NON-CONTRIBUTORY ACCIDENTAL DEATH CERTIFICATE OF INSURANCE GROUP POLICY: MZ0926217H0000A POLICYHOLDER: RECREATIONAL GROUP INSURANCE TRUST C/O THE GOOD SAM CLUB PARTICIPATING ORGANIZATION: THE GOOD SAM CLUB We certify that, subject to the terms of the Group Policy, the Member to whom this Certificate is issued (referred to as you, your and yours) is insured for the benefits described in this Certificate on and following the effective date on which he is eligible. Your eligible Dependent, if any, for whom premiums have been paid is also insured for the benefits described in this Certificate. You and your Dependent are referred to as the Covered Person. In this Certificate Monumental Life Insurance Company will be called we, our or us. This Certificate summarizes certain provisions of the Group Policy. All coverage and provisions are subject to those in the Group Policy issued to the Policyholder. LIMITED BENEFIT, PLEASE READ CAREFULLY 1

SCHEDULE OF BENEFITS BENEFIT AMOUNTS MEMBER AND DEPENDENT Death Benefit An Amount of Insurance as selected by the Member from $5,000. Dependent spouse is insured for 100% of the Member's benefit. No Cost Member and Spouse Travel Accidental Death benefit in force for life (benefits do not terminate as long as they remain a member. $5,000 death benefit if member/spouse killed accidentally while travelling in an RV, pulling a 5 th wheel or at a camp ground once activated. ADDITIONAL BENEFITS AMOUNTS & LIMITS Accident Hospital Indemnity Emergency Room $100 per day. Maximum Benefit Period: 730 days. $25 per visit. DEFINITIONS When used in this Certificate the following words and phrases have the meaning given. The use of any personal pronoun includes both genders. CAMPGROUND means a state park or a designated area that requires payment of a daily fee to camp on the grounds. CONFINED OR CONFINEMENT means that the Covered Person is a registered bed patient in a Hospital and is charged room and board by the Hospital. He must be in the Hospital on the advice of a Physician and under the regular care and treatment of a Physician. Confinement does not include treatment received in the outpatient department of the Hospital. Outpatient treatment means service rendered for a period of less than 24 hours. DEPENDENT means your spouse under age 70, unless you are legally separated. A spouse who is insured under the Group Policy as a Member will not be eligible as a Dependent. FIFTH WHEEL means a two level Recreational Vehicle that is designed to be hitched to and towed by a pickup equipped with a special hitch in the truck bed. HOSPITAL means an institution which meets all of the following requirements: (1) it must be operated according to law; (2) it must give 24 hour medical care, diagnosis and treatment to the sick or injured on an inpatient basis for which a charge is made; (3) it must provide diagnostic and surgical facilities supervised by Physicians; (4) registered nurses must be on 24 hour call or duty; and 2

(5) the care must be given either on the Hospital's premises or in facilities available to the Hospital on a pre-arranged basis. A Hospital is not a rest, convalescent, extended care, rehabilitation or other nursing facility. It is not a place which primarily treats mental illness, alcoholism or drug addiction; nor does it include any ward, wing or other section of the Hospital that is used for such purposes. It is not a facility where, in the absence of insurance, there is no legal obligation to pay. INJURY means bodily injury caused by an accident while a licensed driver of or passenger in a Recreational Vehicle, while in a vehicle pulling a Fifth Wheel, or while at a Campground. The accident must occur while the Covered Person s insurance is in force under the Group Policy. The Injury must be the direct cause of the loss and must be independent of all other causes. The Injury must not be caused by or contributed to by Sickness. PARTICIPATING ORGANIZATION means an Organization which has signed a Participation Agreement adopting the Policyholder's plan of insurance. RECREATIONAL VEHICLE means a vehicle designed for highway movement, is licensed for use on public highways, provides temporary sleeping accommodations and has one of the following: sink, toilet, eating facility, oven, stove, refrigerator, or shower. PHYSICIAN means a person licensed by the state in which he is resident to practice the healing arts. He must be practicing within the scope of his license for the service or treatment given. The Physician may not be you or a member of your immediate family. SICKNESS means an illness or disease which results in a covered Loss while your insurance is in force under the Group Policy. ACCIDENTAL DEATH BENEFIT When we receive due proof that a Covered Person dies, we will pay the benefit shown on the Schedule of Benefits to his named Beneficiary; provided; (1) death occurs as a direct result of an Injury; and (2) death occurs within 365 days of the accident causing the Injury. EXCLUSIONS We will not pay a benefit for a Loss which is caused by, results from, or contributed to by: suicide, attempted suicide or intentionally self-inflicted Injury, while sane or insane (in Missouri and Colorado while sane). declared or undeclared war or any act of war. full-time military service. participating in a riot; committing an assault or felony. Sickness or its medical or surgical treatment, including diagnosis. bacterial infection except through a wound accidentally sustained. operating or riding in any aircraft except as a fare-paying passenger on a regularly scheduled commercial flight or as a passenger in a transport plane operated by the Air Mobility Command (AMC) of the United States of America. 3

