In addition, subject to our approval and issuance of a credit card account in connection with this application and any renewals and replacements of such account ( Credit Card Account ), you authorize the RBC Wealth Management Financial Advisor ( FA ) named on this application, and any other FA or other person(s), now or hereafter, handling your account(s) at RBC Wealth Management (each an RBCWM Representative ), and only while any such RBCWM Representative remains employed by RBC Wealth Management, to undertake certain actions on your behalf with respect to the Credit Card Account in order to facilitate routine servicing of your account, including but not limited to: (1) obtain your address(es), phone number(s), current balance, credit limit, reason for any specifically identified transaction declination, statement mail date, payment due date, payment transaction date and posting date, and last payment date and amount information from us, (2) notify CNB (and/or its credit card servicer) of any damaged, lost or stolen card(s) and request replacement card(s), which replacement cards will only be sent to the address shown in CNB s records for the Credit Card Account, (3) advise CNB to place a travel notification on the Credit Card Account, and (4) confirm automatic payment has been established (if applicable).
