METHOD OF PAYMENT Payment for goods and services is expected at the time of arrangement and required prior to the service time and use of our merchandise. For your convenience, we accept cash, check, credit card (Visa, MasterCard, Discover, American Express), debit cards or an irrevocable insurance assignment. Our prices reflect a 3% cash discount. If making payment other than by cash or check, this 3% cash discount cannot be honored and the account total will be adjusted to reflect the actual non-discounted price. Examples: credit and debit cards, insurance assignments, government payments and vouchers, everything but cash and check. The goods and services herein are those that we provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items that you did not specifically ask for we will explain the reason in writing, on the statement we provide describing the funeral goods and services selected.. Federal and State agencies have determined that itemization of funeral and cremation services is essential for the benefit of the general public. To assure that consumers are not asked to pay for services they do not receive. In compliance with these rules, you will find a detailed itemization below. The consumer may choose the services that are appropriate for their needs. To help simplify arrangements, we have developed package plans that the majority of our families choose from. These options will provide additional savings beyond our already reasonable prices. As these options include various types of services, no credit for deletions is possible. If you ask us to provide additional items, charges may apply. Families who purchase a casket from our mortuary will be offered a package discount. It will include the service and items traditionally selected on a funeral service. You will note by the options that follow, the savings in selecting the package discounts in comparison to the itemized prices. MINIMUM SERVICES OF THE FUNERAL DIRECTOR AND STAFF: This fee for our basic services will be added to the total cost of funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains.) This fee includes the following: Personnel available 24 hours a day, 365 days a year to respond to initial call Arrangement conference Coordinating service plans with cemetery or crematory, or other parties involved in the final disposition of the deceased Clerical staff for completion of various forms and permits associated with a funeral Also covers overhead, including facility maintenance, equipment and inventory costs, insurance and administration expenses and governmental compliance $850.00 PREPARATION OF THE BODY: Except in certain cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. 2

FORWARDING REMAINS TO ANOTHER FUNERAL HOME: This charge includes removal of remains, minimum service of staff, necessary authorizations, embalming, use of preparation room and transportation to local airport or other place of shipment within a 50 mile radius (excluding shipping charges and shipping container). This charge does not include visitation of ceremonies. This charge applies to shipment within the United States only. $1,475.00 RECEIVING REMAINS FROM ANOTHER FUNERAL HOME: This charge includes minimum services of staff, transportation from shipping destination (within a 50 mile radius) and transportation of remains to cemetery or crematory. This charge does not include visitation or ceremonies. $1,150.00 DIRECT CREMATION: If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The cremation containers that we offer are constructed of heavy cardboard, hardboard or hardwood.. Our charges for direct cremation (without ceremony) include, removal of remains, transportation to crematory, refrigeration (up to 72 hours) and minimum services of staff. The crematory fee is included. Holiday, weekend, second attendant or after hours fees may apply as shown in itemized price list. 1.
