Help to keep your finances healthy Income Protection+ is a long term policy that pays out if, during the policy term, you can t work and suffer a loss of earnings due to illness or injury. The policy has no cash in value at any time. If you stop paying your premiums, we ll stop providing cover and we won t pay any money back to you. We ll only make a payment if a successful claim is made. It s a sad fact of life; your outgoings wouldn t stop if your income did. But with one of our Income Protection+ policies in place, we can help you cope financially if you can t work because you re sick or injured. The impact of ill health As one of the largest income protection providers in the UK we know only too well what an impact illness and injury can have on earning a living. That s why we ve created a flexible policy that can be designed to suit you. When you re covered, you can relax, and get on with enjoying life with your loved ones but our policy offers much more than basic financial protection. A range of extra benefits and support services are included as standard so if you were unable to work due to illness or injury, you know we ll be there to support you. 2 Why choose Income Protection+?

How Income Protection+ works Income Protection+ pays a proportion of your income each month for a fixed period of time if you are unable to work due to sickness or injury. You decide: How much you could receive each month as a benefit - within our limits How long you could receive those payments - from 2 years up to 52 years When you d like your payments to start you can choose a deferred period from as little as 4 weeks up to a maximum of 104 weeks. How much cover can you have and how much benefit will we pay? When you start your policy we ll do a calculation on your earnings before tax. That will define how much we can insure your income for (the maximum amount). If you make a claim, we ll do the same calculation on your earnings in the 12 months before your claim. That will show you the maximum amount you can claim. Once we ve calculated the maximum benefit amount you may be eligible for our benefit guarantee. We ll then pay the lower of either the calculation when you make a claim, or the amount you re insured for on your policy. We may also need to take off the amount of any sick pay received from your job from the benefit amount we pay. 4 Why choose Income Protection+?

Our Income Protection+ policy includes a waiver of premium as standard. This means that, if you make an eligible claim, we ll help out by paying your premiums for you from 13 weeks after you stop working, or from the end of your chosen deferred period whichever comes first. Annual benefit increases If you expect your salary to go up every year, or you want your level of benefit to increase annually, then it s worth talking through our increasing cover options with your broker. Your benefit could increase each year in line with any rise in the Retail Prices Index or by a fixed annual rate of either 3% or 5%... see which one is right for you. Any increase in cover will mean that your premium will go up too but at a higher percentage rate. We will review your cover every year. If you have chosen to increase your cover, we will write to you every year to explain what your new level of cover is and the cost. If you don t want to take the increase in cover then you just need to let us know and we ll offer it again the year after. Benefit guarantee We all hope our salaries will go up. But we also know things can be unpredictable. Sometimes you might find your income going in the wrong direction. To help, we ve included a benefit guarantee. This way, small variations in earnings should not impact what you ll get if you have to make a claim. NB. If you re working less than 16 hours a week immediately before your illness or injury then you won t be covered for the benefit guarantee aviva.co.uk 5

Claims Income protection claims can last for months, years, or in some cases decades. Knowing there ll still be money coming in if you can t work will be a huge relief for you and your loved ones and if you could never work again, the benefit could be invaluable. More support for you When recovering from illness or injury, getting back to work can present a number of challenges. Our claims team offer a range of support services that will help. These services may even make the difference between being off for a couple of weeks and being unable to work for a much longer period. What s more, we don t believe in one size fits all. Our support is tailored, so you re benefiting from the services that will help you. Our claims team includes clinicians and specialists in rehabilitation, psychiatry and physiotherapy. There ll be advice from our intervention team on medical services that may help, such as physiotherapy or psychiatric therapy. Our rehabilitation claims visitors can tell you about the benefits you may be able to claim while you can t work and our claims consultants will also be on the end of the phone to answer any questions and provide support throughout your claim and illness. We ll get the right support for you as quickly as possible. 6 Why choose Income Protection+?

Back to work benefit Our back to work benefit can help make getting back to work easier. It pays a reduced benefit that takes into account any loss of earnings you incur due to your illness or injury when returning to work. You can claim back to work benefit if you were in a paid job and: you return to work in a different job because of your incapacity; or you return to your old job but the illness or injury that caused incapacity restricts what duties you can do or hours you can work; and your earnings are reduced. aviva.co.uk 7

Extra benefits, more support As soon as you take out Income Protection+ you and your immediate family can access these extra benefits and support services. Unless we say otherwise, these are available whether you re making an income protection claim or not. Extra benefits Family carer benefit When a spouse, civil partner or child needs care, it can have a huge effect on the family. If your spouse, civil partner or child suffers from an illness or injury that means they can t perform certain activities of daily living for more than three months, we will pay you the amount of benefit you ve chosen or 1,500 per month, whichever is lower, for a maximum of 12 months during the policy term. This gives you short term financial support while you come to terms with a possibly life changing situation. It lets you think about the future. We ll pay this extra benefit whether you carry on working or not, so that you can provide the support that s necessary when it s needed most. Please be aware however that, if you are already claiming main, restricted or back to work benefit, family carer benefit won t be paid. 8 Why choose Income Protection+?

