GREENSBORO HOUSING AUTHORITY Request for Proposals FOR LANDSCAPING, LAWN MAINTENANCE AND POTENTIAL SNOW REMOVAL SERVICES March 2015 Introduction The Greensboro Housing Authority (GHA) issues this Request for Proposals (RFP) from qualified contractors to provide labor, materials, and equipment to accomplish the work for landscaping and lawn maintenance for GHA s central office. Being the central office, it s vital this property maintains a constant pristine appearance which lends to the importance of high quality landscaping and lawn maintenance. The contractor(s) or individual(s) doing so is automatically agreeing to abide by all terms and conditions listed herein and with the following attachments. GHA is currently operating under the HUD mandated asset management system. Depending on the quotes received GHA intends to issue a contract for the contract period, July 1, 2015 through June 30, 2016, (12 months) with two one-year extension options. Scope of Services GHA is seeking landscaping and lawn maintenance services (Required). In addition GHA is seeking snow removal and ice control services if available (Not Required). The contractor shall be responsible to provide labor, materials, and equipment to accomplish the following work: (NOTE: All Contractors information must be submitted on the Landscaping, Lawn Maintenance and Snow Removal Services Proposal Information Forms ) 2

Turf Mowing 1. The lawn area to be mowed on a weekly basis from March 15 th to November 15 th, and as required the rest of the year to provide a neat and uniformly finished lawn. The grass to be maintained at a height of three to four inches. 2. All areas inaccessible to mowers will be trimmed using a string trimmer. All trimming will be at the same height of cut as the mowers. 3. All beds will be edged bi-weekly with a steel blade type edger. 4. All hard surfaces will be edged weekly with a steel blade type edger to give a clean crisp edge. All walks and curbs will be blown off to remove debris and grass clippings. 5. Property will be scouted for sticks and trash. Turf Care 1. The entire lawn area will be fertilized with a slow release fertilizer. 2. Soil test will be performed once each year to check nutrient levels. 3. The lawn areas will have a pre-emergent herbicide applied. Post-emergent herbicide will be applied as needed. a. The pre-emergent is applied to prevent the emergence of Crabgrass, Spurge and other annual weed seeds. This pre-emergent does not control weeds such as Nutsedge, Wild Violets, Dallisgrass, or any other perennial weeds. b. The post emergent is applied to control perennial weeds once they have emerged. This herbicide will control weeds that include Clover, Virginia Button weed, Dandelion, Plantain, and Ground Ivy. It will not control Nutsedge, Wild Violets, Dallisgrass or other perennial weed. A specific chemical may need to be sprayed and will be billed at an extra cost outside the contract. c. Following the emergence of Nutsedge, applications of a specialty herbicide can be applied to ensure its control. 4. The entire lawn area to be aerated in the fall using a hollow tine coring device. 5. The lawn area will be over-seeded yearly with turf-type tall fescue. The seed will be free of noxious weeds AND other crop seed as verified by the certified seed tag. 6. All bare spots larger than one square foot to be prepared, seeded and mulched. All seeding to be performed in conjunction with fertilization and aeration. Bed Maintenance - Shrubs and Small Trees (Ornamental Trees 4 Caliper 10 high or less) 1. All beds will be treated with pre-emergent herbicide for control of weeds. 2. All plant beds will be sprayed or weeded by hand to control weeds. 3. All hard surface cracks will be sprayed to treat for grass and weeds. 4. Soil test will be performed once each year to check nutrient levels. 3

