During Open Enrollment 2018 all participants must login to PeopleSoft and make elections. This Guide is an overview and reference for details of the changes coming in 2018. It will notify you of actions required during Open Enrollment, and help you make informed choices. To avoid the last-minute Open Enrollment rush, make your benefits decisions early. WHAT YOU NEED TO KNOW FOR 2018 You must log in to PeopleSoft and make your elections by November 3, 2017 at 5pm. CHANGE TO PLAN YEAR Changing to calendar year in 2018 2017 Benefit Year ends December 31, 2017 PRORATED PLAN NUMBERS Deductibles, co-insurance and plan maximums were prorated in 2017 and are based on a 12 month plan year in 2018 NEW VENDORS Cigna Dental replaces EMI as the county s dental administrator COSTS INCREASE Premiums will increase by 7% for the medical plans No change to dental premium WHY WE NEED YOUR EMAIL ADDRESS Salt Lake County personnel are unable to make elections on your behalf, you must login to PeopleSoft and make changes or elect Cigna Dental to have dental coverage. You must elect Cigna Dental to have dental coverage. You will receive an email from county Information Technology with a information about your secure login. Visit benefits.slco.org for a link to PeopleSoft. It s available online so you can enroll from any internet connection. If you don t have an email address or the ability to enroll on your own, come to the county for a computer lab session for help. 2

TAKE ACTION & MAKE CHOICES PREPARE Review this Benefits Guide Attend a Benefits Fair or Cigna Dental presentation during Benefits Fairs on October 24th or November 1 Attend a Cigna Dental presentation October 24th and November 1 You must enroll in Cigna Dental coverage this year, your medical coverage will continue unless you make a change REVIEW Review the Cigna Dental network at www.cigna.com. You ll get the most annual benefit and pay the lowest cost for care if you use a Cigna DPPO Advantage provider. You must elect Cigna Dental to have dental coverage in 2018 Visit www.slcohdhp.org for an overview of the county s High Deductible Plan. Even those not actively working can save with an HSA. If you re on a HDHP and still under age 65 without other coverage, you have a unique option to save pretaxed funds for future medical expenses. Studies show not only should you save for retirement expenses, but those who have saved separately for medical costs benefit in their retirement years. ENROLL THROUGH PEOPLESOFT BY FRIDAY, NOVEMBER 3 AT 5:00PM If you provided your email address, you ll receive an email from the county Information Technology Department (IT). Follow the instructions on page 11 to obtain your password link. If you did not provide an email address, contact county IT at 385-468-0700 for help getting your secure login ID. You will be asked for your Employee ID Number or other personal identifying information. 3

RETIREE MEDICAL PLAN OVERVIEW Salt Lake County offers PEHP Summit or SelectHealth SelectMed Plus or (SelectCare Plus if you re living outside Utah) between retirement and Medicare eligibility. Choose between the High Deductible Plan (HDHP) or the Traditional PPO. County retirees have benefited from lower cost premium and tax benefits of High Deductible plans for several years. HSA contributions aren t only for active employees, and you can benefit from the benefits of an HSA. Forbes calls your HSA the crown jewel of tax deferred accounts. You save state and federal tax on contributions, your funds grow tax-free, and as long as you spend your funds on qualified expenses, they re not taxed when they re spent. Most institutions that administer your HSA (HealthEquity, Optum Bank, etc.) offer a competitive lineup of investment options if you re interested in taking a more active role in your funds. Some restrictions apply, depending on when you become eligible for Medicare. You re encouraged to talk to your HSA institution or a tax professional for information about your HSA and when you may wish to stop making contributions. Your coverage will automatically terminate when you become Medicare eligible. For qualified retirees, PEHP offers a Medicare Supplement plan and you can have the premiums drafted directly from your retirement check. Salt Lake County subsidizes retiree premiums based on your years of county service. If you elect coverage for your dependents, you pay the full cost of premium. You may be eligible for a subsidized premium for PEHP s Medicare Supplement plan when you turn age 65. If your birthday is the first day of the month, Medicare begins the first day of the month prior to your 65th birthday. If your birthday is the 2nd through the last day of the month, Medicare begins the first day of the month you turn age 65. PEHP recommends you contact Medicare and PEHP several months before you turn age 65, so your benefits are in place when you need them. Salt Lake County Aging and Adult Services is an excellent resource for your Medicare questions. 5

Your prescription drug costs count towards your medical plan deductible and out-of-pocket maximum. You pay the total discounted cost of the prescription drug until you have met your plan deductible, then you pay: 30 day retail supply: Tier 1: $10 co-pay Tier 2: 25% coinsurance with $25 minimum and $75 maximum Tier 3: 50% coinsurance with $50 minimum and $100 maximum 90 day mail order supply: Tier 1: $20 co-pay Tier 2: 25% coinsurance with $50 minimum and $150 maximum Tier 3: 50% coinsurance with $100 minimum and $200 maximum Specialty and injectable medications: 20% coinsurance Maintenance medications (for HDHP only): Deductible is waived for certain drugs that are identified as preventive or treat chronic conditions such as cholesterol and high blood pressure. COMPLETE YOUR OPEN ENROLLMENT ONLINE COMPLETE YOUR ENROLLMENT BY FRIDAY, NOVEMBER 3 AT 5pm Update your PeopleSoft login and password by following the steps below. If you provided your email address: 1. Go to https://mypassword.slco.org. 2. Select Reset My Password. 3. Enter the email address you provided to the county. 4. Shortly after entering your email address, you ll receive an email with a link to set your password. The link you receive is time sensitive and for one-time use. 5. After clicking the link in the email, create a password. The county system requires 11 characters with upper and lower case, numbers, and special characters. 6. If you forget your password, use the website in step 1 and follow the steps above to reset it. If you didn t provide your email address, contact county IT at 385-468-0700. County IT will validate you re you by asking you to provide your Employee ID Number (EIN). If you re not a county employee but have coverage because of your spouse, you ll have a 9 digit POI number that begins with C. If you don t know your EIN or POI number, you ll need to provide other personal identifying information to IT, like your date of birth, address, or social security number. Next, go to https://pshcm.slcounty.org to complete your Open Enrollment: 1. Enter your username and password (from the above steps) 2. From the Main Menu > Self Service > Benefits > Benefits Enrollment 3. Click Select in the Open Benefit Events box 4. Review your elections and who s covered on your plan. Remember, to continue with dental you must enroll in Cigna dental. 5. Submit NEED HELP? For password or log in issues, call county IT at 385-468-0700. For questions about benefits or your enrollment call Salt Lake County Benefits at 385-468-0580. 11

COMPLETE YOUR OPEN ENROLLMENT ONLINE WHEN YOUR LIFE CHANGES If you experience a life event such as marriage or a divorce, it is important that you make the available changes to your health benefits coverage at the right time to be sure you receive coverage when you are eligible and most important when you need it. When experiencing a life-changing event, you have 31 days from the date of the event to make changes to your benefits. If you experience a life event, call the Benefits Department at 385-468-0580. MAKE SURE YOUR FAMILY MEMBERS ARE ELIGIBLE FOR COVERAGE Salt Lake County is committed to providing comprehensive, affordable health care coverage for retirees and eligible dependents. Salt Lake County s health plans have specific requirements for who can be covered as an eligible dependent.