alcohol intoxication, as defined in the state where the accident causing the Injury occurred. taking of any drug, medication, narcotic, or hallucinogen, unless as prescribed by a Physician. taking of alcohol in combination with any drug, medication, or sedative. voluntary gas inhalation or poison voluntarily taken, administered, or inhaled. riding or driving as a professional in any kind of race for prize money or profit. WHEN COVERAGE ENDS Your insurance automatically ends on the first of the following dates: The date the Group Policy is terminated. The premium due date after you are no longer a member of the Participating Organization. The premium due date next following the date the Participating Organization ceases to participate in this plan of insurance. The anniversary date you fail to re-enroll if required, by the RECREATIONAL GROUP INSURANCE TRUST C/O THE GOOD SAM CLUB. Your Dependent's insurance automatically ends on the first of the following dates: The date your coverage terminates, except as provided in the Continuance of Dependent Insurance provision; or The premium due date after a Covered Person ceases to be an eligible Dependent. CONTINUANCE OF DEPENDENT INSURANCE. If you die while insured under this Group Policy, your spouse may continue coverage if insured. The spouse's premium will be based on his attained age. Termination of this Group Policy will not prejudice any claim originating prior to termination, subject to all other terms of this Group Policy. GENERAL PROVISIONS BENEFICIARY Your Beneficiary shall be as specified on the records of the Administrator. RIGHT TO EXAMINE The Policy is in the possession of the Policyholder; it will be available to be inspected by you at any time during business hours at his office. WHEN THERE IS A CLAIM PAYMENT OF CLAIMS Claims for benefits provided by the Group Policy will be paid as soon as written proof is received. Benefits for loss of life will be paid in accordance with the Beneficiary designation in effect at the time of payment. All other benefits are paid directly to the Covered Person. If a Beneficiary is a minor and there is no parent or legal guardian, or if he cannot give a valid release, the benefit will be paid as follows: to the person or institution we decide has assumed custody or support of the Beneficiary. 4

Any payment that we make in good faith will fully discharge us to the extent of that payment. PHYSICAL EXAMINATION AND AUTOPSY At our expense, we have the right to have the Covered Person examined as often as necessary while a claim is pending. At our expense, we may require an autopsy unless the law forbids it. LEGAL ACTIONS No legal action may be brought to recover against the Group Policy within 60 days after written proof of loss has been given. No such action will be brought after three years from the time written proof of loss is required to be given. If a time limit of the Group Policy is less than allowed by the laws of the state where you live, the limit is extended to meet the minimum time allowed by such law. ACCIDENT HOSPITAL INDEMNITY BENEFIT RIDER This Accident Hospital Indemnity Benefit Rider is a part of the Certificate to which it is attached. It is issued in consideration of the application and the continued payment of the required premium. Upon receipt of due proof that a Covered Person is Confined as a result of an Injury which occurs while insurance is in force, we will pay the benefit shown on the Schedule. The benefit payable is subject to the following condition(s): The Confinement must begin within 90 days of the accident causing the Injury and while insurance is in force for the Covered Person. Benefits begin on the first day of Confinement. The Covered Person will receive benefits as long as he is Confined up to the Maximum Benefit Period specified in the Schedule for each period of Confinement. One thirtieth of the monthly benefit will be paid for each day of a partial month of Confinement. Successive periods of Confinement will be considered as separate periods of Confinement; unless: the new period of Confinement is due to the same cause or causes as the prior one; and the new period of Confinement starts less than 90 days after the prior one stopped. Benefits provided by this Rider are paid in addition to the Amount of Insurance shown on the Schedule. Benefits are subject to all terms and conditions of the Policy. This Rider does not waive, alter or extend any provisions or limitations of the Policy except to the extent shown above. EMERGENCY ROOM RIDER This Emergency Room Rider is a part of the Certificate to which it is attached. It is issued in consideration of the application and the continued payment of the required premium. Upon receipt of due proof that a Covered Person incurred expenses for medical treatment received in a Hospital emergency room as a result of an Injury, we will pay the benefit shown on the Schedule. Usual and Customary Expenses means the normal charge made by most Physicians or medical service providers for a similar service or supply in the locality where the service is rendered or supply purchased. This benefit will not duplicate payments made under any other provision of the Policy. Benefits are subject to all terms and conditions of the Policy. This Rider does not waive, alter or extend any provisions or limitations of the Policy except to the extent shown above. SUMMARY OF THE LIFE AND HEALTH INSURANCE PROTECTION ASSOCIATION ACT AND NOTICE CONCERNING COVERAGE LIMITATIONS AND EXCLUSIONS INTRODUCTION Residents of Colorado who purchase life insurance, annuities or health insurance should know that the insurance companies licensed in this state to write these types of insurance are members of the Life and Health Insurance Protection Association. The purpose of this Association is to assure that policyholders will be protected, within limits, in the unlikely event that a member insurer becomes financially unable to meet its obligations. If this should happen, the Association will assess its other member insurance companies for the money to pay the claims of insured persons who live in Colorado and, in some cases, to keep coverage in force. The valuable extra protection provided by these insurers through the Association is