Minimum Essential Coverage MEC Administered by KBA There are 63 preventive services covered at 100% under the required government list of Preventive and Wellness Benefits when you visit a network provider. The benefits drop to 40% if you use an out-of-network provider. Services covered include immunizations, blood pressure screenings, diabetes and cholesterol screenings, prenatal visits for pregnant women and more. 15 Preventive Services for Adults 22 Preventive Services for Women 26 Covered Preventive Services for Children Short Term Disability Short term disability replaces a portion of your income if you are unable to work as a result of an accident or sickness. Benefits are guarantee issue; no health questions, for benefit eligible employees. If you don t enroll the first time you have an opportunity, you will be required to provide evidence of good health upon enrolling in this benefit at a later date. Features of the Plan include: Nonoccupational coverage - Covers off-the-job injuries and illness only Guaranteed Issue - guaranteed issue available during the initial enrollment only up to $1,500 per month 14/14 Elimination Period Minimum and Maximum Benefit 60% of gross salary with a minimum of $300 up to a maximum of $5,000 per month Maximum Monthly Benefit Period 6 months Critical Illness Insurance with Cancer Rider Critical illness insurance pays a lump-sum cash benefit directly to you following the diagnosis of a covered critical illness or diagnosis for certain cancers. Benefit can help pay for out-of-pocket expenses associated with treatment and the costs of living, including rent, mortgage or day care costs. These benefits are paid directly to you in addition to any other medical coverage you may have. Benefits are guarantee issue and no questions during the initial offering of the Plan and for newly benefit eligible employees. If you don t enroll the first time you have an opportunity, you will be required to provide evidence of good health upon enrolling in this benefit at a later date. Features of the Plan include: Pays lump sum benefit for specified illnesses such as such as heart attack, stroke, organ transplant, End Stage Renal Failure, coma, paralysis and certain cancers Employee coverage amounts from $5,000 to $50,000 (Guaranteed Issue Amount: $25,000) Spouse coverage amounts from $2,500 to $25,000 (Guaranteed Issue Amount: $15,000) Automatic coverage for 50% of the primary insured amount for children age 26 and under with employee coverage Wellness Screening Benefit - $50 Group Accident Insurance Accident insurance provides coverage for work and non-work related injuries and medical expenses such as emergency room care, hospital confinement, fractures, accidental death and more. Features of the Plan include: On and off the job coverage (24 hour coverage) No limit on the number of claims Pays regardless of any other insurance plans in which you may be enrolled Benefits available for legal spouse and dependent children (age 26 and under) Coverage includes but not limited to emergency room, surgery, anesthesia, follow-up visits, physical therapy and labs/x-rays Accidental death rider benefit Sickness Hospital Confinement Benefit ($300 per day, to a maximum of 30 days, for employees and spouse, and $225 per day for children) available

Group Accident Insurance Unum s group accident insurance can pay lump-sum benefits based on the injury you receive and the treatment you need, including emergency room care and related surgery. The benefit can help offset the out-of-pocket expenses that medical insurance does not pay, including deductibles and co-pays. A wellness option can pay an annual benefit for preventive care. What is covered? The list of covered injuries includes: broken bones burns torn ligaments cuts repaired by stitches coma due to a covered injury eye injuries ruptured discs concussion Some covered expenses include: emergency room treatment outpatient surgery facility doctor office visit hospitalization occupational therapy speech therapy chiropractic visit physical therapy Advantages of the plan Coverage is available to all eligible employees who are actively at work. * You can buy coverage for your spouse and dependent children. No health questions to answer. If you apply, you automatically receive the base plan. Coverage is portable. You may take the coverage with you if you leave the company or retire without having to answer new health questions. Unum will bill you directly. This plan includes convenient payroll deduction, so you don t have to remember to write a check for your premiums. Coverage becomes effective on the first day of the month in which payroll deductions begin. Benefits are paid for accidents that occur on and off the job or off the job. A Catastrophic Benefit is included with this plan. This pays an additional sum if a covered individual has a serious injury such as loss of sight, hearing or a limb. Benefit amounts reduce after age 65. Additional coverage option Sickness Hospital Confinement Benefit depending on your plan, your employer may have chosen to include this benefit or you may have the option to select it. This benefit pays the insured employee, spouse or child a daily benefit if he or she is in the hospital for a covered illness. Based on the plan your employer selects, the amount you receive can be $300 per day. Children s coverage pays 75% of the employee amount. This benefit is available to family members who are covered by the base plan. There is an additional charge for this feature. IMPORTANT NOTICE: THE BASE POLICY DOES NOT PROVIDE COVERAGE FOR SICKNESS. *Eligible employees must be actively at work to apply for coverage. Being actively at work means on the day the employee applies for coverage, the individual must be working at one of his/her company s business locations; or the individual must be working at a location where he/she is required to represent the company. If applying for coverage on a day that is not a scheduled workday, the employee will be considered actively at work as of his/her last scheduled workday. Employees are not considered actively at work if they are on a leave of absence. This policy or its provisions may vary or be unavailable in some states. Whole Life insurance Unum s whole life insurance is designed to pay a death benefit to your beneficiaries but it can also build cash value you can use while you are living. This benefit offers an affordable, guaranteed level of premium that won t increase with age. Unlike term life insurance offered through the workplace, this coverage can continue into retirement. Advantages of the plan Coverage is available to eligible employees age 15 to 80 who are actively at work. * You can buy coverage for your spouse and dependent children. Guaranteed at a rate of 4%. The policy can build cash value, which you may be able to borrow from during your working years. Later in life, you can use this cash value to buy a smaller paid up policy with no more premiums due. You get affordable rates when you buy this policy through your employer. The premiums do not increase with age and are conveniently deducted from your paycheck. You own the