Graded Death Benefit Underwriting The application and underwriting process is simplified. There are no medical exams, no blood or urine tests, and no attending physician's statements (APS). If your client can answer NO to the questions on the application and the answers are not contradicted by the MIB, Pharmacy data, and meet height and weight criteria, they may qualify for the graded death benefit products. Administrative Procedures Contracting/Licensing Information Vector One Agents listed on the Vector One report are not eligible for contracting. Licensing Fees All initial licensing fees are paid by the Company except for the state insurance department examination fee or fees for license renewals. For current examination fee rates, telephone NWL or the individual state insurance departments. Welcome Letters When the appointment of license has been received and an agent control number has been assigned, a letter will be sent to the appropriate person and manager. The letter will include a file copy of the contract, the agent control number, and any specific compliance information relative to the state of residence of the appointee. The Welcome Letter will provide a PIN to provide access to the National Western Life website. NWL Website National Western Life s website address is www.nationalwesternlife.com The Company has developed an extensive interactive information system for our agents to use as a valuable resource. Go to the company and click on US English. BEACON is accessible with your Agent Number and the same PIN that is now used with our voice-response system. BEACON will give you access information regarding NWL News, Products, Pending Application Status, Active Policy Information, Commission Information, Agency Bulletins, Contracts, Illustration Software, Information on contest current production standing and upcoming incentive trips. Much of this information is confidential and proprietary to National Western Life. Access under your agent number and PIN number is limited to your use and use by others are not authorized. Commission Payments Electronic Funds Transfer of Commission Payments (EFT) Commissions payable to the agent can be automatically deposited into the agent s bank or savings account via electronic funds transfer (EFT). To request EFT, the agent should complete the EFT Information Form (SA-8786), attach a voided check to the form, and forward them to the Commission Department. Commissions not paid by EFT will be paid by Company check. Frequency of Commissions Daily commissions are calculated every business day. Selection of this option allows the agent to enjoy the combined features of weekly and monthly payments PLUS the ability to receive additional commission payments any other business day for which the agent has a credit balance of at least $500. Monthly commissions are calculated at the end of each month and include all commission transactions for the month. In order for a commission check or EFT to be issued, the commission account must have a minimum credit balance of $50. Semimonthly commissions are calculated on the 15th of the month and at the end of the month. In order for a commission check or EFT to be issued, the commission account must have a minimum credit balance of $50. Weekly commissions are calculated at the end of each week. A commission statement is prepared on the first business day of each week (usually Mondays) to include all commission transactions for the previous week. The commission statement for the week will provide detailed information only about the prior week s transactions. A weekly commission statement is prepared and check or EFT is forwarded when the agent has a credit balance of at least $50 on the first business day of the week. All active agents will be paid earned commissions on a weekly basis unless the Company is notified by the agent in writing that the commission payment frequency should be changed. Graded Death Benefit Term & Whole Life policy fee is commissionable. Graded Death Benefit commissions are paid As Earned. The company will forward commission payments according to the schedule selected by the agent. (a) Commissions forwarded daily include commissions for premiums processed on issued business as of the prior business day. All debits to the account will be deducted, and the total commission due the agent must be at least $500. An account statement explaining the commission payment is available through the Company website. (b) Commissions forwarded on Mondays include commissions for premiums processed on issued business during Monday-Friday of the prior week. All debits to the account will be deducted, and the total commission due the agent must be at least $50. An account statement explaining the commission payment is available through the website. (c) Commissions forwarded semimonthly are based on commissions calculated as of the 15th of the month and at the month-end. Commissions due active agents must be at least $50 in order for payment to be forwarded. An account statement explaining the commission payment is available through the website. (d) Commissions at month-end are forwarded by the 7th of the following month and will include commissions not previously forwarded to the agent during the month. The month-end commission statement lists all commission entries for the entire month. Commission due active agents must be at least $50 in order for payment to be forwarded. The statement can be viewed on the Company website. Split Commission Payments If two or more agents, licensed and contracted with the Company, secure an application jointly, the credit for such business shall be equally divided (unless otherwise shown on the application) among those whose names appear on the application. Each agent will be paid commissions and receive production credits only on his/her share of the business. The Company will resolve cases where claims to commissions are disputed or questioned. (ESP), a national consumer-reporting agency, to accurately and confidentially verify application information. Elite Sales Processing 1-888-348-9320 Monday - Thursday 8:00 am - 9:00 pm (CST) Friday 8:00 am - 5:00 pm (CST) Please have applicant s physician s name and contact information ready as this will be asked by the interviewer. After the sale is made and the application is completed, Agent and Applicant call ESP: 1. Agent and applicant agree that applicant will apply for insurance with NWL (Review all underwriting guidelines in this guide prior to sale). 2. Agent completes application including the Medical/MIB/HIPAA authorizations, informs applicant or applicants state fraud statement, informs applicant of the MIB Pre-Notice, FCRA Notice and gives applicant notices. 3. Agent calls ESP and talks with interviewer who obtains agent information, applicant information and asks the agent for permission to record the interview information. (Client not seen business: Agent and client call ESP together.) 4. Interviewer then asks the agent to place the applicant on the telephone. 5. Interviewer greets applicant and thanks them for placing their trust with National Western Life and asks applicant for permission to record the conversation and authorization for MIB and IntelliScript (Prescription History). If No, interviewer will ask to speak with agent. The interviewer informs the agent that if the company cannot obtain an authorization then the applicant will not qualify for rules based program. (Client not seen business: interviewer asks for applicant s permission to record the conversation with agent present.) 6. Interviewer will ask applicant identifying data for recording and re-ask the permission question so that it is on the recording. The Medical/HIPAA/MIB/IntelliScript authorization is read to applicant and when finished applicant is asked if they agree with authorization. If Yes, MIB/IntelliScript search is initiated so that the interviewer will have the result back during the interview. If No, application cannot be processed. 7. Interviewer will ask the applicant if they have been given the MIB Pre-Notice and FCRA Notice. If no, application cannot be processed. Agent is responsible to give applicant the notices prior to the calling ESP. 8. Interviewer will then ask all of the medical questions and additional information on the application. If any medical question is a Yes, does not meet height and weight criteria, or either the MIB or IntelliScript is not within NWL guidelines, interviewer will thank the applicant and inform the applicant that is all of the questions and ask to speak with the agent. Interviewer informs agent that application does not qualify based on a Yes answer to the question or confidential information. Interviewer cannot tell the agent what was discussed with applicant or confidential information from MIB and IntelliScript based on privacy laws. Agent is told to submit application to NWL and a letter will be sent to application explaining the decline. 9. If all questions are No in the medical history section, meet height and weight requirements, and the MIB and IntelliScript are within NWL guidelines the application is accepted. The interviewer will inform the applicant that is all of the questions and will ask to speak to the agent. Interviewer will inform agent that applicant has qualified. 10. If accepted, interviewer completes agent check list with agent. Page 11

11. Agent is informed to send application to NWL if accepted or declined. (Direct to consumer-client not seen business: Agent must send application and all state required forms to applicant to obtain all outstanding signatures by secure-email, mail or fax. Agent must obtain all paperwork from proposed insured to send to NWL.) 12. If not accepted, agent informs the applicant the application has not been accepted. The application should be submitted to NWL to complete processing and for record keeping purposes. 13. ESP notifies NWL of the decision and forwards interview to home office for processing. 14.
