APPLICATION FOR FINANCIAL ASSISTANCE BEFORE COMPLETING THE APPLICATION FORM, PLEASE READ THE NOTES BELOW CAREFULLY. When you have completed the application form, you should detach these notes and keep them for your reference. Anglian Water s Assistance Fund ( the fund ) is administered by Charis Grants Limited (Charis, Company No. 4762902) on behalf of Anglian Water Services Ltd., visit www.anglianwater.co.uk/awaf. A full list of grant making funds administered by Charis can be viewed online at www.charisgrants.com. In this form we/us means Charis. CAN THE ASSISTANCE FUND HELP YOU? The fund can consider helping you if you are in debt with your water and/or sewerage charges to Anglian or Hartlepool Water and you are a current domestic account holder of Anglian or Hartlepool Water. Only the account holder (person/persons named on the bill) can apply for help with their water and/or sewerage debts. If the account holder is unable to complete the application, please explain why someone else is doing so on their behalf. The account holder MUST sign the declaration. You are not eligible to apply to the fund if you are already having water and/or sewerage charges deducted from your benefits via the Water Direct scheme. WHAT ARE THE OBJECTIVES OF ANGLIAN WATER S ASSISTANCE FUND? To administer a programme of assistance aimed at reducing water and sewerage debt for customers experiencing poverty and hardship throughout the region. To help people recover from the burden of debt and become financially more stable. To make a positive long-term difference to an applicant s financial situation. HOW CAN THE FUND HELP? The fund may be able to help you to clear arrears of domestic water and sewerage charges by offering a number of solutions. Anglian or Hartlepool Water can provide you with advice on making sure you are on the most appropriate tariff and offer water efficiency advice. PLEASE NOTE: The fund is unable to consider helping with any items other than with an individual s water and/or sewerage debt to Anglian Water and Hartlepool Water. SENDING IN YOUR APPLICATION Once completed, the form should be returned to: Anglian Water Assistance Fund, PO Box 42, Peterborough, PE3 8XH Please check you have put the correct postage on the envelope. Incorrect postage will result in delays in our receiving and dealing with your application. FILLING IN THE APPLICATION FORM Please complete all sections of the application form. If a section is not applicable, please insert N/A. Some information must be provided, otherwise your application cannot be assessed. These fields are highlighted. When filling in the application form, it is very important that you give us as much information as possible PLEASE NOTE THAT THE DECISION OF THE FUND IS FINAL AND WE DO NOT OPERATE AN APPEALS PROCESS. about your circumstances and that you send us proof of your income. Please supply copies of all supporting documents as we do not return originals. If you need help with the form you should contact a local advice centre such as a Citizens Advice Bureau. In section 5, you can give your authority for us to contact an agency, relative or friend with queries regarding your application. Please note, if a provisional award is made, the fund will need to correspond with you directly from this time. Please do not forget to sign the declaration in section 11 if you do not sign it, we cannot assess your application. If you are a domestic customer of British Gas or EDF Energy and have gas or electricity debts, you may be eligible for help from other Trust Funds. Please complete section 8 if we can help we will write to tell you this. HOW WE DEAL WITH YOUR APPLICATION As soon as we receive your application it is logged and the assessment process begins. No hold will be placed on your account until the assessment process is complete and we would strongly recommend you contact Anglian or Hartlepool Water to make arrangements to pay whilst your application is being considered. If you are claiming certain benefits, namely, Income Support, income-based Job Seeker s Allowance, Employment and Support Allowance or Pension Credit, Anglian Water will initially apply to the Department of Works and Pensions (DWP) on your behalf for the Water Direct scheme which will allow your debts to Anglian Water to be cleared by making small regular instalments out of your benefits direct to Anglian Water. This has the following advantages for you: Making payment is easy and free. You do not have to remember to make payments. Your will no longer be chased to clear your debt to Anglian Water. Your application will be put on hold until an outcome from the DWP is known. If you are eligible for Water Direct you will be informed in writing by the fund. If you are not eligible for Water Direct your application will be assessed. If it appears that the fund is able to help with your water and/or sewerage debts we will write to tell you that a provisional award has been made. Your debt will not be cleared immediately. During a specified period (between six and twelve months depending on your level of debt) following your provisional award you will be required to demonstrate your commitment to improving your own personal financial stability, i.e. the intention to pay current and future costs on an ongoing basis and avoid getting into debt in the future. Your commitment would be demonstrated by: Discussing with Anglian or Hartlepool Water how you might be able to reduce your bills including whether any of their reduced tariffs or metering would be beneficial for you. The fund does recognise that this may not be appropriate for all applicants. Anglian or Hartlepool Water will send you information to help you decide. Payment