(Enclose necessary proof of registration) In case not registered, you should submit a declaration to the effect that in case if empanelled with the Bank then you shall get registered with Employees State Insurance Corporation subject to its applicability as per the respective law. 18 Whether you are registered with Employees Provident Fund Organization (EPFO)? I/We understand and agree that Bank reserves the right to inform/recommend unsatisfactory/incompetence of the empanelled Contractor to the respective Statutory Body such as Electrical Inspectorate, State Electricity Board or to any other organization etc in this regard for blacklisting or such other necessary action. 9. I / We would like to state that, in the event it is established that I / We are involved in unfair practices including false/distorted reports, which ultimately lead to distressed assets to Bank, my / our empanelment in the Bank can be terminated. In addition to the removal from the panel of your Bank and you may approach concerned professional body with complaint of misconduct against me / us. 10. TERMS AND CONDITIONS OF EMPANELEMENT 1) After empanelment any change in the Address, Contact information including Phone Numbers, Constitution etc has to be informed to the Bank within 15 days from the date of such change. Any other change which affects the information furnished by you in the application also need to be informed within 30 days from the date of such change. 2) This empanelment will be valid for a period of 3 years or till such time the Bank forms a new empanelment subsequent to this empanelment. However the Bank will have liberty to cancel this empanelment at any point of time even in the initial 3 years period also without assigning any reason. Bank may also review the empanelment every year for retaining good performing Contractors and to remove non-performing Contractors based on their performance in the Bank's Panel. 3) The minimum eligibility criteria mentioned is furnished below. a. The Applicant should have minimum seven years of experience in the field of Electrical Contract works. b. The Applicant should possess valid class-i/class-a Electrical Contractor's License. 4) Based on the experience of the applicants matching to Bank's requirements, the Bank may consider or not the applicant for selection of empanelment in the Bank. Bank's decision in this regard shall be final and shall be binding on the applicant. The Bank will have right to increase or decrease the qualification depending upon availability of sufficient number of candidates for empanelment. 5) The applicants should have Service Tax Registration with Central Service Tax Department. The applicants having service tax registration may be preferred as it is compulsory for firms to register under service tax with annual turnover more than Rs 10 Lahks. Those applicants who do not have registration with Service Tax Department may be considered for smaller works only. 6) The Applicants those who do not have service tax registration should submit a declaration in their letter head along with Application to the effect that their turn over is less than the threshold amount stipulated for Service tax Registration and their Company/Firm/Proprietorship/etc is not subjected to the Service Tax Registration Act and rules. Further the applicant undertakes to inform the Bank that as soon as the rule becomes applicable to them, it shall inform the same to the Bank immediately.

7) The Applicant also should have registration with Income Tax Department and PAN Number has to be furnished in the application. 8) The Applicant should have registration with State Commercial Tax Department for payment of VAT. The TIN Number has to be furnished in the Application. 9) The selected Contractor will have to coordinate with different Agencies / Consultants for the smooth execution of the work. Also they have to comply with the directions that will be given by our Security Division and Information Technology Division under intimation to this Department. 10) In the event it is established that the empanelled Electrical Contractor is involved in unfair practices including false/distorted reports, which ultimately lead to distressed assets of our Bank or such other damages, then the empanelment of such Contractors in our Bank will be terminated. In addition to the removal from the panel of our Bank we may approach concerned professional body or other organizations with the complaint of misconduct against such Contractor. 11) The empanelled contractors will be invited for quoting their rates through tenders/quotations of the bank. Depending on estimated value of the work / Project, the Bank reserves the right to tender the work/project amongst empanelled contractors or to make it an open tender to the public. The rights for type of tender to be called rest entirely on Bank and empanelled members have no say in the matter. 12) In case, after empanelment, the contractor is not participating in three consecutive tendering processes then the Bank may delist their name from empanelment at its discretion. 13) In case the empanelled Contractor is not successful in any of the tenders or only in few tenders then the Bank may not consider their name for continuance in the Panel of the Bank at its discretion. 14) In case of urgency, tender documents will be floated through email communication and it is deemed to be official communication from the Bank and the empanelled Contractor need to submit the completed tender in sealed manner to the bank as mentioned in the tender document. 15) Any email communication from the bank is to be treated as official communication from the Bank.

16) Awarding of work is at the sole discretion of the Bank and payment of Bills for the works executed will be subject to the norms prevailing in the Bank from time to time. 17) Services of the empanelled Electrical Contractors will be utilized as and when needed by the Bank and just empanelling of the contractor in the bank's panel does not mean any guarantee of awarding works to the contractor. 18) The Contractors when awarded the work shall take all the required labour registrations duly complying with the formalities under Contract Labour Act and Rules and to indemnify the Bank against any claims in this regard. 19) It is the sole responsibility of the contractor for the safety of his workers at the time of execution of any works in the bank and he should obtain necessary group insurance for the compensation to his workers and bank is not viable for payment of any compensation/ medical treatment for any of its workers in event of any accident. 20) In case of urgency the Bank will have right to award petty electrical works to any of the empanelled Electrical Contractors on single quotation basis at reasonable rate on rotation among the empanelled contractors. 21) The Bank does not undertake any responsibility to compensate the empanelled Electrical Contractor for any monetary loss other than the Bill amount payable to the Contractor as per norms, if any work is awarded. 22) The Bank reserves its right to delete the name of any Contractor from the Panel of Contractors at any time without assigning any reason thereof. GUIDELINES FOR FILLING THE APPLICATION 1) Application can be downloaded from the Bank s website www.corpbank.com. Those who are downloading from the Website, a demand draft for Rs.800/- favoring Corporation Bank and payable at Vadodara shall be enclosed along with the application form. 2) Application can also be obtained from the Zonal Office, Vadodara on payment of Rs.800/- (Rupees Eight hundred only) by means of Demand Draft in favour of Corporation Bank and payable at Vadodara. 3) Completed application along with all required supporting documents have to be enclosed in Envelope and super scribed as Application for empanelment of Electrical Contractors for Zonal Office, Vadodara and has to reach/submit the same at Premises Department, Corporation Bank Zonal Office, Vadodara on or before 30.04.2016 by 3.00 p.m. 4) Applicants should not make any change/alteration to any of the clause of the Application. It will make the applicant disqualified. The applications should be complete in all respects. Incomplete applications will be rejected. 5) It is compulsory that the applicant should furnish valid Email address in the application. 6) Self attested xerox copies of all documents supporting eligibility criteria and other details furnished in the application form are to be submitted along with the application form. On demand by the Bank, the applicant should produce originals of all documents submitted along with application for empanelment. 7) Bank will not be responsible for any loss of application either in transit or otherwise and for non-receipt of the same in time. Hence applicants are advised to ensure that the completed application reach us in time. 8) In case a single applicant applies for empanelment for both Electrical Contractor as well as Electrical Consultant, then the Bank shall consider the application in one suitable category depending on the merit of the application.