5. Leaves will be removed five (5) times in the fall before re-mulching and once in the spring. (Site Foreman to determine schedule) 6. The removed leaves will be placed on the curb for city pick up. If the leaves are to be hauled off by an outside company there may be an additional fee. 7. All plant material and small trees will be pruned to encourage growth, to remove dead material, and to provide a uniform shape. a. Pruning will be performed through the use of hand pruners, loppers, and hand saws. All pruning devices will be maintained with sharp blades in order to provide clean cuts. b. Crape Myrtles will be pruned in a horticulturally correct manner. 8. All shrubs and small trees will be scouted and sprayed for insect and disease control as needed throughout the year. 9. All shrubs and small trees will be fertilized. Types and rates of application vary according to plant requirements. Dormant Season Visits 1. Property will be visited twice a month during the winter (December thru February). 2. All hard surfaces will be blown off as needed to remove debris. 3. Dormant season tasks will be performed (pruning, shrub fertilizers will be applied, scout for weeds, and remove debris, etc.) Lime Application (Not included be can be upon approval) 1. Lime will be added once per year based upon the recommendations from the soil test. Annual Color 230 sq. ft. 1. Spring and Fall annuals will be planted by June 15 th and November 15 th. 2. Tulips will be incorporated with the Fall planting. 3. Annuals beds will be properly prepared for maximum performance and then mulched with mini pine bark nuggets. 4. Annuals plantings will be thoroughly watered at time of installation. 5. Annual beds will be deadheaded to remove spent blooms and maintain plan vigor. Annual Color Containers (Not included but will be quoted at time of installation) 1. Center Pieces in containers will be changed out twice per year with the exception of Taxus, Cephelotaxus and Boxwood centerpieces. 2. Root pruning of the centerpieces to remain is not included in contract. 4

Mulching (Labor and materials for applying mulch is included in the contracted price) 1. All bed and hard edges will be trenched and beds replenished with pine needles once a year. 2. All edges will be tucked neatly with a straight edge tool to provide a neat appearance. 3. Plant material will be free of stray needles after re-mulching is complete. 4. Pine needles will be tucked under shrubs to completely mulch the bed. Turf Fungicides and Insecticides 1. Turf fungicides and insecticides are not included in this contract but can be applied at an additional cost upon approval of client. Compliance, Limited Liabilities & Scope of Work 1. All pesticides will be applied under the supervision of a licensed applicator in accordance with all state and federal regulations. 2. Maintenance contractor cannot be responsible for plants dying due to excessive water or lack of water (i.e. Acts of Nature) 3. This contract is limited to the scope of work that is directly described within. Any and all additional services that are requested and approved by the customer will be treated as an extra billable. Snow Clearing and Ice Control Requirements: Although desired services, snow clearing and ice control are not mandatory for RFP proposal submissions. All building entranceways, exits, and sidewalks shall be clear of accumulation of snow, ice or slush as designated by GHA management for the applicable location. Calcium chloride or a comparable product will be used to prevent icing on sidewalks. No sodium chloride or rock salt may be used on sidewalks. All chemicals shall be spread using a broadcast style spreader to prevent clumping. Contractor shall take care to prevent over-accumulation of chemical product, including the sweeping away of excess chemicals from doorways to maintain safe conditions. As part of clearing the sidewalks, contractor(s) will ensure that all fire hydrants are clear of snow during a snow event and will make sure there is access to the hydrants from both the parking lots and the sidewalks. Special care will be taken with respect to dumpster areas and any exterior storage facilities to ensure that access is available both during and after a snow event. Snow clearing shall be available on a continuous 24 hours per day and 7 day per week basis. The contractor will be required to regularly inspect properties during snow events or ice storms, without notice from GHA, to determine what snow clearing or ice control operations need to commence. The 5

