Assume a construction value for projects as outlined on the cost summary sheet in the attached Application for Preliminary Qualification of Bonds. Proposals shall be delivered in a sealed envelope or box addressed to the Contact Issuing Office above and titled: Request for Proposals Construction Management Services The digital copy of your proposal shall be sent to: lee.andrea@stantec.com 1 of 10

SELECTION CRITERIA The School District will review all proposals and select a minimum of three (3) companies to interview. Selection criteria will include but not be limited to the following criteria: Personnel dedicated to the project Current workload and ability to staff project Proposed work and staffing plan Communication Report accounting Teamwork Fee and expenses Bentley Community Schools reserves the right to reject any or all proposals received for reasons it deems to be in its best interest. DRAFT DESIGN CONSTRUCTION SCHEDULE The attached Draft Design/Construction Schedule was developed to meet district priorities and create a draw schedule as included in the Bond Application. The schedule is subject to change after review by the Owner/Architect/CM team. CONTRACT FORM AIA C132 2009 Edition Standard Form of Agreement between Owner and Construction Manager as Advisor and A232 2009 General Conditions of the Contract for Construction, Construction Manager as Advisor Edition. QUESTIONS I General 1. List your company address, ownership profile and main contact information 2. Provide a brief history of your company 3. List your in-house professional disciplines and number of staff in each discipline 4. Have you had litigation, arbitration or a claim filed against your company by an educational client or have you filed the same against an educational client in the last 8 years? If yes, explain the action. 5. Provide information on your general liability insurance and professional liability insurance policies. Include an insurance certificate. 6. List you current and projected workload currently under contract. 2 of 10

II Experience 1. Provide a brief project description of 5 similar projects by your company. Include Owner reference, project costs and time frame of each project. III Project Personnel 1. Provide a project organization chart. Include a one-page resume for each project team member highlighting relevant experience. Indicate current project assignments and durations for each. 2. Will you contractually guarantee your proposed Project Manager and Site Project Superintendent for the entire duration of the project? IV Project Approach 1. After reviewing the bond application, preliminary project schedule and touring the district facilities, outline the process you will take to deliver the project work. Include a preliminary work and staffing plan highlighting personnel hours and duration of each project. 2. Briefly describe how you will help maximize the district s investment and keep the district projects on budget and on schedule. 3. Briefly describe your process for providing quality control. 4. Describe how you will manage work on occupied facilities. Describe your safety procedures. 5. Describe your reporting procedures including project accounting and board updates. Include examples from similar projects. V Why your company? 1. Explain on one page why your firm is the best suited for Bentley Community Schools. VI Familial Disclosure 1. All proposals shall be accompanied by a sworn statement disclosing any familial relationship that exists between the owner(s) or any employee of the bidder and any member of the Board of Education of the School District or the Superintendent of the School District as attached. VII Iran Sanctions Act 1. All proposals shall be accompanied by a completed Iran Economic Sanctions Act Affidavit of compliance as attached. VIII Fee Proposal and Responsibility Matrix 1. EXHIBIT D I. AFFIDAVIT OF COMPLIANCE IRAN ECONOMIC SANCTIONS ACT II. Michigan Public Act No. 517 of 2012 The undersigned, the owner or authorized officer of the below-named construction manager ( CM ), pursuant to the compliance certification requirement provided in the Bentley Community Schools (the School District ) Request For Proposals - Construction Management Services (the RFP ), hereby certifies, represents and warrants that the Architect (including its officers, directors and employees) is not an Iran linked business within the meaning of the Iran Economic Sanctions Act, Michigan Public Act No. 517 of 2012 (the Act ), and that in the event CM is awarded a contract as a result of the aforementioned RFP, the CM will not become an Iran linked business at any time during the course of performing any services under the contract. The Construction Manager further acknowledges that any person who is found to have submitted a false certification is responsible for a civil penalty of not more than 250,000.00 or 2 times the amount of the contract or proposed contract for which the false certification was made, whichever is greater, the cost of the School District s investigation, and reasonable attorney fees, in addition to the fine. Moreover, any person who submitted a false certification shall be ineligible to bid on a request for proposal for three (3) years from the date the it is determined that the person has submitted the false certification.
