Request for Bid Proposal for Janitorial Services for the Cabarrus Health Alliance The Cabarrus Health Alliance is now accepting proposals for Janitorial Services. Attached are specifications. Please review these specifications carefully. All proposals are due by June 7, 2016. Each bidder shall submit a proposal on the enclosed bid proposal form provided. The bid must be signed by an authorized official of the firm. Bids may be rejected if they show any omissions, alterations of form, additions not called for, conditional bid or any irregularities of any kind. Bids can be hand delivered or mailed to Cabarrus Health Alliance, 300 Mooresville Road, Kannapolis, NC 28081 in a sealed envelope. No bids will be accepted after the time and date stated above. A bidder may withdraw its bid prior to the deadline that bids are due by sending a written request to Erin Shoe. Bids may be withdrawn after the deadline only in accordance with N.C.G.S. 143-131.1. SPECIFICATIONS FOR PROVIDING JANITORIAL SERVICES CABARRUS HEALTH ALLIANCE, NORTH CAROLINA PURPOSE The purpose of these specifications is to establish requirements and standards for providing janitorial services for the several buildings occupied by the Cabarrus Health Alliance. These are minimum requirements to maintain the building in a clean and orderly manner. The successful bidder shall be prepared to perform each task as stated according to the work schedule without prodding or reminders by Cabarrus Health Alliance staff. EQUIPMENT AND SUPPLIES Contractor shall provide two bids. One bid will include the contractor providing all equipment and supplies necessary to provide the services required by these specifications. Supplies shall include but not be limited to: floor wax, paper towels, toilet tissue, cleanser, liquid hand soap, plastic trash can liners, window cleaner and air freshener. The second bid will assume Cabarrus Health Alliance is providing all equipment and supplies necessary to provide the services. GENERAL CONDITIONS Bidder Qualifications He/she shall have been in the industrial cleaning business for a minimum of one (1) year. Insurance The successful bidder, throughout the term of this agreement shall maintain in force adequate applicable insurance coverage for property and general liability, malpractice, workers compensation, and vehicle liability. When the minimum required insurance is determined to be insufficient, the Contractor will maintain in force insurance reasonably appropriate to the work to be undertaken by the Contractor. When exempt from workers compensation or other insurance coverage, the Contractor shall provide documented proof of exemption. Further, the Contractor agrees to provide a Certificate of Liability to the City for all applicable insurance coverage. Minimum Requirements: General Liability $1,000,000 per occurrence Workers Compensation $1,000,000 per accident A certificate of insurance must be provided to the Purchasing Manager, naming the Cabarrus Health Alliance as additional insured. In addition, 30 days notice of cancellation, non-renewal, or material change in coverage is required. Award of Bid The bid will be awarded to the lowest responsive responsible bidder taking into consideration information furnished with the bid as well as outside references. 4
Payment Payment will be made on a monthly basis. All invoices should be mailed to Cabarrus Health Alliance, Attn: Accounts Payable, 300 Mooresville Road, Kannapolis, NC 28081. Or emailed to kelly.chavez@cabarrushealth.org. Contract Period Contract shall begin on July 1, 2016 and end upon termination of all leases for the facilities covered by this agreement. Notice of such termination shall be given no later than thirty days prior to the effective date. Cancellation This contract may be cancelled upon thirty (30) days written notice by either party unless work proves unsatisfactory and after notice is not corrected, it may be terminated immediately. Contractor and Personnel The contractor shall use only qualified personnel on the job and shall be responsible for insuring that employees abide by all rules and regulations set forth for the building. Employees shall be instructed not to disturb desks, drawers, materials lying on desks, or use the telephone for personal use. Contractor and employees shall be responsible for insuring all keys are handled properly. The contractor will provide to Cabarrus Health Alliance the names all employees who shall perform work under this contract. The contractor will provide a signed statement that all employees have completed and passed a satisfactory background check with the contractor. Bidders are required to see the area to be cleaned before bidding and familiarize themselves with the work. The bidder is expected to have become familiar with, and take into