Hospital benefit Being in hospital can mean more outgoings for you and your family. Things like travelling expenses and parking, for example. Your income may have stopped too, so this extra payment will help out while we assess your claim. We ll pay this if you re in hospital as a result of your illness or injury for more than six consecutive nights during your deferred period. We ll pay 100 for each night you re in hospital, provided you have been there for at least 6 nights, up to a maximum of 90 nights during the policy term. Trauma benefit Some events - such as going blind or losing limbs - can have a profound impact. An extra payment can go a long way, helping someone adjust to a new way of living. We ll make one payment of six times the amount of benefit shown in your policy schedule or 40,000, whichever is lower, if you suffer one of six traumatic events. aviva.co.uk 9

Life change benefit Without the need for any further medical questions to be answered, life change benefit may allow you to increase your cover amount if your circumstances change. Life change benefit covers the following life events: Marriage or civil partnership Divorce, dissolution of a civil partnership or separation Becoming a parent Increasing a mortgage due to a house move or purchase, or carrying out home improvements Change of employer or promotion 20% increase in salary 10 Why choose Income Protection+?

More support Unless stated otherwise the following support services are available to the policyholder, their spouse/partner and any dependent children over the age of 18. These support services are all non-contractual and may be withdrawn by us at any time. Physio treatment (just for the policyholder) Aches and pains in muscles and joints can affect anyone, at any time, and they re all frustrating. That s why Income Protection+ offers physio treatment through IPRS Health to help you restore body movement after an injury or illness, and you can contact this service twice a year for different problems. Using the benefit couldn t be easier all you have to do is call. You ll get a clinical assessment over the phone with a qualified clinician who ll talk through your symptoms and assess the severity of your condition. Depending on the outcome, you ll be offered either: an online personalised programme of exercises, supported by videos and emails with more information and guidance, and follow-up calls to check on your progress; or access to our network of physiotherapists for treatment in your local area, so they can assess your symptoms face to face. We ll provide up to three physiotherapy sessions twice per year for different conditions, if required (six in total) at no extra cost to you. If your condition can t be managed by physiotherapy, you ll be referred to your GP who may then arrange for you to see a specialist. aviva.co.uk 11

Counselling and carer support (available for the policy holder, their partner or spouse and dependent children over the age of 18) Life can be tough sometimes, so it helps to have someone you can turn to. That s why each of our protection policies includes access to counselling and carer support services provided by Workplace Options. Workplace Options offer face to face or telephone based support for you and your family, giving help with: Mental wellness issues such as bereavement, work-related stress and relationship breakdown. Facing the challenges that come with being a carer. Second opinion by Best Doctors (available for the policyholder, their spouse/partner and any children under the age of 18 or 21 if in full time education.) Second opinion from Best Doctors gives you access to a worldwide network of peer recommended medical experts. If you have been diagnosed with a serious or worrying medical condition, it s understandable that you will want to have as much information as possible and explore the treatment options available to you. Once you contact Best Doctors, they will collect your medical records and select an expert to provide an independent review of your condition and treatment options. You will then receive a detailed case report, which provides the case doctor s professional medical opinion, including; Confirmation of your diagnosis Treatment recommendations 12 Why choose Income Protection+?

Bupa Anytime Healthline Bupa Anytime Healthline is a telephone support service that gives you access to a qualified nurse 24 hours a day, 7 days a week. You can use this service to get answers to any questions or concerns you may have about a specific medical condition or to get practical advice on your general health and well being. You can also use the service with regards to your loved ones for example, if you re worried about your children s health. Product flexibility We ve built Income Protection+ with the long haul in mind (after all, your policy could last your whole working life). If you need to, you can make changes to it but because we know you don t want to constantly be contacting us, we ve tried to make the policy as intuitive as possible. What happens if I m on maternity/parental/adoption leave? If you re ill or injured during the first 12 months of your leave, we ll treat your claim as though you re still working. I ve been made redundant? For the first 3 months we ll treat your claim as though you re still in work. I m not working? We ll assess your claim on the occupation you were doing in the 12 months before you stopped working. And pay a benefit for up to 12 months. Deferred period for NHS doctors & surgeons: If you re a doctor or surgeon employed directly by the NHS, you can set up your policy with dual deferred periods of 26 weeks and 52 weeks, so it coincides with the maximum amount of sick pay you can get from the NHS. However, you ll be eligible for benefit when your sick pay from the NHS stops, or reduces. It s important to review your cover regularly as your earnings and/or sick pay may change over time and discuss any concerns you have about the suitability of the policy for you with your financial adviser. aviva.co.uk 13

Optional benefits 14 Why choose Income Protection+?

Fracture cover Fracture cover is available at an extra cost and offers additional cover for a range of fractures. We ll pay a lump sum if you suffer one of 18 fractures during any 12 month period. We ll pay one successful claim each year. Global treatment If you or your child were to suffer from a serious illness, it s only natural that you d want expert diagnosis and treatment. We ve teamed up with Best Doctors to offer global treatment. This gives access to worldwide medical specialists and covers the cost of overseas treatment for certain serious illnesses or medical procedures, as defined in the policy conditions. This is an optional benefit that renews every three years, available at an additional cost. Please refer to the policy conditions for further information about this. Global treatment is a way to make overseas treatment possible as it provides access to international hospitals. This is an option that may be unattainable to most because of financial barriers. aviva.co.uk 15
