2 Scpe 2.1 This E-PGN tgether with any assciated Prcedures and Guidance Ntes, shall be bserved by all emplyees f the Trust and is applicable t all lift installatins directly perated r managed by the Trust. Fr clarity it des nt include lift installatins maintained under SLA n ther nn NTW Trust Hspital sites where we perate Clinical Services r frm leased prperties where the landlrd has full maintenance respnsibility, hwever, the Trust shall expect the same standards are applied as a minimum in these areas. The E-PGN is t be administered and implemented by the Trust Estates Department. 3 Statement 3.1 The Nrthumberland, Tyne and Wear NHS Fundatin Trust (the Trust / NTW) is cmmitted t effective management and cntrl f lift installatins in rder t reduce the risks t patients, staff, visitrs and members f the public. 3.2 The Estates Department will put in place an peratinal and managerial structure that accrd with Legislative Standards, Apprved Cdes f Practice, natinal and lcal guidance and current best practice. In particular the Trust shall cmply with: Health and Safety legislatin, including the Management f Health and Safety at Wrk regulatins 1999 and the Wrkplace (Health, Safety and Welfare) Regulatins 1992 (as amended); The Lifts Regulatins 1997 and the Lifting Operatin and Lifting Equipment Regulatins 1998 (LOLER); All relevant British and Eurpean standards including BS EN 81-1:1998 (Electric Lifts), BS EN 81-2:1998 (Hydraulic Lifts), pren 81-28 (remte alarms n passenger and gds passenger lifts), pren 81-70 (rules f accessibility f disabled peple t lifts), pren 81-73 (behaviur f lifts in the event f a fire); Building Regulatins Part M Access t and use f Buildings ; Equality Act 2010. 3.3 The Trust will ensure that the Estates Department is supprted by the resurces required t meet these bjectives. 3.4 In line with the LOLER 1998 we will ensure that lifting equipment supplied in prperties is f adequate strength and stability, and makes adequate prvisin fr the safety f its users. We will nly install lifts that meet the requirements set ut in the Lift Regulatins 1997. 3.5 We will maintain all lift installatins in efficient wrking rder and in gd repair. We will arrange maintenance and specified safety checks by trained cmpetent persns. We will respnd t all reprts f repairs t lift installatins in line with ur standard repairs timescales. In general, repairs t lift installatins will be cnsidered t be Urgent, and where pssible t be cmpleted within 7 days. 3.6 Fr all installatin, repair and maintenance wrk we will use Lift and Escalatr Industry Assciatin (LEIA) affiliated cntractrs. 3.7 In line with LOLER 1998 we will ensure that a thrugh examinatin is carried ut every six mnths by a cmpetent persn. We will als carry ut supplementary testing t ensure that health and safety cnditins are maintained and that any deteriratin can be detected and remedied in gd time. We will keep recrds f these examinatins fr a minimum f tw years, and ensure that a written scheme fr inspectin is available fr each lift. 4 Respnsibilities 4.1 Intrductin 4.1.1 The Chief Executive has verall respnsibility fr ensuring that this Lift E- PGN is implemented. This respnsibility will be delegated t the Directr f Estates and Facilities, fr implementatin. The Trust s Estates Department will be instructed t cmply with the requirements f the E- PGN and implement the wrk as detailed. 4.1.2 Lift installatins are a cmplex service with ptentially significant areas f risk. Effective management f this service requires gd site knwledge f equipment and key internal / peratinal vertical traffic requirements, tgether with gd industry knwledge and high quality servicing and statutry cmpliance arrangements. 4.1.3 The maintenance and peratin f lift installatins invlves a number f key persnnel either directly emplyed estates staff r specialist cnsultants and cntractrs. 4.2 Directr f Estates and Facilities (Designated Persn - Lifts) 4.2.1 The Designated Persn (Lifts) in accrdance with HTM 08-02 is the Directr f Estates and Facilities. 4.3 Head f Estates and Facilities 4.3.1 The Head f Estates and Facilities shall ensure the executin this E-PGN relating t lift installatins Trust wide. They shall ensure the identificatin and priritisatin f system imprvement schemes, and appint and audit the Respnsible Officers and ensure that they are adequately resurced. 