Variable Universal Life Insurance Supplemental Application Tips for Completion This tip sheet highlights key points in each section of the VUL Supplemental Application. Please be familiar with these items as you and your client complete the information on this Supplemental Application. Accurate completion of the Supplemental Application will minimize delays and improve the app to issue timeline. Section A Initial Allocation Percentages Premium & Deduction Allocations (Required Section) Premium Allocations. Definition Investment options selected to apply funds from incoming premium payments. If the Lapse Protection Benefit (GMDB) Rider is selected, investment requirements will apply. See the prospectus or sales illustration for details. Deduction Allocations. Definition Investment options selected to deduct monthly account charges and any applicable surrender charges. If you do not specify an allocation of monthly charges or if there is not enough money to deduct from the selected investment options, charges will be deducted from the remaining investment options on a pro-rata basis. Allocations MUST be made in whole percentages AND total of all equal to 100%. Section B Dollar Cost Averaging (Optional Section) Definition - Dollar Cost Averaging is the systematic method of investing where securities are purchased at regular intervals so the cost of securities is averaged over time. There is a $5,000 minimum accumulation value to select this option. If the Lapse Protection Benefit (GMDB) Rider is selected, Dollar Cost Averaging is NOT available. Dollar Cost Averaging is NOT available if Automatic Rebalancing is selected (Section C). Section C Automatic Rebalancing (Optional Section*) Definition This feature automatically redistributes the accumulation value to correspond with current premium allocations. Rebalancing involves periodic buying or selling of assets within the portfolio to maintain the original level of stated investment goals. There is a $5,000 minimum accumulation value to select this option; this minimum is waived if the Lapse Protection Benefit (GMDB) Rider is selected. Automatic Rebalancing is NOT available if Dollar Cost Averaging is selected (Section B). The AGL Declared Fixed Interest Account is not available for Rebalancing. If this account was designated for premium allocations (Section A), the percentage designated for this allocation must be redistributed among the other funds chosen for premium allocations. If redistribution is needed, please complete the rebalance allocation in this Section C. Rebalancing Allocations MUST be made in whole percentages AND total of all equal to 100%. * If Lapse Protection Benefit (GMDB) Rider is selected, quarterly Automatic Rebalancing is REQUIRED. Section D Authorization for Transactions (Optional Section) If the client wishes AGL to act on telephone instructions or e-service instructions, this section will collect information on who is authorized to provide this information to us. Please check the desired box on the Supplemental Application. Section E Suitability (Required Section) All questions MUST be answered in this section. Section F Electronic Delivery Consent (Optional Section) If the client wishes to receive information from AGL electronically, this section will collect an email address. The owner must approve electronic delivery by initialing next to the email address provided. Please provide a complete and legible email address in the section provide. Section G Signatures (Required Section) This document requires the signatures of the registered representative, primary proposed insured, owner and joint owner, if applicable. Please obtain all signatures and dates prior to returning this form to the Home Office. AGLC108638

Policy Owner(s) or the Agent/Registered Representative who is appointed to represent AGL and the firm authorized to service my policy. AGL and any person designated by this authorization will not be responsible for any claim, loss or expense based upon telephone instructions and e-service instructions received and acted on in good faith, including losses due to telephone instructions or e-service communication errors. AGL s liability for erroneous transfers and allocations, unless clearly contrary to instructions received, will be limited to correction of the allocations on a current basis. If an error, objection or other claim arises due to a telephone instruction or e-service instruction, I will notify AGL in writing within five working days from receipt of confirmation of the transaction from AGL. I understand that this authorization is subject to the terms and provisions of my variable universal life insurance policy and its related prospectus. This authorization will remain in effect until my written notice of its revocation is received by AGL at its home office. Section E - Suitability All questions must be answered 1. Have you, the Proposed Insured or Owner (if different), received the variable universal life insurance policy prospectus and information describing the investment options? 2. That the policy applied for is variable, employs the use of segregated accounts which means that you need to receive and understand current prospectuses for the policy and the underlying investment options? b. That any benefits, values or payments based on performance of the underlying investment options may vary? c. That any benefits, values or payments based on performance are not guaranteed by AGL, any other insurance company, the U.S. Government or any state government? d. That any benefits, values or payments based on performance are not federally insured by the FDIC, the Federal Reserve Board or any other agency, federal or state? e. That in essence, all risk is borne by the owner except for funds placed in the AGL Declared Fixed Interest Account? f. That the policy is designed to provide life insurance coverage and to allow for the accumulation of values in the underlying investment options? g. That the amount or duration of the death benefit may increase or decrease, depending on the investment experience of the separate account? h. That the policy values (Accumulation Value, Cash Value, Cash Surrender Value) may increase or decrease, depending on the investment experience of the separate account, the AGL Declared Fixed Interest Account accumulation, and certain expense deductions? 3.
