Selected Medical Plan Information Section Overview: The Selected Medical Plan Information section contains standard quote reports with options similar, if not identical, to the Classic QuoteStar system. All reports in this section show only plans selected during the plan selection period of the quoting process. When selecting multiple reports as shown above, all reports will sort by options chosen in this section. These reports have the ability to show employer contribution based on the information entered into the system. If you do not want to display this information, select No in the circled section above. Selected Medical Plan Information Plan Grid Report Description: The Plan Grid report shows a listing of all plans selected to quote, with basic benefits and total monthly premium for the group. Report Variations: You can choose to show this report with or without the Low/Standard/High RAF monthly premium totals for each plan. Suggested Usage: Overview piece used early in the quoting process to help pare down plan choices. Overview for all plans in a carrier portfolio. Use either this Plan Grid report or the Cost Comparison report. 4

Selected Medical Plan Information Side by Side Report Description: The Side by Side comparison report shows a grid of plans selected to quote. The grid contains a basic summary of benefits, both in and out of network, and the total monthly premium, broken down by employee cost, dependent cost, and employer contributions. It will show percentage change versus current if current plan information is available and entered into the system. Report Variations: You can show this report with or without the monthly premium amounts. To leave out the dollar amounts, select the Benefits Only box in the circled section above. You can have anywhere from two to seven plans across per page, as illustrated above. You can include current and renewal plan information, if available and entered into the system, by selecting the Yes button as shown above. Suggested Usage: Easy-to-read side-by-side comparison with basic benefit categories. Overview for all plans in a carrier portfolio. Use either this Side by Side report or the Side by Side Detail report. 6

Selected Medical Plan Information Side by Side Detail Report Description: The Side by Side Detail report shows a spreadsheet-like selection of plans to compare benefits and premiums; it is more detailed than the Side by Side report. Additional benefits shown on this report include out-patient surgery, ER Visit, and Lab & X-ray. It will show percentage change versus current if current information is available and entered into the system. Report Variations: You can show this report with or without the monthly premium amounts. To leave out the dollar amounts, select the Benefits Only box in the circled section above. You can have either two or three plans across per page, as illustrated above. You can include current and renewal plan information, if available and entered into the system, by selecting the Yes button as shown above. Selected Medical Plan Information Employee Rate Side by Side Report Description: The Employee Rate Side by Side report shows the employee rates, broken down by employee only and employee total cost, underneath a brief summary of plan benefits. It will show four plans per page, listed in columns, with totals at the bottom. Report Variations: You can include current and renewal plan information, if available and entered into the system, by selecting the Yes button as shown above. Suggested Usage: Side-by-side comparison of employee rates. Useful for showing employee rates across a portfolio of plans. Selected Medical Plan Information Employee Rate Detail Report Description: The Employee Rate Detail (Breakdown) report is a detailed rate page showing employee only, dependent and employee total rates for each employee. It contains basic benefit information. This report will print as many plans per page as will completely fit, based on the number of employees quoted. Report Variations: None. Suggested Usage: Useful as backup information when showing either the Side by Side reports or the Dual/Multi Option reports. Use either the EE Rate Side by Side report or this Employee Rate Detail report. 18

Selected Medical Plan Information Single Plan Detail Report Description: Single Plan Detail is a one-plan-per-page, detailed report that shows a simple summary of benefits, followed by premium totals with an employee level rate breakdown at the bottom. The main page of the report can extend to two pages if the census is larger than 25 employees. This report is most likely to increase the size of your proposal. By decreasing the number of selected plans, you can decrease the size of the proposal overall. Report Variations: Add a page of full benefits (one plan per page) by selecting the Add Full Benefits checkbox as shown above. Do not select the Full Benefits report if selecting this option. Add a page of table rates and rate averages (one plan per page) by selecting the Add Table Rates checkbox as shown above. Suggested Usage: Useful for showing detailed rate and benefit information for selected medical plans. Use either this Single Plan Detail report or a combination of the Full Benefits and Employee Rate Detail reports. 20

General Plan Information Current/Renewal Report Description: Current/Renewal is a detailed report of current versus renewal information. It contains a basic summary of benefits, as well as a complete listing of employee premiums, separated by each plan a company currently offers. The group totals are by plan, with a summary of all plans at the end, showing total change in premiums. Report Variations: You can choose to print the report with either a basic summary of benefits or full benefits pages. Suggested Usage: Useful as a starter piece for brokers to send to their clients at renewal, especially when a group has multiple carriers. 30

General Plan Information Cost Comparison Report Description: The Cost Comparison report displays a complete list of plans available in the marketplace for a group, based on selected carriers. It shows a breakdown of HMO and PPO plans based on the requested plan type. Report Variations: You can show plans sorted by premium or by carrier. You can include or exclude current and renewal information. Suggested Usage: Useful as a complete listing of plans to show a client that you have reviewed all plans available. Useful for reducing the size of a proposal by using this report in addition to limiting plan selection for the Single Plan Detail report. Use either this Cost Comparison report or the Plan Grid report. 34

It contains employee rates for each family status, with one page per employee. Please note: Employee Elect refers to any portfolio of medical plans offered by any carrier, not only to the Anthem Blue Cross EmployeeElect portfolio. Report Variations: By selecting Multiple Packages, you choose more than one carrier s portfolio and print a page per employee, per portfolio. When selecting EE Elect in the circled section above, the report changes into a grid format geared for employers. You can include a full benefits page for each plan included in the portfolio. Dual/Multi Option Proposal: Dual Option Report Description: Dual Option is a summary report of dual option combinations, showing a PPO option on top and a HMO option on the bottom, with a summary of both premium amounts below. It includes basic benefit summaries. This report will not accommodate a dual choice PPO/PPO or an HMO/HMO. If you have one of these dual choice plan combinations, use the Multi Option Report. Report Variations: You can choose to include or exclude current and renewal. You can choose to have benefit information, cost summary information, or both. You can choose to include or exclude an employee rate breakdown. You can choose to include or exclude available rider information. This report will print as many plans per page as will completely fit, based on the number of employees quoted. Report Variations: You can include or exclude the Life Underwriting Guidelines with this report. You can choose to quote either a flat life amount per person or a multiple of salary. Special Settings LISI Settings Button What is the LISI settings button: The LISI settings button produces a quote in the standard LISI format* with the touch of a button. Once you have chosen your plans and arrive at the Report Options page, pressing the LISI button will prompt the system to mark the appropriate checkboxes and options, and to produce a LISI standard proposal. When the screen refreshes and the checkboxes and options are marked by the system, you must choose the Life carriers and Amounts, as well as the Vision Plans you wish to quote.
