The annual maximum is $2,650. Employees with an HSA are only eligible to participate in a Limited Purpose Flexible Spending Account. Dependent Health Savings Account This benefit allows full time to save tax-free dollars to pay for eligible dependent care expenses. The annual maximum is $5,000. A Health Savings Accounts (HSA) is a tax exempt account set up exclusively for medical, dental and vision expenses for all members covered under a qualified High Deductible Health Plan (HDHP). The annual maximum contribution as regulated by the IRS is $3,450 for single, $6,900 for family with a $1,000 catch-up allowance (55 and older). Employer Contributions: All plans receive a $350 employer contribution DISABILITY Group Short Term Disability The short-term disability plan pays 60% of the employee s gross salary to a maximum of $1500 for a maximum of 26 weeks after a 7 day elimination period. No cost to. Group Long Term Disability The long-term disability plan pays 60% of the employee s gross monthly salary to a maximum of $10,000 per month after a 180 day elimination period. No cost to employee. LIFE AND ACCIDENTIAL DEATH & DISMEMBERMENT 3
Group Life Insurance All eligible are covered with $50,000 of term life insurance. No cost to employee. Group Accidental Death & Dismemberment All eligible are covered with $50,000 in AD&D coverage. No cost to employee. Voluntary Employee Life Insurance Voluntary term life insurance is available in increments of $10,000 with a minimum of $20,000 and a maximum of the lesser of $500,000 or 500%. Guarantee Issue is available only at the time of initial eligibility at $100,000 and at Open Enrollment. Voluntary AD&D Voluntary AD&D is available in increments of $10,000 with a minimum of $20,000 and a maximum of the lesser of $500,000 or 500%. Voluntary Dependent Life Insurance All eligible dependents of regular full time Voluntary spouse term life available in increments of $5,000 up to a maximum of $250,000 not to exceed 50% of the employee life. Guarantee Issue is available only at the time of initial eligibility at $25,000 and at Open Enrollment. Voluntary child term life available in the amounts of $5,000 or $10,000. 401k 401k All who are 21 years old and work a minimum of 500 service hours Employees may contribute up to the annual IRS maximum ($18,500). Employees age 50 or older can contribute an additional catch-up contribution ($6,000) as regulated by the IRS. Bearence will match 50% of the first 3% of employee s contributions. Employees are always 100% vested in employee contributions and profit sharing contributions. Employees are eligible to receive the match upon 6 months of employment. 4
WORK-LIFE BALANCE 37.5 Hour Work Week A regular full-time employee works 37.5 hours to support work-life balance. Flexible Work Schedules Upon Manager Approval We believe in providing a flexible work environment for our to meet the demands of a busy life. Work hours can vary within the parameters of normal business hours and approved by manager. Virtual Work All Upon Manager Approval Employees may have the opportunity to work virtually dependent of their role and job performance, with approval from manager. PTO Generous paid time off is available to all. Employees may carry over 40 hours per year with a maximum of 120 hours. Employees start at a pro-rated 17 days in their first year and earn 1 additional day for each year of service, up to the maximum of 28 days. Holidays Bearence recognizes 9 Company Holidays in addition to PTO. Bereavement Leave Paid absence of 3 days in the event of a death in the immediate family. Volunteer Time-Off Employees receive three days of volunteer time-off to give back to the community. EAP and immediate family and household members This free service provides professional, confidential assistance to help address issues that may interfere with well-being, job performance or health. 5

Insurance Referral Business Employee Referral Program All All non-producers who refer business to Bearence will receive a finder s fee based on the first year origination on all new Bearence accounts. This is based on the total annual commission generated by the account. This benefit only applies to who do not have sales responsibilities. All Some of the best in our company come to us as referrals. To acknowledge this contribution to our organization, we have a program paying $1,000 for each referral hired. Professional Designations With prior approval, Bearence will pay 100% of the courses and/or tests associated with obtaining the designation. Monetary rewards will be according to the schedule outlined in the policy. 6
