Full of choices, our benefit offerings allow you to select the plans that best suit your personal needs. Enrolling in Benefits You have 31 days to enroll from the date you become eligible. You will receive email notification of your personal enrollment deadline. If you do not enroll, you will be automatically enrolled in our default benefits. Enroll online at my.ascension.org. Enroll To enroll, you ll need: Your Ascension user ID and password to log in to the Ascension Portal Names, birth dates and social security numbers for any dependents you wish to enroll Your most recent tax return to apply for the Health Benefits Subsidy Your physician s name to assure he/she is in the Ascension network of providers and to complete your PCP designations Complete wellness activities at www.myascension-wellness.org Health Risk Questionnaire Wellness visit PCP designation Set up your Health Savings Account If you enroll in the HDHP, you ll receive followup instructions to set up your HSA and a debit card from ConnectYourCare, our health account manager. 1

Benefits Eligibility Associate benefit status Full-time, benefit-eligible Part-time, benefit-eligible Scheduled to work 36-40 hours per week (72-80 hours per pay period) 20-35.99 hours per week (40-71.99 hours per pay period) PRN associates and those scheduled to work less than 20 hours per week are only eligible for the voluntary Retirement Savings Program and the Employee Assistance Program. Dependents Eligible dependents include: Spouse Child younger than 26 years old (single or married, living with you or not) Unmarried child age 26 or older who became permanently and totally disabled before age 26 while covered under the plan AND: Receiving over half of his/her support from the associate or associate's spouse Eligible to be claimed as a dependent on associate's federal income tax return or associate's spouse's federal income tax return Legally Domiciled Beneficiary (LDB) You may designate coverage for either a spouse or one legally domiciled beneficiary not both in the medical, dental and vision plans. A legally domiciled beneficiary refers to a recipient of an associate's health insurance benefits who is not a spouse or dependent of the associate. This person may or may not be related to the associate and must have lived in the associate s household for at least six months. Proof of residency within the associate's household is required. 2016 Benefit Overview Benefit Health Protection Medical Coverage Options Options Available When you use providers in the Ascension Network: SmartHealth PPO 500/15 $500 individual deductible/15% coinsurance; $1,000 family deductible/15% coinsurance $15 primary care office visit copay $50 urgent care copay after deductible $2,200 (individual); $4,400 (family) out-of-pocket maximum SmartHealth HDHP 1300/10 $1,300 individual deductible/10% coinsurance; $2,600 family deductible/10% coinsurance Deductible applies to all eligible medical services; 10% coinsurance after deductible satisfied $50 urgent care copay after deductible $2,300 (individual); $4,600 (family) out-of-pocket maximum Prescription Drug Coverage Health Reimbursement Arrangement (HRA) Health Savings Account (HSA) SmartHealth Wellness Program All medical plan options automatically include prescription drug benefits administered by Cigna Retail and Specialty pharmacies available Available with the SmartHealth PPO medical plans; earned wellness rewards are contributed on your behalf; no associate contributions Available with the SmartHealth HDHP medical plans; employer makes annual base contributions of $500 individual/$1,000 family (if associate contributes at least $1 per pay period) and earned wellness rewards; associates may make pre-tax contributions Educational tools, fitness tracking device and services provided; earn up to $400 in rewards for participating in healthy activities; enroll at www.myascension-wellness.org 2

In addition, Ascension recognizes eight holidays: New Years, Martin Luther King, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas. 3

2016 Bi-weekly Payroll Deductions Both you and Ascension contribute to the total cost of your health benefits. This means that the bi-weekly payroll deductions (or premiums) for Ascension s SmartHealth medical plans are determined by your annual base pay. Associates who earn more, pay more. And for those who earn less, Ascension pays more of the premium. 4

SmartHealth Medical Plans: Designed By Ascension What makes SmartHealth medical plans unique is that they are designed by Ascension. We leverage our experience in all aspects of healthcare to design plans for our associates that are simply smart. And because we are not purchasing coverage from a traditional insurance plan, we have more control over how we manage costs. This means you have high-quality medical services through our own facilities and providers at affordable costs that are typically lower than traditional insurance rates. A Choice of Networks When you need medical care, SmartHealth gives you a choice of providers, and the level of coverage you receive is based on the provider network you choose. Typically, people have two network options through their employer plan either in-network or out-of-network. With SmartHealth, we offer three different levels so your choices are even greater: Ascension 1. 2. National Network 3. Network Our physicians and facilities offer the highest level of coverage at the lowest costs. To find an Ascension Network provider, view the physician directory at www.mysmarthealth.org These providers offer coverage comparable to other employers in-network coverage. To find a National Network physician or facility, visit the website listed on your medical plan ID card. Out-of-Network These providers are not contracted with the Ascension Network or the National Network. You ll pay a much larger portion (at least 50%) of the expenses, up to the out-ofpocket maximum. Reap Rewards with SmartHealth Wellness Program The SmartHealth Wellness Program is designed to reward associates for actions taken to achieve personal wellness goals. The program will help you identify areas of risk and provide resources to maintain or improve your health. Network Health and You As an Ascension associate, you have access to our care management program through Network Health. Network Health expands your benefits at no additional cost. Network Health provides: Assistance managing a chronic illness: If you are currently enrolled in SmartHealth and have a history of chronic disease or a catastrophic condition, you will be automatically enrolled in Network Health. Our goal is to help you manage chronic disease or an illness so that it doesn t lead to a high-risk condition. Examples of conditions we help manage include acute myocardial infarction (heart attack), cancer, cerebro-vascular accident (stroke), diabetes, heart failure, end stage renal disease, premature infancy, high-risk pregnancy or spinal cord injury. Case management support: You will be assigned a case manager that will coordinate care and assist with communication between you and your physicians. You will be able to call or email your case manager whenever you have health questions or concerns. Learn more: www.mysmarthealth.org Network Health 800-236-0208 Free Fitbit with HRQ Get started by taking the Health Risk Questionnaire (HRQ) at www.myascension-wellness.org. After completing the HRQ, you ll be registered for the program and can order a free Fitbit device. How to earn rewards If you enroll in a SmartHealth medical plan, you can earn up to $400 for your out-of-pocket healthcare expenses. Your rewards are deposited in your health account (HRA or HSA) three times a year. IMPORTANT: If you and your spouse are both associates, you ll each have to enroll in a SmartHealth medical plan (one of you can cover any dependents) to be eligible for your own $400 Complete a wellness visit with your physician and earn $160 in rewards. Earn up to an additional $240 in rewards by completing activities in three out of five health categories: clinical, educational, fitness, lifestyle and spirituality. Each activity you complete earns points, which are totaled and rewarded with a deposit to your health account in May, August and November. Your coverage ends when you leave employment or lose benefiteligible status. Medical, dental and vision coverage ends at the end of the month.
