Sierra Performing Arts Center - Studio Info Classes begin Tuesday September 5 th, 2017 Tuition Details Tuition is based on a Dance Season beginning Tuesday, September 5 th, 2017 June 14 th, 2018, includes allowances for Holidays, and fees are not prorated for missed classes due to illness, vacations, etc. The third student taking the lesser number of classes within a family receives classes at a 50% discount. Tuition is divided into ten monthly payments for your convenience. Payments are due and payable on the 1st of each month, Tuition is considered late on the 11 th of each month, and a $15.00 late fee will be charged. Accounts 60 days past due must be brought current or student will not be allowed to continue class. All accounts must be current to participate in Dance Fest. Returned checks will result in a $25.00 charge to your account. Pre-pay for the entire year by August 25 th to receive a 5% discount Costume Payments: Payment in full for dance fest costumes is due by Wednesday, November 15 th, 2017. You may make payments towards your costume in the office at any time prior to November 15 th. Average Costume cost is approximately $90.00. Food NO gum, drinks or food other than water are allowed inside the dance class room. Water and snacks are available for purchase at the front desk. Dance Fest 2018 Dance Fest 2018 will be held Friday June 15 th (Contemporary Show) and Saturday June 16 th (Story Ballet) at the L.J. Williams Theatre in Visalia. Release and Waiver of Liability and Indemnity Agreement (Read Carefully Before Signing) In consideration of being permitted to participate in any way in the Sierra Performing Arts Center Program indicated below and/or being permitted to enter for any purpose any restricted area (here in defined as any area where in admittance to the general public is prohibited), the parent(s) and/or legal guardian(s) of the minor participant named below agree: The parent(s) and/or legal guardian(s) will instruct the minor participant that prior to participating in the below Sierra Performing Arts Center activity or event, he or she should inspect the facilities and equipment to be used, and if he or she believes anything is unsafe, the participant should immediately advise the officials of such condition and refuse to participate. I understand and agreed that, if at any time, I feel anything to be UNSAFE, I will immediately take all precautions to avoid the unsafe area and REFUSE TO PARTICIPATE further. I/WE fully understand and acknowledge that: There are risks and dangers associated with participation in Sierra Performing Arts Center events and activities which could result in bodily injury partial and/or total disability, paralysis and death. The social and economic losses and/or damages, which could result from these risks and dangers described above, could be severe. These risks and dangers may be caused by the action, inaction or negligence of the participant or the action, inaction or negligence of others, including, but not limited to, the Releasees named below. There may be other risks not known to us or are not reasonably foreseeable at his time. I/WE accept and assume such risks and responsibility for the losses and/or damages following such injury, disability, paralysis or death, however caused and whether caused in whole or in part by the negligence of the Releasees named below. I/WE HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE the Sierra Performing Arts Center used by the participant, including its owners, managers, promoters, lessees of premises used to conduct the Sierra Performing Arts Center event or program, premises and event inspectors, underwriters, consultants and others who give recommendations, directions, or instructions to engage in risk evaluation or loss control activities regarding the Sierra Performing Arts Center facility or events held at such facility and each of them, their directors, officers, agents, employees, all for the purposes herein referred to as Releasee...FROM ALL LIABILITY TO THE UNDERSIGNED, my/our personal representatives, assigns, executors, heirs and next to kin FOR ANY AND ALL CLAIMS, DEMANDS, LOSSES OR DAMAGES AND ANY CLAIMS OR DEMANDS THEREFORE ON ACCOUNT OF ANY INJURY, INCLUDING BUT NOT LIMITED TO THE DEATH OF THE PARTICIPANT OR DAMAGE TO PROPERTY, ARISING OUT OF OR RELATING TO THE EVENT(S) CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE RELEASEE OR OTHERWISE. I/WE HEREBY acknowledge that THE ACTIVITIES OF THE EVENT(S) ARE VERY DANGEROUS and involve the risk of serious injury and/or death and/or property damage. Each of THE UNDERSIGNED also expressly acknowledges that INJURIES RECEIVED MAY BE COMPOUNDED OR INCREASED BY NEGLIGENT RESCUE OPERATIONS OR PROCEDURES OF THE RELEASEES. If, despite this release, the participant makes a claim against any of the Releasees, the parent(s) and/or legal guardian(s) will reimburse the Releasee for any money which they have paid to the participant, or on his behalf, and hold them harmless. 2017-2018 CLASS TUITION Tuition is based on classes beginning Tuesday Sept 5, 2017 Thursday June 14, 2018, and includes weekly classes with allowances for holidays and extra studio rehearsals. (See Holiday schedule above for specific dates.) Annual Tuition is divided into 10 monthly payments for your convenience, and is due on the first of each month. A $15.00 late fee will be billed to all delinquent accounts paid after the 10 th of the month. Third family member taking the lesser number of classes receives classes at a 50% discount. Dancewear Policies Ballet/Pointe Girls: Pink Ballet shoes/pink Pointe shoes. Any color leotards and tights. Ballet skirts allowed. No booty shorts allowed.
