OneHealthyOlympus Dear Olympus Employee and Family, BIG CHANGES ARE COMING TO THE OLYMPUS WELLNESS PROGRAM. Employees who wish to enroll in Olympus sponsored medical coverage for 2017 will have a choice: If you choose to participate in the 2016 Wellness program, you and your covered spouse or domestic partner will be required to meet certain health goals to earn a premium credit for 2017. If you choose not to participate in the program, you and your covered spouse or domestic partner will still be able to enroll in Olympus medical benefits, but at an increased cost to you next year. THIS IS A TRANSITION YEAR. All benefit eligible employees will still be able to earn a cash incentive for 2016, but will also be working towards a 2017 medical premium credit. Premium Credits will range from $250 to $800, annually. In order to execute our business strategies, Olympus needs engaged and productive employees. Living a healthy lifestyle is critical to employee engagement and productivity. At its core, the Olympus Wellness Program is designed to motivate and enable our employees to make healthier life choices that will benefit themselves and their families. Why the change? Each year, the cost for our Company to provide healthcare to employees increases by 6% to 10%. There are many factors that drive this cost increase, including higher rates demanded by providers, the use of more expensive technology, and more access to care by patients. Claim costs paid by Olympus are consistently rising. Even the healthiest person gets sick occasionally, and we don t want to charge more to an employee just because he or she needs medical care. We chose an approach that will lessen the impact on employees that make healthy choices and strive to manage their own health. What can I do? Please read through the information on the following pages to discover the requirements for participating in the Olympus Wellness Program. 2016 PROGRAM To earn the 2016 cash incentive and the 2017 medical premium credit, you must complete the Biometrics Screening (onsite, via LabCorp, or your physician) and have your results fall within the Healthy Ranges (see page 6 for a definition of Healthy Ranges) and complete Two (2) Preventative Exams. If you do not meet the 3 out of 5 criteria of Healthy Ranges, you still have an opportunity to earn the incentive by completing Additional Activities. All Olympus employees are eligible to participate in the program even if you are not covered by an Olympus sponsored medical plan. 2016 Wellness Incentive (Cash) Similar to the Wellness Programs in prior years REQUIREMENTS Biometrics Screening/Lab Work Achieve Healthy Ranges OR Additional Activities EMPLOYEE INCENTIVE Please Note: Employees must be in an active status at the time of the wellness incentive distribution to be eligible for an incentive. Deposits to your HSA are tax free. Payments made via Payroll are taxable. The 2016 incentives will be included in the employee s paycheck at the end of the year. If you would like your incentive deposited into your HSA account, you must update your Incentive Distribution Method selection in the Wellness Portal under My Profile>Profile. Be sure to click Update Personal Information to save your choice. If you do not make a selection, incentives will automatically be included in the employee's paycheck. IMPORTANT! Your 2016 Wellness Incentive results will determine your premium credit effective January 1, 2017. If you are within Healthy Ranges and have two (2) Preventative Visits Receive a Wellness Premium Credit of $400 for employee and $400 for spouse or domestic partner. A total of $800 is possible if both you and your spouse or domestic partner are within healthy ranges. If you are not within Healthy Ranges Receive a Wellness Premium Credit of $250 for employee and $250 for a spouse or domestic partner that completes the Additional Activities and two (2) Preventative Visits. The 2017 Wellness Incentive Premium Credit Program is not applicable to the unionized members of the Norwalk facility. REQUIREMENTS Just as in previous years, spouses and domestic partners are eligible to participate. In order to qualify, you must complete all the requirements below by December 1, 2016. STEP 1 WELLNESS PORTAL To track your participation and to become eligible for any incentive, employees and spouses must have a Wellworks For You Portal account. If you already have an account, you do not need to create a new one. New Member? Register Now! 1. Go to www.wellworksforyoulogin.com 2. Click the link to create an account as a New Member 3. Enter your COMPANY ID: Location ID Bartlett: 10263 Breinigsville: 10109 Brooklyn Park: 10264 Center Valley: 10109 Field: 10262 Houston: 10990 Kennewick: 10989 4. Create a Username and Password 5. Accept the terms of the Consent Form 6. Enter the required Personal Information Miami: 10269 Norwalk: 10265 OCTA: 11018 Puerto Rico: 11012 San Jose: 10268 Southborough: 10266 State College: 10991 Waltham: 10984 Linking Spouse/Domestic Partner Accounts Spouse/domestic partner incentives will be combined with the employee incentive. Please make sure your accounts are linked in the Portal so both incentives can be distributed. This can be done on the Wellness Portal on the spouse/domestic partner's Portal account. See instructions below: 1. The spouse/domestic partner must have their own account in the Wellness Portal. Use the instructions above to create an account. 2. Once the spouse/domestic partner logs into their Portal, go to My Profile>Profile 3. Make sure the Account Type is set to Spouse/Domestic Partner 4. Enter the username of the Olympus employee in the Primary Username field 5. Click Update Personal Information to save your changes 4 P a g e

