Healthcare Flexible Spending Account (Full FSA) All FSA members receive a visa debit Benny Card providing an automatic way to pay for qualified expenses. It is still important to keep all receipts. Claim forms (if needed) as well as account balances, reimbursement instructions, eligibility, and plan information is available on Chard Snyder s website. Each year you must reelect FSA through your Benefits@ account during Open Enrollment. All monies in your account at the end of the plan year s grace period (March 15 th ) will be forfeited.
Dependent Daycare FSA Dependent Daycare FSA enrollment is available regardless of health plan enrollment decision $240 annual minimum and $5,000 annual maximum for Dependent Daycare FSA Used for children under 13 who qualify as dependents on your federal tax return Funds for dependent care are available only after the money has been deducted from your paycheck and you have incurred the dependent care expense All monies in your account at the end of the plan year s grace period (March 15 th ) will be forfeited.

Basic Health Plan Health Savings (HSA) HSA is offered to Basic Health members because it is a high-deductible consumer-driven health plan, so you may use the HSA for qualified medical expenses University contributes money into your HSA based on your health plan dependents, marital and family status (amount prorated based on your date of hire) You can also make voluntary pretax contributions into your HSA account through your paycheck Balance rolls forward at the end of each year and your HSA account earns interest Annual limits to the amount of combined pre-tax dollars that can be contributed to your HSA by employer and employee range from $3,350 to $6,650 in 2015, depending on your health plan coverage
Basic Health Plan Limited FSA Because the Basic Health Plan is a high-deductible consumer-driven health plan, in addition to your HSA you may choose to open a Limited Healthcare FSA Limited FSA funds may ONLY be used for eligible vision and dental expenses Annual minimum $240, annual maximum $2,500 All FSA and HSA benefits are administrated by Chard Snyder All FSA and HSA programs allow you to access your funds with your visa debit Benny Card
