If you are self-employed do you have last year s tax return along with a current year s profit and loss statement or a current year s work calendar? 10. If you are claiming $0 income, have you completed the Declaration of No Income Form? This form requires information on how you meet your basic needs and how you will pay for summer camp. 11.
