Starting a regular contribution plan (and Direct Debit Request) Newcastle Permanent Superannuation Plan Superannuation Division When you invest in the Superannuation Division of the Plan, the Trustee will also accept regular contributions on your behalf. This form allows you, your employer or your spouse, to make regular contributions by direct debit from a nominated bank account, building society, credit union or cash management trust account. This form also allows you to change your regular contributions. The Trustee of the Plan is the holder of Direct Debit User ID Number 395740 under which it is authorised to debit your nominated bank account in accordance with this Direct Debit Request. Note that you must have provided your Tax File Number (TFN) to the Plan before you can make after-tax contributions. If your TFN has not been provided, these contributions cannot be accepted by the Plan and will be returned to you. Employer and salary sacrifice contributions, and contributions for which you are claiming a tax deduction will be accepted by the Plan, however these will be subject to additional contributions tax unless your TFN is provided. You should refer to the current Product Disclosure Statement for the Superannuation Division of the Newcastle Permanent Superannuation Plan (PDS). If you do not provide the personal information requested, we may not be able to manage your superannuation. We may disclose your information to various organisations in order to manage your super, including your employer, the fund s administrator, our professional advisors, insurers, our related companies which provide services or products relevant to the provision of your super, any relevant government authority that requires your personal information to be disclosed, and our other service providers used to assist with managing your super. In managing your super your personal information will be disclosed to service providers in another country, most likely to our administrator s processing centre in India. Our Privacy Policy lists all other relevant offshore locations. Our Privacy Policy sets out in more detail how we deal with your personal information and who you can talk to if you wish to access and seek correction of the information we hold about you. Direct debit request service agreement 1. Debiting your account 1.1 By signing a direct debit request, you have authorised us to arrange for funds to be debited from your account. You should refer to the direct debit request and this agreement for the terms of the arrangement between you and us. You should not sign a direct debit request (or if you have already done so prior to receipt of this agreement, you should let us know that you wish it to be revoked) unless you have read and understood this agreement and agree to be bound by its terms. It is your responsibility to ensure that the authorisation given to draw on the nominated bank account is identical to the signing instructions which apply to that account. 1.2 We will only arrange for funds to be debited from your account as authorised in the direct debit request. 1.3 If the debit day falls on a day that is not a business day we will direct your financial institution to debit your account the following business day. 2. Changes by us 2.1 We may vary any details of this agreement or a direct debit request at any time by giving you at least fourteen (14) days notice. 3. Changes by you 3.1 Subject to 3.2 and 3.3, you may change the arrangements under the direct debit request by contacting us (see below for contact details), or through your nominated financial institution. 3.2 If you wish to stop or defer a debit payment you must notify us in writing at least ten (10) business days before the next debit day. This notice should be given to us in the first instance even if you make this change through your nominated financial institution. 3.3 You may also cancel your authority for us to debit your account at any time by giving us the (10) business days notice in writing, before the next debit day. Your obligations 4.1 It is your responsibility to ensure that there are sufficient clear funds available in your account to allow a debit payment to be made in accordance with the direct debit request. 4.2 If there are insufficient clear funds in your account to meet a debit payment: (a) you may be charged a fee and/or interest by your financial institution; (b) you must arrange for the debit payment to be made by another method or arrange for sufficient clear funds to be in your account by an agreed time so that we can process the debit payment. 4.3 You should check your account statement to verify that the amounts debited from your account are correct. 4.4 Advise us if the account nominated by you, to receive the direct debit is altered, transferred or closed. 4.5 You should also be aware of your obligations under clause 6 of this agreement. 5.Dispute 5.1 If you believe that there has been an error in debiting your account you should notify us directly by contacting us (see below for contact details). 5.2 If we conclude as a result of our investigations that your account has been incorrectly debited we will respond to your query by requesting your financial institution to adjust your account (including interest and charges) accordingly. We will also notify you in writing of the results of our investigation. 5.3 If we conclude as a result of our investigations that your account has not been incorrectly debited we will respond to your query by providing you with the results of our investigation. 5.4 Any queries you may have about an error made in debiting your account should be directed to us in the first instance so that we can attempt to resolve the matter between you and us. If we cannot resolve the matter you can still refer it to your financial institution which will obtain details from you of the disputed transaction and may lodge a claim on your behalf. 6. Accounts 6.1 You should check: (a) with your financial institution whether direct debiting is available from your account as direct debiting is not available on all accounts offered by financial institutions; (b) your account details which you have provided to us are correct by checking them against a recent account statement; and (c) with your financial institution before completing the direct debit request if you have any queries about how to complete the direct debit request. 6.2 You should ensure you comply with clause 6.1 above, as we shall not be liable to you for any loss or damage you may suffer as a result of you providing us with incorrect details of your account (or other incorrect information) in the direct debit request. You will indemnify us should we suffer any loss or damage as a result of incorrect details or information provided by you. This indemnity is a continuing obligation, separate and independent from your obligations and survives termination of this agreement. 7. Frequency options 7.1 Direct debits can be effected: Monthly - The direct debit is made on 10th or 20th of each month. Quarterly - The direct debit is made on 10th or 20th for the months of September, December, March, and June of each year.
8. Confidentiality 8.1 We will keep any information (including your account details) in your direct debit request confidential. We will make reasonable efforts to keep any such information that we have about you secure and to ensure that any of our employees or agents who have access to information about you do not make any unauthorised use, modification, reproduction or disclosure of that information. 8.2 We will only disclose information that we have about you: (a) to the extent specifically required by law; or (b) for the purpose of this agreement (including disclosing information in connection with any query or claim). 9 Notice 9.1 If you wish to notify us in writing about anything relating to this agreement you should write to The Plan Administrator, Newcastle Permanent Superannuation Plan, GPO Box 4303, Melbourne Vic 3001 9.2 We will notify you by sending a notice in the ordinary post to the address you have given us in the direct debit request. 9.2 Any notice will be deemed to have been received five business days after it is posted. If there are more than one, you means each of them separately and every two or more of them jointly.