contractor shall be required to begin plowing when three inches of snow has accumulated at a location and continue to plow at every three inch interval thereafter. The contractor shall be required to begin services within one hour of any service request from GHA. The contractor will be responsible to continue snow-clearing operations until bare pavement has been exposed and the snow event has finished. Additional clearing of parking areas may be necessary after a snow event has finished. It is the responsibility of the contractor to ensure the lot has been properly cleared. In the event precipitation intensifies and additional equipment is needed to maintain the integrity of the facility (specifically front end loader and articulated loaders to move or remove snow piles), the contractor will request authorization for the use of such equipment from GHA. Ice control and traction materials shall be available on a continuous 24 hours per day and 7 day per week basis. The contractor will be required to regularly inspect properties during Snow Events or ice storms, without notice from GHA, to determine what snow clearing or ice control operations need to commence. Contractor shall be required to begin services within one hour of any service request. During ice storms or icy conditioned outside of a snow event the vendor is required to apply ice control and traction materials to maintain safe, bare pavement conditions. Pricing: These bundled services will be based on a per "Snow Event" price. A "Snow Event" is defined as a period of continuous snowfall that results in significant accumulation with a five (5) hour separation between Snow Events before a new classification will be issued. Snow Event Pricing will be tiered based on total accumulation for that event. If service is available, submit two pricing structures. 1. Landscaping and Lawn Maintenance Services 2. Landscaping, Lawn Maintenance Services, Snow Clearing and Ice Control General Notes 1. Precautions will be taken to prevent harm to personnel and damage to property resulting from flying debris. Contractor is responsible for the cost of reimbursement or repairs. 2. Care to be taken to prevent damage to trees, shrubs, buildings and personal belongings while mowing. Contractor is responsible for cost of reimbursement or repairs 6

Locations The following the location included in this RFP. It shall be the responsibility of the Contractor to visit each referenced site to obtain the necessary site data and to verify all data relative to the existing site conditions necessary to prepare his RFP. GHA takes NO responsibility for the data needed and not included in the specifications as required for the submission of a realistic RFP. Annual pricing for the quotation work shall be submitted by site as provided in this RFP. No other combination of pricing will be accepted. Instructions and Notice for Proposals 1. General The instructions below provide guidance for the preparation and submission of proposals. Their purpose is to establish the requirements; format and content of proposals so that proposals are complete, contain all essential information and can be evaluated fairly. The following represents the extent of the instructions and conditions: 2. Inquiries Inquiries concerning the RFP should be submitted in writing to the issuing office: 3. Submission Date Greensboro Housing Authority 450 North Church Street Greensboro, North Carolina 27401 Attention: John Brittain, IT and Financial Controls Manager Email: jbrittain@gha-nc.org Proposals shall be submitted in original and three (3) copies to enable the Evaluation Committee to thoroughly evaluate the proposals and to arrive at a sound determination as to whether or not the contractor can meet the requirements set forth in this RFP. Proposals shall be submitted in sealed envelopes and marked Landscaping, Lawn Maintenance, and Snow Removal Proposal. All proposals shall be received no later than 2:00 p.m. local time on Friday, June 1, 2015, and submitted to the address identified in paragraph 2.1 above. Faxed or email proposals shall not be accepted. Since proposals are submitted in confidence and may contain proprietary information, they are not opened publicly. After the closing date, all proposals received are opened and evaluated in confidence. Proposals and any changes to those proposals are shown only to PHA personnel who have been authorized by the Contracting Officer as having a legitimate interest in them on the condition that information in the proposals will not be released to anyone who has not been so authorized. Faxed or emailed proposals shall not be accepted. All proposals shall be valid for 90 days. All proposal information must be submitted on the attached Landscaping and Lawn Maintenance Services for Central Maintenance Facility Proposal Information Forms. 4. Acknowledgment of Amendments Contractors shall acknowledge in their proposals receipt of amendment(s) to this RFP by signing the document on the acknowledgment line of the amendment. A contractor s failure to acknowledge an amendment may result in rejection of the offer. 8