limited, however. As noted in the box below, this protection is not a substitute for consumers' care in selecting companies that are well-managed and financially stable. IMPORTANT DISCLAIMER The Life and Health Insurance Protection Association may not provide coverage for this policy. If coverage is provided, it may be subject to substantial limitations or exclusions, and require residency in Colorado. You should not rely on coverage by the Life and Health Insurance Protection Association in selecting an insurance company or in selecting an insurance policy. Coverage is NOT provided for your policy or any portion of it that is not guaranteed by the insurer or for which you have assumed the risk. Insurance companies or their agents are required by law to give or send you this notice. However, insurance companies and their agents are prohibited by law from using the existence of the association to induce you to purchase any kind of insurance policy. SUMMARY The state law that provides for this safety-net coverage is called the Life and Health Insurance Protection Association Act. Below is a brief summary of this law's coverages, exclusions and limits. This summary does not cover all provisions of the law; nor does it in any way change anyone's rights or obligations under the act or the rights or obligations of the Association. Coverage. Generally, individuals will be protected by the Life and Health Insurance Protection Association if they live in this state and hold a life or health insurance contract, or an annuity, or if they hold certificates under a group life or health insurance contract or annuity, issued by a member insurer. The beneficiaries, payees or assignees of insured persons are protected as well, even if they live in another state. Certain parties to structured settlement annuity contracts may be entitled to coverage benefits as well based on defined circumstances. Exclusions From Coverage. Association if: Persons holding such policies or contracts are not protected by this they are not residents of the State of Colorado, except under certain very specific circumstances; the insurer was not authorized or licensed to do business in Colorado at the time the policy or contract was issued; their policy was issued by a nonprofit hospital or health service corporation, an HMO, a fraternal benefit society, a mandatory state pooling plan, a mutual assessment company or similar plan in which the policyholder is subject to future assessments, or by an insurance exchange. The Association also does not provide coverage for: any policy or portion of a policy which is not guaranteed by the insurer or for which the individual has assumed the risk; any policy of reinsurance (unless an assumption certificate was issued); plans of employers, associations, or similar entities to the extent they are self-funded or uninsured (that is, not insured by an insurance company, even if an insurance company administers them); interest rate yields, crediting rate yields or other factors employed in calculating returns, including but not limited to indexes or other external references stated in the policy or contract, that exceed an average rate specified in the Association Act; dividends; experience rating credits; credits given in connection with the administration of a policy or contract; any unallocated annuity; annuity contracts or group annuity certificates used by nonprofit insurance companies to provide retirement benefits for nonprofit educational institutions and their employees; policies, contracts, certificates or subscriber agreements issued by a prepaid dental care plan; sickness and accident insurance when written by a property and casualty insurer as part of an automobile insurance contract; unallocated annuity contracts issued to an employee benefit plan protected under the federal Pension Benefit Guaranty Corporation; policies or contracts issued by an insurer which was insolvent or unable to fulfill its contractual obligations as of July 1, 1991, except for annuity contracts issued by a member insurer which was placed into liquidation between July 1, 1991 and August 31, 1991; policies or contracts covering persons who are not citizens of the United States; any kind of insurance or annuity, the benefits of which are exclusively payable or determined by a separate account required by the terms of such insurance policy or annuity maintained by the insurer or by a separate entity. Limits On Amount Of Coverage. The Act also limits the amount the Association is obligated to pay out. The Association cannot pay more than what the insurance company would owe under a policy or contract. Also, for any one insured life, no matter how many policies or contracts were issued by the same company, even if such contracts provided different types of coverages, the Association will pay a maximum of: $300,000 in net life insurance death benefits and no more than $100,000 in net cash surrender and net cash withdrawal values for life insurance; for health insurance benefits - $100,000 for coverages not defined as disability, basic hospital, medical and surgical or major medical insurance, including any net cash surrender and net cash withdrawal values; $300,000 for disability insurance; or $500,000 for basic hospital, medical and surgical, or major medical insurance; $250,000 in the present value of annuity benefits, including net cash surrender and net cash withdrawal values; or with respect to each payee of a structured settlement annuity, $250,000 in present value annuity benefits, in the aggregate, including net cash surrender and net cash withdrawal values; or $300,000 for long-term care benefits. The Association shall not be liable to expend more than $300,000 in aggregate, with respect to any one life except that with respect to benefits for basic hospital, medical and surgical and major medical insurance, the aggregate liability of the Association shall not exceed $500,000 with respect to any one individual.
