DOCUMENT PRODUCTION REQUEST LIST Please check the appropriate box below each request to indicate your response: 1. Copies of Income Tax Returns for the past three (3) years. 2. Income tax records for the current tax year, and a copy of any estimated income tax return filed for the current year. 3. A copy of any document prepared by your employer which shows your pay and any deductions taken from your pay for the most recent pay period. 4. All bank books and savings books for accounts in institutions representing accounts in which you have any legal or equitable interests. 5. All personal and business bank statements including canceled checks for the twelve (12) months immediately preceding the date of this request. 6. Copies of any orders under which you pay or receive child support for any person who you claim as a dependent. 7.
