This user guide provides direction on how to use the MCV Associated Physicians website. The website may be used by all employees, including new hires/first time enrollees, current Fidelity Investments account participants, or TIAA account participants. All screen examples in this User Guide are for illustrative purposes only.

MCV Associated Physicians 403(b) Plan and 40(a) Retirement Plans online access Your MCV Associated Physicians 403(b) Plan and 40(a) Retirement Plans are important benefits, so you need the right information, resources, and support to help you make decisions with confidence. Use this guide as a resource to help you take an active role in your retirement planning regardless of which retirement provider or investment elections you choose. The MCV Associated Physicians website provides plan features, investment information, and tools and resources. You are not required to log on to view MCV Associated Physicians plan details. In addition, you can enroll in the plan, update your current contributions, and modify your retirement providers (Fidelity and/or TIAA) once you log on to your NetBenefits account. You will want to bookmark this site to access your Fidelity and/or TIAA account. All employees can access the MCV Associated Physicians website at www.netbenefits.com/mcv. 4

Enroll in the plan Follow these steps if you are a new hire or would like to enroll in the plan. You will be prompted to create a NetBenefits username and password to access your plan account. Employees hired on or after July, 07, will be automatically enrolled in the VCU Health System Savings Plan 457(b) at a contribution rate of %. Follow the Standard Enrollment steps to opt-out or to increase your contribution rate. Click Enroll from the Pre-Login home page and follow the prompts to establish a NetBenefits username and password. 3 Once registered, you have two enrollment choices: Begin Easy Enroll to get started or continue with Standard Enrollment. Easy Enroll (enroll in 60 seconds) A predetermined deferral percentage amount and investment election is directed to Fidelity as the retirement provider. Standard Enrollment (customized choices) You can choose your deferral percentage amount, retirement provider (Fidelity and/or TIAA) and your investment options. If you have questions about the MCV Associated Physicians website or need assistance with your NetBenefits account, please call the Retirement Service Center at 800-343-0860. Service Representatives are available Monday through Friday, 8 a.m. to midnight ET. For questions regarding TIAA accounts, please call the TIAA Service Center at 800-84-5. TIAA Representatives are available Monday through Friday, 8 a.m. to 0 p.m., or Saturday, 9 a.m. to 6 p.m. ET. 5

Easy Enroll Select from one of the pre-set Starter Package contribution amounts. Change Annual Increase amount, if desired. 3 Estimate paycheck impact using the Take Home Pay Calculator. Employees can see an estimate of their paycheck impact for each package. 4 Click Enroll Now. You re enrolled! Print and save your confirmation. Once enrolled, you can name beneficiaries for your account. 6

Standard Enrollment Click Begin to get started. Set contributions. Decide how much you d like to contribute from each paycheck. Employees hired on or after July, 07, will be automatically enrolled in the VCU Health System Savings Plan 457(b) at a contribution rate of %. To opt-out you must change your contribution rate to 0%. 7

Select your retirement provider (Fidelity and/or TIAA). You can choose 00% to one provider or split your deferral election, to contribute a portion to each retirement provider. For example, if you make a 0% deferral election, you can then elect to direct 50% of your deferral (5%) to Fidelity and the other 50% of your deferral (5%) to TIAA. Or you can elect any other split you prefer as long as the total equals 00%. 8

3 Choose investments. You have options to help you choose investments. If you selected TIAA as your retirement provider, you will be directed to the TIAA website to choose your investments at TIAA. See Step 4 for the final step to make elections with TIAA. Once you have selected your investments, review your elections and click submit. A confirmation screen will appear. You can enroll in other plans, consolidate accounts, or skip to the next section to add your beneficiaries. 9

4 Final Step if you chose to direct any of your deferrals to TIAA. Click on the blue box with the arrow in it to continue to the TIAA website. Follow the instructions from TIAA to select your investments. If this step is skipped, your deferral will still be directed to TIAA; however, it will be invested in the default fund for the Plan. Once you have made your elections, be sure to designate your beneficiary directly with TIAA. 0

Update current contributions Click Update from the Pre-Login home page and follow the prompts to log on with your username and password. 3 Choose Contribution Amount and follow the steps to update your election. Deferral elections will be withheld during each pay period and directed to your chosen retirement provider(s) and investment elections.