5. Default by Contractor In the event of default by the successful contactor, GHA may procure the services specified from other sources. The contractor agrees to reimburse GHA for any additional costs incurred as a result of such default. 6. Awards GHA reserves the right to cancel this RFP or to reject, in whole or in part, any and all proposals received in response to this RFP, upon its determination that such cancellation or rejection is in the best interests of GHA. GHA further reserves the right to waive any minor informality s in any proposals received if it is in the public interest to do so. The decision as to who shall receive a contract award, or whether or not an award shall be made as a result of this RFP, shall be at the absolute, sole discretion of GHA. GHA reserves the right to and make award to a single contractor or to make awards to multiple contractors. 7. Complete and Accurate Submission A contractor s failure to provide accurate information in response to this RFP may disqualify the contractor from further participation in the landscaping and lawn maintenance services selection process. A proposal may be corrected, modified, or withdrawn, provided that the correction, modification, or request for withdrawal is made by the contractor in writing and is received at the place prior to the date and time designated in the RFP for final receipt of proposals. After such date and time, the contractor may not change any provision of its proposal in a manner prejudicial to the interests of GHA and/or fair competition. 8. Retention All proposals are the property of the Greensboro Housing Authority, shall be retained by GHA, and shall not be returned to the contractor. 9. Insurance Within ten (10) days after the award of the contract and prior to the commencement of work, the Contractor shall furnish GHA with evidence showing that the following insurance is in force and will cover all operations of the contract: Worker s Compensation Insurance in accordance with state law, for all employees working on the project. Contractors Liability in limits not less than $100,000/$300,000 bodily injury and $100,000 9

property damage, or as required by law. This insurance should protect the Contractor against claims for personal injury, death, and damage to the property of others. This insurance shall cover the use of all equipment and vehicles on the work site. Automobile Liability in limits not less than $100,000 bodily injury and $50,000 property damage. All insurance shall be carried with companies that are financially responsible. If such insurance is due to expire during the contract period, the Contractor shall not permit the coverage to lapse and shall furnish evidence of coverage to GHA. 10 Subcontractors Unless otherwise stated within the RFP documents, the successful contractor may not use any subcontractors to accomplish any portion of the services described within the RFP documents or the contract without the prior written permission of the GHA. The subcontractor shall not enter into any sublet work assigned and contracted through the prime contractor. The prime contractor shall be responsible for the compliance by all subcontractors. A breach of this compliance may be grounds for termination of the contract and for debarment as a contractor and subcontractor. 11. References At least three references, including company name, contract person, addresses and telephone number, ail addresses of public housing authorities and/or similar organizations for whom has been performed within the past three years, must be submitted as part of the proposal. (Refer to Section V, Proposal Information Form, page 11). 12. Required Documents 1. Certified statement that neither the firm nor members of the firm are debarred, suspended or otherwise prohibited from professional practice by any federal, state, or local oversight, regulatory or law enforcement authority. 2. Statement that the firm is financially sound and has financial resources sufficient to successfully execute this prospective project in the time frame outlined. 3. Statement that the firm operates in full compliance with all applicable civil rights and nondiscrimination statutes, executive orders, rules and regulations. 4. Statement evidencing firm s knowledge of federal, state and local codes, especially as they relate to this project. 5. Affidavit of non-collusion. 6. HUD Forms 5369, 5369-B and 5369-C (attached). 7. 9. U. S. Department of Labor Wage and Hour Division Form (WH-347 attached). Evaluation Process and Criteria All proposals will be evaluated based on the evaluation criteria outlined below. The Evaluation Committee will review all proposals according to the evaluation factors and points and determine which proposals are competitive and which are noncompetitive. Those proposals determined to be competitive will be further evaluated and scored. Negotiations may be held with one or more proposer with negotiations resulting in a contract for services. Evaluation Criteria 1. Experience of the contractor in all aspects of landscaping and lawn maintenance services on projects of similar size and scope. This criteria includes contractor s past performance for GHA whether it was satisfactory or unsatisfactory: 45 points 2. The contractor s capacity to handle this project in a timely manner: 35 points 3. Cost of Services: 20 points. The cost will not be the sole determinant for award of contract. Payment Procedure Greensboro Housing Authority has the following policy for payment on invoices. In order to be paid on schedule, the invoice must be received by GHA with all supporting documentation attached by 5:00 p.m. on the last working day of the month and will be paid on/or before the 15 th day of the following month. Invoices received later than the due date will not be paid until the following scheduled payment date. The GHA will procure the applicable goods or services by issuance of a contract. 11
