U Care Voucher Application Criteria Before applying, please read the following eligibility criteria and take note of all required supporting documents. Important: The value of U Stretch and Back-To-School vouchers are determined based on final donations received from our patrons. Successful applicants will know the vouchers values at the point of voucher collection from 19 November 18 onwards. 1. ELIGIBILITY CRITERIA: 1.1 Member must have at least 6 months of continuous paid-up union membership as of July and must not be in arrears. 1.2 Member WITHOUT Dependents staying in the Same Household in Singapore - Personal Gross Monthly Income + must be $1,500 and below, inclusive of overtime and allowances. 1.3 Member WITH Dependents staying in the Same Household in Singapore - Total Monthly Gross Household Income + must be $3,400 and below, inclusive of overtime and allowances; OR Per Capita Income must be $850 and below if gross household income exceeds $3,400. 1.4 If you are applying for BTS Vouchers: Member s child/children must be attending school in 2019 at an eligible educational institution in Singapore as follows: Kindergarten 1 or 2; Government / Government-Aided / Independent Schools (Primary or Secondary) under the Ministry of Education (MOE); Junior Colleges; Institute of Technical Education; Millennia Institute; Polytechnics; OR Special Education Schools Note: Graduating students (e.g. final year junior college students) who will not be attending school in Year 2019 will not be eligible for the Back-to-School Vouchers. 1 P a g e MED UCare Online Guide
2. SUPPORTING DOCUMENTS REQUIRED 2.1 If Member is Employed: - Photocopy of recent payslip (dated year ) OR - CPF Statement (showing contribution history for the past 3 months) If Member is Self-Employed or Works Part-Time: - Notice of Assessment from IRAS (Year of Assessment ) OR - CPF Statement (showing contribution history for the past 3 months). If no CPF account, a written letter from CPF board is required If Member is Unemployed - CPF Statement (showing contribution history for the past 3 months). If no CPF account, a written letter from CPF board is required AND - Photocopy of Spouse s payslip or CPF Statement (showing contribution history for the past 3 months) if Member is Married 2.2 If you are applying for BTS Vouchers: - Photocopy of Child/Children s Birth Certificate(s)^ For foreign birth certificate(s), please also submit Certificate of Singapore Citizenship / Re-entry Permit AND Student Pass. ^ Note: If member has previously submitted photocopy of his/her child/children s birth certificates for Back-to-School Voucher Programme in 2012 or later, member need not submit the birth certificates again. 3. IMPORTANT INFORMATION Applicant may be asked to submit other supporting documents if necessary, for verification and audit purposes. U Care Voucher Online Application Guide The U Care System (UCS) is an online system that aims to improve the efficiency and processes in the administration of U Care Voucher Programmes. The UCS allows union members to submit an online application for the voucher programme. It also stores union members applications and supporting documents in the system, thereby providing ease of retrieval and evaluation, and you can access this website 24/7. Step 3: Apply for U Care Voucher Programme 1. Click E-Services located at the top of the page 2. Select Care & Share Services 3. Click Read More 1 2 3 4. Ensure Payment is Up to Date 7. If you have outstanding membership fees, please make payment before applying: a) Click on Make Payment displayed on the error message, and a new window will open. b) Complete your payment via Credit Card/Debit Card (VISA/Mastercard) and you can proceed to apply online on the same day. c) Otherwise, you may pay via AXS stations (NETS) and apply online on the same day. Update Your Particulars & Employment 8. Your membership particulars will be automatically shown. Please check your particulars and employment information. You may update these fields before proceeding. 8 Click here to update your Home Address, Contact No, Marital Status. A new window will open. Please note that in UCS: Updated particulars will be reflected immediately. Press F5 button or click Refresh icon updated particulars. (at the webpage address bar) to see your Updated employment details will be reflected after 3 working days. You may still proceed with your application. Add or Edit Family Members (Dependent) 9. Any previous records of your immediate family members will be automatically reflected in UCS. 10. Click on Add to update your family members particulars or additional member and indicate their gross monthly income and occupation. 91 Click here to add new family members. Dependent Receiving Back-To-School Voucher 11. If your dependent is receiving Back-To-School voucher, click Yes: You need to provide his/her education level and school name. Upload the following documents: a) Relationship Supporting Document (e.g. Birth Certificate): Mandatory for all b) Supporting Document (Student Ezlink card, student card): only for foreign students Repeat to add more dependents Please note that: If you have previously uploaded the children s Birth Certificates for Back-to-School Voucher Programme in 2012 or later, you are not required to upload again. The maximum file size for the supporting documents is 1 megabyte (MB) each. The file format should either in:.pdf,.jpg,.jpeg,.png,.gif All content in the uploaded document is sharp and clearly visible. Update Your Income & Select Collection Venue 12. Key in your Gross Monthly Income and upload your latest supporting income documents If you are Employed: Photocopy of recent payslip (dated year ) or CPF Statement (showing Contribution History for past 3 months) If you are Self-Employed or Work Part-Time: Notice of Assessment from IRAS (Year of Assessment ) or CPF Statement (showing Contribution History for past 3 months). Submitting Your Application (Complete Information) 15. Ensure that all the mandatory fields are filled and complete mandatory documents are uploaded. 16. Check the Declaration check box to declare that all the information submitted is true before clicking on Next. 15 16 17. Please note that the application cannot be submitted if: (a) Any mandatory field is not filled or (b) Any mandatory supporting documents is not uploaded, or (c) If the Declaration check box is not checked, or (d) The length of membership does not meet the selected programme s minimum membership tenure criterion, or (d) There is overdue membership fees 18. 19. Click Submit Application to proceed with the submission. If you need to make any change, press Back to edit. 19 After Application Submission 1. Once you have submitted the application, you will receive an auto reply message. 2. You may check the application status via the UCS s My Application function which will show Processing. 3. Submitting Your Application (Incomplete Information/Documents) 1. If the information and supporting documents are not available during your application, you can click Save as draft button to complete your application at a later time before the application closing date. 2. You can check the status of your application via the UCS My Application function, which will show Draft status. Please note: 3. Under Save as draft option, you can edit the information in the application form. 4. The application can be saved as a draft only if the following conditions are met: (a) At least one programme has been selected (b) If a dependent has been added, the dependent s NRIC/FIN has been keyed in. 5. The application details will be kept in the system until the application closing date, 15 July. 6. Edit Draft Application 1. To edit your application, move cursor to the navigation bar on the right and click on View/Edit. 2. All the applications which you have applied will be displayed. To edit an application, click on Edit link 13 P a g e MED UCare Online Guide
3. Proceed to update/edit the application. 4. Click on Next to proceed with the application submission. 5. Once you have clicked on Submit Application, no changes can be made at the previous page. 6. Click Back if you need to make any changes before submission. 7. If you decided to submit the application later, click on Save as Draft.
