Important Health Care Information for US Employees Preparing to Begin an International Assignment CIGNA Global Health Benefits Enrollment Medical, dental and vision coverage is provided through CIGNA Global. A CIGNA Welcome Kit and Enrollment Form are available to you through Cartus Online. You will need to return the completed enrollment form to your Cartus consultant prior to beginning your assignment. Your Cartus consultant will send the enrollment form to CIGNA once your assignment start date is known. Your CIGNA coverage will be effective on your actual assignment start date. Identification Cards If you are in the 90% plan with Cigna you will have two ID cards one from Cigna and one from Express Scripts. Below is information showing which card you should you use where. Overseas for Medical & Rx Cigna card In the U.S. for Medical Cigna card In U.S. for Rx - - Express Scripts card Once your enrollment is complete, it will take approximately 7-10 days to receive the ID cards in the mail. Starting the effective start date of your coverage under CIGNA, and Express Scripts (if you chose the 90% plan), you can print cards as follows: Instructions for printing CIGNA ID cards: Log on to the Cigna Global Health Benefits web portal at www.cignaenvoy.com Select the blue box that says VIEW/PRINT ID CARDS from the home screen Then click the green View/Print ID card link and a new window will open with your ID cards 1
Select the Print icon at the top of the page to print your cards Instructions for printing Express Scripts ID cards: Go to EatonBenefits.com on the internet or, from JOE, select HR Services > My Benefits (in the list on the left side of the page) > Go to Eatonbenefits.com (in the Eaton Benefits section of the page) Click on the Express Scripts link in the left and from the list that drops down click ID cards. Then follow the prompts. Pre-departure Medical & Prescription Drug Questions How to obtain an extended supply of a maintenance medication Contact Express Scripts at 800-792-9596, explain that you are starting an expatriate assignment and ask for an authorization to receive an extended supply of your medication(s). Please be sure to be prepared to provide Express Scripts with the following information: Date you are leaving the country, names of the prescriptions of which you need an extended supply of medication and the best number you can be reached. Express Scripts will contact Eaton to obtain the authorization and you will be notified once this step is complete. Once the authorizations are loaded Express Scripts can provide you with the cost of your order (if obtaining though the mail order pharmacy) or quote you approximate cost at retail. At this time, you will be asked for payment of your order if you are not already enrolled in an auto payment process and have not yet met your family out-of-pocket maximum While you are waiting for the authorization to be approved, contact your provider to obtain a new script. NOTE: The exact amount needing to be dispensed should be indicated on the script (no refills). For example, if the script is for 1 year, it should read dispense one-year (and not 90 day supply x 3 refills). Certain medications, such as controlled substances, cannot be dispensed for extended periods. Once you receive confirmation of the authorization your can submit your new prescriptions at mail or retail to obtain your medication (or your doctor can fax it directly to Express Scripts). Please remember to allow ample time for this process and for obtaining your medications. Credit for Prior Plan Deductibles and Out-of-Pocket Maximums If your expatriate assignment starts mid-year, you are entitled to credit under the Cigna International medical and dental plans for the amount applied toward your U.S medical or dental plan s deductibles and out-of-pocket maximums. To receive this credit, you must obtain a copy of your most recent Anthem and Express Scripts plans Explanation of Benefits (EOB) document(s) showing the current amounts applied to your deductibles or out-of-pocket maximums and submit to CIGNA via the Cigna Global Health Benefits web portal at www.cignaenvoy.com Remember this information cannot be applied before you are actually enrolled in the CIGNA plan. 2
CIGNA will apply the amounts documented on the EOBs to deductibles and/or out-of-pocket maximums under Cigna plan for the remainder of the year. This information will also be shared with Express Scripts if you elect the Cigna 90% option. It takes a few weeks time for this information to be reflected under your new plan. Ensure you provide your EOBs to CIGNA via the Cigna Global Health Benefits web portal at www.cignaenvoy.com as soon as possible to avoid delays. If you have any questions or experience problems receiving your credit, please contact Cigna customer service 24 hours a day / 7 days a week at 1.800.441.2668 Health Savings and Reimbursement Account Questions How your choice of medical plan option can affect your Health Savings (HSA) and Reimbursement Account participation If you are participating in an Eaton medical plan option before your transfer to expatriate assignment and elect to participate in the CIGNA Healthcare 80 option as an expatriate, your participation in the Health Savings Account (HSA) and/or Dental/Vision Reimbursement Account (if applicable) will automatically terminate effective with the date of your transfer since you will no longer be eligible for these plans. NOTE: If you chose to have an HSA account, your address on file at Fidelity must be a U.S. address. The P.O. Box mail forwarding address that Eaton provides for you is acceptable. You cannot have a foreign address on file with Fidelity if you have an HSA account or a brokerage account. 3
How to pay for qualified medical expenses with your HSA account You have several options when paying for qualified medical expenses. 1. Fidelity HSA debit card. Fidelity HSA debit card is designed to work in foreign countries that accept Visa for qualified medical expenses. There are Merchant Code restrictions on the card that could prevent a transaction from occurring. With that said, here is the information you need in order to use your debit card if it is accepted in the country you reside: The HSA debit card is a signature-based card and does not require a personal identification number (PIN). When used, funds come directly from the cash available in your Fidelity HSA, and payment details show up on your account statement making it easy to track the qualified medical expenses paid from your account. If you requested an HSA debit card at the time you opened your Fidelity HSA, you will automatically receive it in approximately two weeks. If not, and you would like to request one, go to Fidelity.com, click on the Customer Service link, Find a Form, Alphabetical List, and then H to download the HSA Debit Card Application for yourself or the HSA Supplemental Debit Card Application to obtain an HSA debit card for your spouse or dependent. 2. Fidelity HSA checkbook. Like the debit card, checks are paid directly from the cash available in your Fidelity HSA, and records are included on your account statement. To request an HSA checkbook, go to Fidelity.com, click on the Customer Service link, Find a Form, Alphabetical List, and then H to download the HAS Checkwriting Form. Additional fees may apply. 3. Pay out of pocket and reimburse yourself. If you pay out of pocket for a qualified medical expense, you have the option to be reimbursed from your HSA at any time and without penalty by: Requesting a check for yourself electronically through Fidelity BillPay for Health Savings Accounts Writing yourself a check using your HSA checkbook Contacting Fidelity to request a distribution check be mailed to you or to transfer money from your HSA to another account NOTE: Remember that debit cards, checkbooks and all other material related to your HSA account will be sent to the address Fidelity has on file for you, which is a US based address.