4.4 Respnsible Officer (Estates) 4.4.1 The Respnsible Officer will have accuntability fr ensuring the requirements f this E-PGN are met and t specifically ensure that the fllwing is undertaken: Underg apprpriate training; Be aware f the requirements set ut in the apprpriate Regulatins and ensure they are cmplied with; Only issue instructins based upn this E-PGN and the supprting legislatin, ACOP and ther relevant guidance; Ensure that relevant infrmatin exists fr all lift installatins; Liaise with the insurance cmpany in respect f statutry examinatins; Identifying all lift installatins in use within yur sectr and maintain detailed recrds n the cmpnents and peratinal criteria f thse systems, in particular the safe perating limits and apprpriate maintenance arrangements; That all existing and prpsed lift installatins must be designed, manufactured and be suitable fr the intended purpse(s), installed crrectly and capable f being perated safely; That suitable prtective devices must be fitted t all lift installatins and kept in gd wrking rder at all times; That all lift installatins must be perated within the safe perating limits determined r apprved by a cmpetent persn; Instructins must be readily available t cver emergencies. That all staff invlved in the peratin, maintenance, etc. f lift installatins must be given sufficient infrmatin, instructin and training; Ensuring that cntractrs are made aware f and cmply with instructins given, relating t lift installatins at inductin, whenever apprpriate; Ensure that this E-PGN is reviewed peridically; Ensure that any n-ging maintenance requirements are adhered t. 4.4.2 The Respnsible Officers are as fllws: Central Sectr: Estates Officer (Electrical) Nrth Sectr: Estates Officer (Electrical) Suth Sectr: Estates Officer (Electrical) 4.5 Estates Maintenance Staff 4.5.1 Estates maintenance shall have a limited range f duties assciated with lift installatins, generally t carry ut checks in between the visits f the lift maintenance cntractr. These checks include: Testing alarm bells; Checking intercms r telephnes; Ensuring all lights are illuminated; Checking emergency lighting systems; Testing the peratin f car dr safety devices; Checking smthness f starting and stpping f the lift car; Listening fr any squeaks r scraping sunds; Checking fr any judder as drs perate; Ensuring that the lift levels are within the nrmal tlerances; Checking that indicatrs are shwing the crrect display; Plant Rm inspectin; Islatin f lift in emergency / failure situatins. 4.5.2 The staff shall fllw the requirements f this E-PGN and assciated Regulatins and wrking instructins, risk assessments prvided. 5 Nrthumberland, Tyne and Wear NHS Fundatin Trust E-PGN-08 - Management and Maintenance f Passenger and Gds Lifts V01 - Issue 1 Issued Nvember 2015 Part f NTW(O)32 Estates Operatins and Maintenance Plicy V01- Issued Nvember 2015

4.6 Cntractrs 4.6.1 The Trust shall use nly use apprpriately qualified and cmpetent cntractrs wh are members f The Lift and Escalatr Industry Assciatin (r ther suitably recgnised natinal federatin). Generally the maintenance cntractrs used are as fllws: KONE St Nichlas Hspital (All Lifts except Building 27); Mnkwearmuth Hspital; Ferndene; The Grange; Plummer Curt; Lyndhurst; Dryden Clinic; Elm Huse; Greenacres. OTIS St Nichlas Hspital - Building 27; Nrthgate Hspital. 4.7 Capital Prjects 4.7.1 The Capital Prjects Department under the directin f the Capital Prjects Manager shall ensure that any new lift installatin shall cmply with all relevant guidance and legislatin generally as set ut in HTM 08-02 (Lifts) and the standards set ut in E-PGN-30 Capital and Minr Wrks Standard Specificatin. 5 Release f Trapped Passengers 5.1 Fr alarm calls t the Switchbard 5.1.1 The lift alarms acrss the Trust are cnnected t ne f three places: Lcal; Direct t the Trust Switchbard; Nrthumberland, Tyne and Wear NHS Fundatin Trust E-PGN-08 - Management and Maintenance f Passenger and Gds Lifts V01 - Issue 1 Issued Nvember 2015 Part f NTW(O)32 Estates Operatins and Maintenance Plicy V01- Issued Nvember 2015 6

Direct t the Maintenance Cntractr. 5.1.2 Shuld the lift alarm cnnect t the maintenance cntractr they will dispatch an Engineer t release any trapped passengers within the hur. Where the lift alarm is lcal t a manned statin r rings the Trust s Main Receptin, the relevant maintenance cmpany OTIS r KONE Lifts are t be cntacted via there lift entrapment number, the Estates Department r the On-call Engineer if ut f hurs shuld als be cntacted. 5.1.3 Bth rganisatins t be advised f the fllwing infrmatin:- Prperty lcatin; Lift identificatin / lcatin; N f trapped passengers if knwn. 5.1.4 Reassurance shuld be given t passengers and cmmunicatins are t be left pen until passengers are released. 5.1.5 Shuld there be n respnse frm ccupants t a lift alarm the Estates Department shuld be infrmed immediately t ensure that n passengers have cllapsed r are in distress in the lift.