Modify your retirement provider Click Update from the Pre-Login home page and follow the prompts to log on with your username and password. 3 Choose Retirement Providers and follow the steps to update your election. 4 Enter your elections and click Change Retirement Provider Elections You can choose 00% to one provider or split your deferral election to contribute a portion to each retirement provider. For example, you can elect to direct 50% to Fidelity and the other 50% to TIAA. Or you can elect any other split you prefer as long as the total equals 00%. Elections for your retirement provider may be selected for each plan account. The election you make in one plan account only applies to that one plan account. Changes will take effect as soon as administratively feasible.

Register to create a NetBenefits username and password If you have a TIAA account or have not previously set up a NetBenefits username and password, you will need to register to create a username and password in order to access your NetBenefits account. If you are a current Fidelity customer or already have a NetBenefits username and password, you can continue to use them to log on to your NetBenefits account. Click Register link at the top of the home page and follow the prompts to establish a username and password. Please do not change the Your Account drop-down option from Fidelity to TIAA. Please keep the option listed as Fidelity when registering. Reset your NetBenefits username and password If you have forgotten your NetBenefits username and/or password, you will need to reset your username and password in order to access your NetBenefits account. If you are a current Fidelity customer or already have a NetBenefits username and password, you can continue to use them to log on to your NetBenefits account. Click Need Help? link at the top of the home page and follow the prompts to reset your username or password. Please do not change the Your Account drop-down option from Fidelity to TIAA. Please keep the option listed as Fidelity when registering. 3

Log on to your NetBenefits account If you already have a NetBenefits username and password, you can continue to use them to log on to your NetBenefits account. From the home page, enter your NetBenefits username and password at the top. Click Log in. Please do not change the Your Account drop-down option from Fidelity to TIAA. Please keep the option listed as Fidelity when logging on to NetBenefits. Direct access to TIAA Change the Your Account drop-down option to TIAA at the top of the home page. Be sure to change the Your Account drop-down option to TIAA. TIAA 3 You will be redirected to the TIAA website. Click Go to TIAA. 4

Update or review your investment elections From the home page enter your NetBenefits username and password at the top. Click Log in. 3 Choose Change Investments in the Quick Links menu for the plan in which you wish to change your investments. 4 From the Investments tab, choose Change Investments. Choose the action you wish to take. Note changing your future contribution elections will not affect existing balances and vice versa. Select Change Investment Elections to change where your future contributions will be invested. Select Exchange ONE Investment or Exchange MULTIPLE Investments to change the balances in your existing investments. 5

Update or review your beneficiary Now is a good time to review or update your beneficiary information if you ve experienced a life event, such as marriage, divorce, the birth of a child, or a death in the family. To update your beneficiaries for your account at Fidelity, follow the steps listed below. To update your beneficiaries for your account at TIAA, please log on to your TIAA account or call the TIAA Service Center at 800-84-5. From the home page, enter your NetBenefits username and password at the top. Click Log in. 3 Choose Profile at the top of the NetBenefits home page. 4 Select Beneficiaries. Select the employer name you wish to update your beneficiary for, if applicable. Select edit to make updates to your beneficiary designations. Review and select Save All. Review the Consent section and click I agree to these terms and Save choices & Finish. 6

Set your preferences Choose how you would like to receive important account information. From the home page, enter your NetBenefits username and password at the top. Click Log in. 3 Choose Profile at the top of the NetBenefits home page. 4 Select Communications. 5 Choose to receive documents by email or U.S. mail. Click Save. 7

Learn more about the options available for retirement accounts with a previous employer From the home page, enter your NetBenefits username and password at the top. Click Log in. 3 Choose Rollovers in the Quick Links menu and follow the prompts. Be sure to consider all your available options and the applicable fees and features of each before moving your retirement assets. 8
