1UNDERSTAND YOUR HEALTH The first step to your employees understanding their health is to complete an online Health Review. This will provide them with their Vitality Age. Their Vitality Age may be higher or lower than their actual age and it serves as an indication of their overall health. One free Vitality Healthcheck per principal member, per plan year. All they need to do is to undertake the Vitality Healthcheck which will confirm to us that they have been tobacco and nicotine free for at least 12 months 10% to 50% cash back when your employees shop in-store and online at LloydsPharmacy 11 3. Flexible individual monthly membership. Joining fees are up to 50 for all clubs other than Classic Collection clubs, where joining fees are up to 150. This offer does not apply to Chiswick Riverside Virgin Active club. 4. Up to a maximum amount of 500 depending on Vitality status. 5. One product per lifetime of the plan. 6. Available to Vitality Plus members. 7. Excludes 3D films and tickets for any film at Vue Leicester Square. 8. Applies to handcrafted drinks only. 9. The discount applies to return economy flights to selected domestic and short haul destinations as advertised on the Fly Vitality website. Based on your Vitality status you could make up to a maximum of two discounted bookings through Fly Vitality per member per plan year. The discount does not apply to optional extras such as special baggage fees. Subject to a minimum stay depending on your day of departure. 10. Up to a maximum amount of 125 depending on Vitality status. 11. All employees are eligible for a 10% cash back. Employees with Vitality Plus are eligible for up to 50%, subject to the online Health Review being completed. Excluding nappies, infant formula milk, prescription charges (NHS or Private), mobile phone top ups and codeine products. Maximum 50 discountable spend per plan per calendar month.
