, you may enroll in the wellness center at any time after you retire at the applicable rate. TIPS ABOUT ACCESSING YOUR BENEFITS RESOURCES AS A FORMER EMPLOYEE The first time you access Your Benefits Resources through the Internet, go to www.yourbenefitsresources.com/mot-solutions. Click on the Register as a new user link from the Log On page. Enter your personal information, answer at least five security questions to keep your account secure, and create your user ID and password. Look for the Completed Successfully page at the end of the registration process. It confirms that your logon information was updated successfully. If you ve already registered and have forgotten your user ID or password, select I forgot my user ID or I forgot my password and follow the prompts. Be sure to review details in our U.S. Benefits Books (available on mysolutions-benefits.com and Your Benefits Resources after your last day worked) for governing rules and conditions. In the event of a difference between information in this checklist and the terms of the applicable Plan documents, the Plan documents shall control.

IN THE EVENT OF RETIREMENT IF YOU RE RETIRING, IT S IMPORTANT TO REVIEW THE FOLLOWING INFORMATION: ITEM YOUR OPTIONS PROCESS DEADLINE CONTACT INFORMATION Retiree Health Care Eligibility Retiree Health Reimbursement Account (HRA) (If eligible, can be used to pay medical, vision, and/or dental coverage premiums and other out-of-pocket expenses) See the Leaving MSI FAQs or contact the Benefits Service Center. If you are eligible for retiree health care benefits, you and your eligible dependents must elect medical coverage through Mercer Marketplace 365 within 60 days either: when you first become eligible for retiree health care benefits (foregoing COBRA coverage at the beginning of your severance period); at the end of your subsidized severance period; during the open enrollment period for the Marketplace plans; or once your 18 months of coverage under COBRA have ended. in order to be eligible for the Motorola Solutions annual contribution to a HRA. You ll receive information from Mercer Marketplace 365 within 30 days of Motorola Solutions providing the Service Center with notification of your Separation Date. If you intend to waive your COBRA/severance benefits coverage, you should contact Mercer Marketplace 365 as soon as you know you will be leaving the company to start the medical plan enrollment process. Contact Mercer Marketplace 365 at least 30 days prior to your active/ COBRA benefits end date to set up an enrollment appointment with a Benefit Counselor. Enroll in medical coverage at least 15 days prior to your active/cobra benefits end date by contacting Mercer Marketplace 365. Medical coverage under the individual or family plan you elect through Mercer Marketplace 365 must become effective within 60 days of the date you first become eligible for the HRA (the first of the month following your active/cobra benefits end date) or you will lose eligibility for the MSI-funded HRA permanently. IN THE EVENT OF INVOLUNTARY SEPARATION IF YOU RE INVOLUNTARILY SEPARATED UNDER THE MOTOROLA SOLUTIONS U.S. INVOLUNTARY SEVERANCE PLAN (ISP), PLEASE NOTE THE FOLLOWING ADDITIONAL ACTIONS YOU LL NEED TO TAKE. ITEM YOUR OPTIONS PROCESS DEADLINE CONTACT INFORMATION General Release You decide if you want to sign the General Release in exchange for receiving the Severance Allowance and extension of medical and/or vision care benefits under the ISP. After you sign the General Release, you have 7 days in which to revoke it (15 days if you live or work in Minnesota). You ll be sent the General Release with the Older Workers Benefit Protection Act (OWBPA) list to your home address on file generally within one week after your employment ends. You have 49 days from receipt of the General Release to sign and return it to the HR department via email. If you want to revoke your General Release, you must submit an email or letter to the HR department within 7 days from the date you signed the release. Severance benefits (extension of medical and/or vision care coverage in effect as of your employment termination date) You may elect to continue medical and/or vision care coverage after your Severance Benefits end date through the remaining COBRA coverage period which is generally 18 months minus the months of your Severance Benefits coverage. Note : If you haven t already made a COBRA election, when you sign the General Release and don t revoke it, you ll be automatically enrolled for COBRA coverage and your Severance Benefits will be effective as of the first of the month following your employment termination date. After the General Release revocation period has passed, your Severance Benefits will be effective as of the first of the month following your employment termination date. Any COBRA coverage premiums you may have paid that are greater than the subsidized rates for Severance Benefits coverage will be credited to your account and you won t be billed until the premium credit has been exhausted. You have 49 days from receipt of the General Release to sign and return it to the HR department. Elect COBRA coverage within 60 days from receipt of the COBRA information by calling the Benefits Service Center. Send your Severance Benefits (or COBRA) payments, payable to Motorola Solutions, for receipt by the first of each month to: COBRA - Motorola Solutions P.O. Contact the HR team if you want to make changes to your direct deposit information. Schedule an orientation appointment with Right Management Career Transition Center. After the General Release revocation period has passed, your Severance Allowance will be paid in equal installments starting with the next available bi-weekly payroll cycle. All required taxes, withholdings and amounts owed to Motorola Solutions will be deducted from your severance payments. Contact Right Management. You have 49 days from receipt of the General Release to sign and return it to the HR team. Within 90 days of your Employment Termination Date.