STEP 2 BIOMETRICS SCREENING/LAB WORK As a requirement for this year s program, you must obtain a biometrics screening. Options for obtaining a screening are below. Please note: You will only receive credit for completing one (1) of these options, even if you complete all three. REMINDER: You will be required to fast prior to this screening. If you forget to fast, we recommend you reschedule. A. Onsite Screening Olympus will once again be offering a FREE 36- panel Blood Draw Screening Exam for employees, spouses, and domestic partners at the following locations: Bartlett, Center Valley, Brooklyn Park, Miami, Norwalk, and San Jose. Due to state regulations, we are unable to offer onsite screenings in Massachusetts. B. LabCorp Voucher If you are unable to attend one of the onsite screening events, you will have the option to receive the FREE screening at a local LabCorp facility. For more information, login to your Wellness Portal and go to My Profile>Programs and Events>LabCorp Option 2016. C. Physician Results Form Visit your Primary Care Physician to obtain lab work. Print out the Physician Results form located on the Wellness Portal and take it to your doctor. All metrics must be collected to receive credit. Your first submission of results is your final submission of results. If you want to preview your results, we suggest you visit your doctor or lab, at your own cost, prior to your elected screening method. This will give you an opportunity to improve your final results, if necessary. 5 P a g e

STEP 3 - HEALTHY RANGES OR ADDITIONAL ACTIVITIES Meet 3 out of 5 healthy ranges OR earn 30 points by completing additional activities. A. Healthy Ranges Once you have completed the Biometrics/Lab Work requirement (either onsite, via LabCorp, or the Physician Results Form), your results from the screening will be used to determine healthy ranges. Please note, you cannot re-submit results if your initial measurements do not fall within healthy ranges. METRIC HDL Cholesterol Blood Pressure Waist Circumference/ Prenatal Visits Form Fasting Glucose Healthy Triglycerides HEALTHY RANGE Men: greater than or equal to 40 mg/dl Women: greater than or equal to 50 mg/dl Systolic: less than or equal to 129 Diastolic: less than or equal to 84 Men: less than or equal to 40 inches Women: less than or equal to 35 inches Less than or equal to 126 mg/dl Less than or equal to 150 mg/dl B. Additional Activities If you do not meet three (3) out of the five (5) healthy ranges, you must earn a minimum of 30 points by completing additional activities listed below. See additional details for each activity under My Profile>Programs and Events. ACTIVITY Learning Series - Nutrition Complete this 6-week Learning Series under the Learning tab. Learning Series Weight Management Complete this 6-week Learning Series under the Learning tab. Know Your Number Assessment with Health Coaching After completing your Biometrics/Lab Work requirement, complete the Know Your Number questionnaire on the Wellness Portal. Once you receive your results report, you must complete four (4) health coaching sessions by December 1, 2016 in order to earn credit for this activity. Physical Activity Log Track your physical activity on the Physical Activity Log located on the Portal. You must track 75 activities by 12/1/16. Yearly Walking Activity Track 1,500,000 steps in the Wellness Portal by 12/1/16. POINTS 6 P a g e

STEP 4 - TWO (2) PREVENTATIVE EXAMS PLEASE NOTE: There may be a charge associated with the exams listed below. Contact your insurance carrier prior to your appointment. You must complete at least two (2) preventative exams from the list below to earn credit for this requirement. Download the Proof of Preventative Visit form located on the Wellness Portal, print it out, and bring it to your doctor s visit. Exams must take place between December 2, 2015 and December 1, 2016. Annual Physical with Prostate exam Primary Care Physician* Dental Mammogram Vision OB/GYN annual exam Dermatology exam (body Colonoscopy scan/skin check) *If you complete your annual physical with your physician at the same time you receive lab work, both forms (Proof of Preventative Visit and Physician Results form) must be submitted to earn credit for both requirements. NEW! 2017 Wellness Incentive (per pay Premium Credit) IMPORTANT! Your results from 2016 will determine your premium discount effective January 1, 2017. ADDITIONAL INFO Programs and Activities Events are listed on your personal Wellness Portal (www.wellworksforyoulogin.com) under My Profile>Programs and Events. To view extended details about each program, select Click Here for Details next to the event title. Obtaining Documents for Completion All required forms are located on the Wellness Portal under My Profile>Wellness Toolbox. Employees can download and print all PDF forms for completion. Viewing Participation Wondering how many points you have earned throughout the year? 1. Log into your Wellness Portal (www.wellworksforyoulogin.com) 2. Go to the My Profile tab at the top of the page and click ResultsNow Please allow at least 4 weeks for participation to be updated in the Portal after completing an activity. If there are any discrepancies with your participation, please contact info@wellworksforyou.com. Your health plan is committed to helping you achieve your best health. Rewards for participating in a wellness program are available to all employees. If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means.