policy so you can keep this coverage if you leave the company or retire. Unum will bill you directly. Coverage becomes effective on the first day of the month in which payroll deductions begin. During enrollment, you may be able to get this insurance up to a specified amount without taking a medical exam. You may be asked a few health questions. This policy includes a Living Benefit Option Rider. That means if you are diagnosed with a medical condition that limits life expectancy to 12 months or less you can request up to 100% of the benefit amount, to a maximum of $150,000. Your spouse and dependents have this option as well. * Eligible employees must be actively at work to apply for coverage. Being actively at work means on the day the employee applies for coverage, the individual must be working at one of his/her company s business locations; or the individual must be working at a location where he/she is required to represent the company. Summary Description of Benefits for Identity Theft Coverage This Summary is provided to inform you that as a customer of LifeLock who is enrolled in a LifeLock Identity Theft Protection Program that includes identity theft insurance, you are entitled to certain benefits under the Master Policy (the "Policy"), as referenced below. This Summary Description of Benefits does not state all the terms, conditions, and exclusions of the Policy. Your benefits will be subject to all of the terms, conditions, and exclusions of the Policy, even if they are not mentioned in this Summary, and the limit of benefits for the Membership Program for which you have subscribed. A complete copy of the Policy will be provided to you upon request. Benefits In the event you are the victim of identity theft, we shall pay you the following: A. Replacement of documents the cost of replacing documents, including driver s licenses, passports, and birth and marriage certificates, as well as the cost of additional legal expenses such as affidavits and notarizations required in connection with replacing documents as a result of a Stolen Identity Event B. Traveling expenses reasonable expenses, including gas, parking and airline tickets, incurred in traveling to obtain replacement documents, to attend meetings or proceedings, or to rectify records as a result of a Stolen Identity Event C. Loss of income actual lost income for time necessarily taken off work and away from your work premises, whether whole or partial days, solely as a result of your efforts to amend or rectify records relating to your true name or identity as a result of a Stolen Identity Event D. Stolen handbag, purse or wallet actual cost of replacing your stolen handbag, purse or wallet in addition to replacing the cash contained therein, as a result of a Stolen Identity Event E. Childcare and elderly care actual costs of providing additional childcare or care of elderly relatives that you are directly responsible for while having to travel to replace documents, attend meetings or proceedings, or rectify records as a result of a Stolen Identity Event F. Fraudulent withdrawals the principal amount of money you have lost resulting from an unauthorized transfer of monies from one or more of your checking, savings, money market, or other financial accounts, unless you have received reimbursement from another source or failed to request reimbursement from the entity holding the account from which funds were stolen G. Legal costs reasonable and necessary expenses paid to lawyers and other legal professionals appointed by us and with our consent in connection with remediating a Stolen Identity Event, including defending any civil lawsuit filed against you, removing any civil judgment entered against you, defending you against any criminal charges filed against you due to the actions of another while using your identity, and assisting you with an audit or other proceeding or hearing conducted by a governmental agency as a result of a Stolen Identity Event H. Remediation services costs reasonable and necessary expenses paid to investigators with our consent in connection with remediating a Stolen Identity Event, including the costs of recovering control of your personal identity and recovering losses you incurred I. Case management services costs expenses paid to an identity restoration case manager, as needed and approved by us, in complex Stolen Identity Events For purposes of coverage under the Policy, a Stolen Identity Event is the fraudulent use of your personal identification, social security number, or other method of identifying you, including the fraudulent use of your personal identity to establish credit accounts, secure loans, enter into contracts, withdraw money from one or more of your financial accounts, or commit a crime. A Stolen Identity Event does not include the theft or unauthorized or illegal use of your business name, d/b/a or any other method of identifying your business activity. Deductible Receipt of benefits under the Policy is not subject to or conditioned upon the payment of a deductible.

Scope of Coverage The Policy provides benefits to you only if you report a Stolen Identity Event to us and LifeLock as soon as you become aware of a Stolen Identity Event, but in no event later than ninety (90) days after the Stolen Identity Event is discovered by you. Additionally, you must follow the instructions in a claims kit that will be provided to you. These instructions will include notifying major credit bureaus, the financial institution from which your monies may have been stolen and appropriate law enforcement agencies. The claims kit will also provide instructions about how to file for benefits under the Policy if the Stolen Identity Event results in losses that are covered under the Policy. You will only be covered under the Policy if you report the Stolen Identity Event within ninety (90) days of discovery, and you first discover the Stolen Identity Event while enrolled in a Membership Program. You will not be covered if the Stolen Identity Event first occurs after termination of the Policy or termination of your membership in a Membership Program. Other Insurance The coverages provided under the Policy shall be excess over any other insurance, including, without limitation, homeowner s or renter s insurance. If you have other insurance that applies to a loss under the Policy, the other insurance shall pay first. The Policy applies to the amount of loss that is in excess of the limits of your other insurance and the total of all deductibles and self-insured amounts under all of your other insurance. Duplicate Coverages If two or more of this Policy s Coverages apply to the same Loss, or if you are enrolled in more than one identity theft protection program insured by us, we will not pay more than the actual amount of any loss you incur. Limits of Insurance The following identifies the benefit limits available for each type of Loss covered under the Policy for each Membership Program LifeLock offers. You will be eligible to receive up to the maximum benefits for the following covered losses listed under the specific Membership Program for which you have subscribed.
