ANNUAL BENEFITS ENROLLMENT: THE BASICS Annual Benefits Enrollment is your opportunity to elect/change your retiree medical coverage for 2014. Even if you re happy with your current coverage, it s important for you to realize that 2014 plan changes could affect you in terms of premiums and coverage. Be sure to read all the enrollment information carefully. If you don t enroll by midnight Central time on November 22, 2013, your current Retiree Medical Plan elections will carry over into 2014. As a reminder, the Retiree Dental Plan and Retiree Life Insurance Plan will not be offered for 2014. If you re enrolled in either of these plans, your coverage will continue through December 31, 2013. If you have Retiree Life Insurance, you ll receive an informational packet by mail explaining your portability and conversion options. Important: Cancellation of medical coverage is irrevocable for a surviving spouse or surviving dependents. You cannot cancel coverage and re-enroll at a later date. Some things to consider as you prepare for Annual Benefits Enrollment Read this Guide and all other enrollment materials carefully. Check out the Annual Benefits Enrollment web page at hr.phillips66.com. Think about your unique situation. Has anything changed since last year s enrollment? Will you or a covered dependent be eligible for Medicare in 2014? Medicare eligibility affects your choices. Compare medical plans to determine which offers the best value for you in 2014. Ask yourself: How much do you normally spend on health care? Would you rather pay less in premiums, or have a lower deductible and copays? Are you a California resident? You may be able to enroll in a Kaiser Permanente HMO. Do you know which carrier Aetna or BlueCross BlueShield (BCBS) serves your area? IT S EASY TO ENROLL Once you ve done your homework, you re ready to go. From November 1 22, you can enroll online or over the phone and it s easy either way. ONLINE AT YOUR BENEFITS RESOURCES (YBR): From the internet, go to http://resources.hewitt. com/phillips66 and enter your YBR user ID and password. BY TELEPHONE: Call the Benefits Center at (800) 965-4421, 8:00 a.m. to 6:00 p.m. Central time, Monday through Friday. Forgot your password? If you can t remember your YBR user ID or password, click Forgot User ID or Password at the YBR login. Or, you can call the Benefits Center and say I don t know when prompted to enter your password. 2014 Annual Benefits Enrollment: November 1 November 22 NEED ADDITIONAL DETAILS? Health care reform requires Phillips 66 to provide you with a Summary of Benefits and Coverage (SBC), available at http://hr.phillips66.com/ Benefits/Benefits-For-Tomorrow/ Retiree-Medical-And-Dental.aspx. It s a standardized document that highlights key provisions, limitations and exceptions. Note: If this is the first time you ve logged into YBR since May 30, 2013, you ll be required to complete a few security steps to further protect your personal information. Create a user ID (required only if you haven t previously set one up). Choose and answer a series of security questions (these will replace any security questions set up in the past). YOUR 2014 ANNUAL BENEFITS ENROLLMENT CHOICES YOUR PRE-65 RETIREE MEDICAL PLANS When it comes to comprehensive care and services, the Phillips 66 medical plans have got you covered! The Consumer and PPO Plans help pay for in- and out-of-network services, including preventive care, regular checkups and office visits, hospital stays, prescription drugs (retail and mail-order), mental health and substance abuse, and more. And they both offer extensive provider networks either Aetna or BCBS, depending on where you live. The main differences between the plans have to do with when and how you pay for your health care. With the Consumer Plan, your monthly premiums are lower, you have a higher annual deductible, and if you enroll in the Consumer Plan, you can also contribute to a Health Savings Account (HSA). With the PPO Plan, your monthly premiums are higher, but the annual deductible is lower and you have copays for some services. Note that if you are under age 65 and are eligible for Medicare, you can select the Medicareeligible PPO Plan or, if you live in California, the Kaiser Permanente HMO. Medicare will always be considered primary (meaning it pays first) and the plan you elect will coordinate with Medicare. Family members who are not eligible for Medicare may be covered by the Consumer Plan, PPO Plan or Kaiser Permanente HMO. Aetna or BlueCross BlueShield (BCBS) Phillips 66 has added BCBS as our health plan carrier in certain locations. If you re not sure whether your carrier is Aetna or BCBS, contact the Benefits Center at (800) 965-4421 or find a complete listing on hr.phillips66.com. Are Your Current Providers in the Network? If BlueCross BlueShield (BCBS) is the health plan carrier for your area, check to see if your current doctors are in the BCBS network. Go to www.bcbstx.com/phillips66, select the state you live in, type in your provider s name and location (the BCBS network for Phillips 66 participants will automatically be selected for you), then click on Search. If you have any additional questions about locating a provider in your area, call BCBS at (855) 594-4233. If Aetna is the health plan carrier for your area, check to see if your current doctors are in the Aetna network. Go to www.aetnanavigator.com, sign in and click on Find a Doctor, Pharmacy or Facility. For a list of providers in the Kaiser Permanente HMO network, go to www.kaiserpermanente.org, click on Locate our services, then click on Find doctors and locations. About the Health Savings Account (HSA) You must be enrolled in the Consumer Plan to open or contribute to an HSA. Your HSA is owned by you and the funds carry over from year to year. TURNING AGE 65 IN 2014? Medicare coverage for eligible participants begins on the first day of the month of their 65th birthday (or the first day of the prior month if their birthday is on the first day of the month). Pre-65 retiree medical coverage for an enrolled participant will end on the last day of the month before turning 65. (For example, if your birthday is October 15, you will have pre-65 coverage through September 30 and Medicare coverage will begin October 1.) If you or your eligible dependent elects Medicare supplemental coverage through UnitedHealthcare or, if you live in California, Kaiser Permanente HMO, any remaining credits in your Retiree Medical Account (RMA) may be used to reimburse the monthly premiums you paid for you or your eligible dependents. Enrollment information from UnitedHealthcare and Kaiser Permanente HMO is generally mailed approximately 60 days before a 65th birthday.