of current/ongoing usage to Anglian or Hartlepool Water while your application is being dealt with. This will immediately show the fund that you are currently financially stable. You are strongly advised to contact Anglian or Hartlepool Water to discuss ongoing payment arrangements. Direct Debit payments are recommended as they are a more convenient way to make payments and can help with monthly budgeting. Seeking help from expert money advisers/other relevant free support agencies to help with future budgeting if you have not already done so. Following the specified period (six or twelve months) after your provisional award has been made, if it can be seen that you have taken steps towards achieving financial sustainability, you will receive a letter confirming your award and a payment to Anglian or Hartlepool Water will be made to clear your debt. The fund reserves the right to withdraw the application at any point during the provisional award period if inadequate payments have been made. If we cannot help with a water and/or sewerage award we will write to tell you. Please note that the fund does not give particular reasons why an award is not made. As the fund receives many applications for financial assistance it may take a few weeks to process your application. If we think that other organisations might be of interest to you, we may provide you with information about their work or share your details with them so that they can contact you about their services. Your information may also be used for research and analysis purposes by other organisations. You may receive letters, emails, telephone calls or a home visit as part of our assessment process. APPLYING TO THE FUND AGAIN If you receive an award you cannot apply to the fund again. If you do not receive an award, you can apply again after six months from the date of the letter informing you of the fund s decision. This also applies if you have previously applied to the Anglian Water Trust Fund.

Filling in Section 10 The next page is where you must explain to us how a payment from the fund will help you. In deciding whether to help you with water and/or sewerage debts, the fund will: Need to know how you got into difficulties with the particular debts. You should give as much detail as possible about how long you have had problems in paying the bills and why. Need to be sure that, if the fund does help you, you will be able to manage to pay your bills from Anglian or Hartlepool Water in the future. This means that if, in section 7 you have more money going out than you have coming in, you should explain how you think you will be able to keep up with the bills in future. Need to know about any particular hardship within your household e.g. chronic illness, disability, bereavement. Where possible evidence must be provided. SECTION 12 Filling in the application form Please make sure you attach items on the checklist below. All evidence must be less than 3 months old. Please provide copies of documents listed as we do not return originals. Proof of income: You must include proof with your application confirming the following income figures which you have shown in section 7 of the application: The current amount of net wages received by you and any partner. ( Net wages means the pay you receive after all employer s deductions such as tax and National Insurance, have been subtracted. People often refer to net wages as take home pay.) The current amount of all benefits and pensions received by you and any partner. The current amount of all Child Maintenance/Support received by you and any partner. Wages: Photocopies of three recent consecutive wage slips for you and for any partner. If you or your partner cannot provide wage slips, please get a letter from the employer giving your recent average net pay. Self employed applicants should send in recent three line accounts or another document confirming net income. Benefits and Pensions: Photocopies of bank statements showing current benefit, tax credit or pension payments, or Photocopies of recent letters from the Department for Work and Pensions (DWP), HM Revenue and Customs (HMRC) or pension provider showing amounts currently received. If none of these are available, please obtain a letter from the Benefits Agency, Tax Credit Office or pension provider confirming the amount of your benefit, tax credit or pension entitlement. Child Maintenance/Support: a copy of a recent letter from the Child Support Agency or a recent bank statement showing the amount payable to you or your partner or another letter or document confirming the current amount being paid. Please note that we cannot assess your application unless all the necessary proof of income is sent in with the application. Evidence of disability or other illness If you are relying on a disability or illness as proof of hardship or need, please attach some independent evidence which confirms your medical condition: for example, a copy of a current sick certificate, a recent prescription or a letter from your GP/consultant to a third party confirming your condition. Please note that the fund is unable to meet the cost of obtaining medical evidence. PAYING ONGOING WATER AND/OR SEWERAGE CHARGES On receipt of your application, if you are identified as receiving certain benefits (see How we deal with your application section), Anglian Water will apply to the DWP on your behalf for the Water Direct scheme. This will allow small regular instalments to be paid out of your benefits direct to Anglian Water to clear your debts, plus make a contribution to current charges. If you are not eligible for the Water Direct scheme, please indicate how you want to pay for your future water charges.