consideration, working conditions and site conditions which may affect the award. A plea of ignorance of the conditions that exist, or may hereafter exist, at the site of the work, or difficulties that may be encountered in the execution of the work, as a result of failure to make necessary investigations and examinations, will not be accepted as an excuse for any failure, or omission on the part of the successful documents and to complete the work for the consideration set forth herein, or as a basis for any claim whatsoever. SERVICE TO BE PERFORMED Janitorial Services shall be performed after (5:00 pm) normal office hours Daily: Empty all trash cans and keep free of dirt and contamination. Each trashcan shall be kept clean. Trashcan shall be lined with a plastic liner supplied by the contractor. All trash should be removed to dumpsters. Clean all windows in high traffic areas (see breakdown of traffic areas for frequency) Vacuum all conference and meeting room floors and wipe off all tables in these areas (see Appendix 1) Sweep floors in offices, hallways, stairwells and foyers Mop floors in offices, hallways, stairwells and break room areas (see Appendix 1) Carpeted floors will be thoroughly vacuumed unless the need requires greater attention. Vacuum under and around each desk and or work area, including corners and wall jams (see Appendix 1) Clean men s and ladies locker rooms: Clean showers and sinks Sweep, mop and dispose of trash Clean kitchen and break rooms as follows: Sweep and wipe up floors to remove spillage Clean counter tops, tables and chairs (wipe off tables daily) Clean sinks Clean all restrooms as follows: Clean sinks Clean and disinfect toilets and urinals. Clean mirrors, splash areas, around fixtures and partitions, and cleaning off any furniture in restrooms Replace hand towels, tissue, and soap (in dispensers if applicable) Clean walls behind toilets Clean walls around sink basin and around door jams and light switch Weekly : Mop floors in restrooms with disinfectant and scrub, including base board tile Non-carpeted floors shall be maintained with polish. The polish shall be a visible high gloss finish which shall be maintained free of scruff and scratch marks by mopping with treated dust mops, wet mopping, scrubbing, spot buffing and waxing. Medical Records room requires wax. Damp wipe and dust all window sills throughout the building Vacuum, sweep or mop under all desks and tables and corners of walls Mop floors in offices, hallways, stairwells and break room areas not included in Appendix 1 As Needed: Wash and clean exterior of filing cabinets removing fingerprints and leaving free of streaks 6

NOTICE TO VENDORS AND PARTIES TO ANY CONTRACT WITH THE CABARRUS HEALTH ALLIANCE Effective September 4, 2013, House Bill 786/S.L. 2013-418, passed by the General Assembly of North Carolina, requires that business entities and employers with whom a public entity contracts provide proof of enrollment and participation in E-Verify, an internet based system operated by the U.S. Department of Homeland Security, which may be used to determine the eligibility of new hires to work in the United States. This legislation applies to all state agencies, cities, counties, school boards, as well as all private employers doing business in North Carolina who employ 25 or more employees in the State of North Carolina. This law applies to all CHA contracts. HB786 specifically prohibits governmental units from entering into contracts unless the contractor and the contractor s subcontractors comply with the requirements of Article 2 of Chapter 64 of the General Statutes. (Article 2 of Chapter 64 establishes North Carolina s E- Verify requirements for private employers). It is important to note that the verification requirement applies to subcontractors as well as contractors. The terms contractor and subcontractor refers to any person or entity entering any kind of contractual relationship with the City. The laws specifically prohibit governmental units from entering into contracts with contractors who have not (or their subs have not) complied with E-Verify requirements. Although the new statutes don t specify the consequences for entering into a contract in violation of this prohibition, it may be reasonable to assume that the contract would be void. As proof of enrollment and participation in E-Verify, the City requires all parties entering into a contractual relationship with the City to have executed and notarized the E-Verify Affidavit attached hereto. Any violation of this provision by the Contractor, would provide grounds for a breach of contract claim by the City. Should the contractor fail to ensure that his or her subcontractors, if any, or subsequently hired subcontractors are non-compliant, would allow for the contract to be voided by the City.
