This General Price List reflects Emmerson-Bartlett Memorial Chapels commitment to provide consumer information to each family we are privileged to serve. We know it is important that you understand the alternatives available to you as well as the specific costs for each of those services. We are confident you will find the information you need within this price list. However, should you have any questions, we welcome your inquiry. Please allow us to give you full information to assist you in your family s decisions. Our personal service to you also includes assistance with insurance claims, Veteran s benefits, insurance processing, social security assistance, letters of verifications to employers and airlines. We also arrange for appointments of cemetery property, procurement of certified death certificates, notification to newspapers, both local and out-ofarea, and other special services to your family. Our family service staff and funeral directors provide personal services after the funeral, at no additional cost to the family. We are honored by the confidence you have placed in us and are committed to your service and satisfaction. PAYMENT OPTIONS Emmerson-Bartlett Memorial Chapels has established a uniform payment policy to serve all families fairly. This payment policy enable us to control our cost to all the families we serve and requires that all financial arrangements be completed one full day before the service or the use of merchandise. The following are acceptable payment options: 1. Pre-arranged funeral plan through Emmerson-Bartlett Memorial Chapel or other valid assignable insurance upon verification. 2. Cash, personal check, cashier check or money order. 3. Visa, MasterCard, or Discover credit cards. Note: Payment for Pre-arrangements will be paid by check, or cashier s check only.

PROFESSIONAL SERVICE PACKAGES Many of the families we serve tend to select the two types of funeral arrangements and services outlined below. While you are free to arrange services on an itemized basis, the service plans shown below are designated to simplify the funeral arrangement process and offer a cost savings..the options include most everything that is needed for the type of service described.no credits for deletions are possible. These discounted options are directed only on the Professional Service fees, and available only to families who purchase all Merchandise from Emmerson Bartlett Memorial Chapel. Please Note: Certain items such as sales tax, cash advances, Saturday/Holiday Service fees, Vaults, are not included in the Option packages below. There will be additional charges. OPTION A ~ TRADITIONAL SERVICE This package includes Funeral Director and Administrative staff; professional staff for coordination and direction of the Funeral Service; procuring and documenting the death certificate, and other necessary state and local documents; transfer of deceased from place of death to funeral home; embalming/alternative care of remains; use of facilities for viewing during normal business hours; use of facilities for funeral services or transport to Church; funeral coach, and utility/ flower vehicle. This package does not include a Vigil Service or DVD memorial. The Goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items that you may ask us to buy for you, such as musicians, cemetery or crematory services, flowers, and newspaper notices. The prices for these items will be shown on your bill or the statement describing the funeral goods and services you selected. In some cases we charge you for our services in buying these items or arranging these services. The survivor of the deceased who is handling the funeral arrangements, or the responsible party, is entitled to receive, prior to the drafting of any contract, a copy of any agreement that has been signed and paid for, in full or in part, by or on behalf of the deceased, and that is in the possession of the funeral establishment. BASIC SERVICES OF FUNERAL DIRECTOR, STAFF & OVERHEAD......... $ 1,895 Availability of funeral director and staff to respond 24 hours a day; administrative services including preparation of death certificate, securing doctor s signature, and filing with proper authorities; securing burial or cremation authorization and/or transit permit locally; clerical administration and preparation of various forms of including Social Security and Veterans Administration; planning, supervision and follow-up on all details of arrangement conference with cemetery, crematory, clergy, or other third parties; and shelter of remains. In addition, this fee includes a proportionate share of our basic overhead costs. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already in our charges for cremations, immediate burials, and forwarding or receiving remains.) EMBALMING.... $ 650 Except in certain special cases, embalming is not required by law. Embalming may be necessary; however, if you select certain funeral arrangements, such as funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it such as direct cremation or immediate burial. (California state law requires embalming or refrigeration of deceased if held for more that 24 hours after time of death.) USE OF RECEPTION ROOM Our reception room is a value to our families. It helps keep family members and friends together before or after the funeral, memorial service, and/or viewing.
