TULLAHOMA UTILITIES BOARD TULLAHOMA, TENNESSEE INVITATION TO BID FOR UNDERGROUND TRENCH LESS CONDUIT INSTALLATION The Tullahoma Utilities Board is currently seeking sealed proposals for underground trench less conduit installation for work continuing through June 2015. Sealed Proposals will be received at the office of the Tullahoma Utilities Board, 901 South Jackson Street, Tullahoma, Tennessee 37388, Attention: Randall Tucker, on or before 10:00 AM local time on November 17, 2014. Immediately thereafter all bids will be publicly opened and read aloud. The contractor must own suitable horizontal directional drilling equipment and meet the insurance requirements as detailed in the specifications. Copies of bid specifications may be obtained at the Tullahoma Utilities Board Service Center, 901 South Jackson Street, Tullahoma, Tennessee, 8:30 A.M. through 4:00 P.M. Monday through Friday. The Tullahoma Utilities Board reserves the right to reject any proposal for failure to comply with all requirements of the notice or of any of the specifications; however it may waiver any minor defects or informalities at its discretion. Tullahoma Utilities Board Underground Trench-less Conduit Installation BIDDING INSTRUCTIONS The Tullahoma Utilities Board located at 901 South Jackson Street, Tullahoma, Tennessee 37388 will receive sealed proposals to be opened at 10:00 AM Central Time on November 17, 2014 for Underground Trench-Less Conduit Installation by means of Horizontal Directional Drilling for the period of November 1, 2014 through June 30, 2015. Any questions should be directed to Randall Tucker, Purchasing Agent, telephone 931-455-4515. The work to be performed shall be as described in attached Specifications dated October 16, 2014. Insurance requirements shall be as specified in the same said specifications. The Tullahoma Utilities Board recognizes that the lowest cost proposal does not necessarily indicate the best proposal in terms of capabilities, quality, or service. Therefore, the selection will be determined by the Tullahoma Utilities Board on the basis of which proposal is in the best interest of the Tullahoma Utilities Board. Considering the responsibility of the Tullahoma Utilities Board to seek the best proposals, the Tullahoma Utilities Board reserves the right to: A. Reject any or all proposals, B. Waive any or all technicalities or informalities, and/or, C. Accept any proposal deemed to be in the best interest of the Tullahoma Utilities Board. THE INSTALLATION OF ELECTRICAL CONDUIT BY MEANS OF HORIZONTAL DIRECTIONAL DRILLING. The work shall include installing Tullahoma Utilities Board provided underground electrical conduit using the horizontal directional drilling (HDD) method of installation, also commonly referred to as directional boring or guided horizontal boring. The work shall include all services, equipment, and labor for the complete and proper installation of the conduit including site restoration and environmental protection. The scope of work is as follows: Installation of Tullahoma Utilities Board provided underground electrical conduit, PVC SCH 40, HDPE or better, sizes up to and including 2.5, at a depth of no less than 48. The location of the conduit installations will be at various locations throughout the Tullahoma Utilities Board electric service area. 2. REQUIRMENTS OF WORKERS All personnel must be fully trained and capable of performing in a safe manner, any and all aspects of the work being bid on by the drilling company. 3. GENERAL The work shall be completed within 30 days from the notice to proceed that will be given in writing by Tullahoma Utilities Board. The contractor shall furnish all manpower and equipment necessary to perform this service. 2
4. PAYMENT TERMS All payments for any and all services and/or work under the contract will be a minimum of Net 30 Days. All itemized invoices will reference the applicable purchase order number assigned by the Tullahoma Utilities Board. INSURANCE REQUIREMENTS CONTRACTOR shall take out and maintain during the life of this agreement Public Liability (Bodily Injury and Property Damage) Insurance and shall protect itself and any subcontractor performing work covered under this agreement from claims for damages because of bodily injury, including accidental death, and from claims for property damages which may arise from operations be by it or by any subcontractor, or by anyone directly or indirectly employed by either of them. The CONTRACTOR S insurance shall likewise protect the OWNER and save him harmless for acts of the CONTRACTOR. The CONTRACTOR shall take out and maintain during the life of this agreement Workmen s Compensation Insurance, including occupational disease provisions, for all of its employees employed at the site of the project and, in case any work is sublet, the CONTRACTOR shall require any subcontractor similarly to provide Workmen s Compensation Insurance, including occupational disease provisions, for all the latter s employees unless such employees are covered by the protection afforded by the CONTRACTOR. 6. ENVIRONMENTAL PROTECTION Contractor shall adhere to all applicable state, federal, or local environmental regulations. 7. SAFETY Contractor shall adhere to all applicable state, federal, and local safety regulations and all operations shall be conducted in a safe manner. Contractor shall be responsible for making Tennessee One-Call locate requests and having all underground utilities located before starting work. 8. SITE RESTORATION Following drilling operations, contractor will de-mobilize equipment and restore the work site to original condition. All excavations will be backfilled and compacted to 95% of original density. Landscaping will be restored to original. All mud shall be disposed of by the contractor. 9. PERMITS Contractor shall be responsible for obtaining any and all state, federal, and local permits required for the project. 10. DAMAGES Contractor shall be held financially responsible for all damages that occur during the course of his work. This shall include any hidden damages that are discovered during or up to one year after the work is complete. 4
