Important Information We are pleased to introduce new features and innovations for your Flex Spending Account (FSA) and/or Health Savings Account (HSA) in 2016. This letter aims to get you off on the right path to ensure a successful 2016 plan year. 2016 NEW System Highlights TASC moved to a NEW simple and easy-to-use online system to manage all of your accounts. https://partners.tasconline.com/etfemployee The new online system offers: Fast and consistent notifications regarding required documentation. Pay-the-Provider reimbursement option. Online reimbursement upload Online substantiation upload New co-branded TASC Debit Card that manages multiple Plans. New Mobile App to assist in managing benefit accounts. (e.g. : account balances, upload reimbursement requests, upload documentation to substantiate transactions, etc. ). Important Dates and Reminders March 30, 2016 is the deadline for all 2015 Flex Spending Account (FSA) Activity. Reimbursement Requests: The last day to submit reimbursement requests for eligible 2015 FSA expenses is March 30, 2016. Your current 2016 TASC Debit Card cannot be used to pay for 2015 FSA expenses. Submit 2015 reimbursement requests through the 2015 TASC online system at: www.tasconline.com using your 2015 12 digit MyTASC ID. Unsubstantiated FSA Claims: the deadline date to submit documentation to resolve any unsubstantiated 2015 FSA Claims is March 30, 2016. 2015 FSA expense(s) not substantiated by the deadline date will be considered ineligible and repayment will be required. Wages may be garnished for repayment of ineligible expenses. Contact TASC Customer Care if you need assistance regarding missing documentation or ineligible expenses. All 2015 reimbursement requests or substantiation documentation must be received by March 30, 2016. Any 2015 reimbursement requests or substantiation documentation received after March 30, 2016, will not be processed. Please note that when submitting reimbursement requests, you must enter a date of service within the 2015 plan year in order to be reimbursed. All 2015 reimbursement requests or substantiation document submissions must be sent through the 2015 TASC online system at www.tasconline.com. 2015 Carryover Funds: You will see your 2015 Health Care FSA, Parking, and/or Transit funds automatically carried over to your 2016 TASC accounts by the end of April. For 2016, TASC moved to a NEW online system. All 2016 TASC Account Activity must be completed through 2016 TASC online system at: https://partners.tasconline.com/etfemployee. You were issued a new username and password for the 2016 online system. See below section How to Effectively Manage Your Accounts for more information. 2016 Reimbursement Requests for services incurred within the 2016 plan year, must be submitted through your online 2016 TASC account.

How to Effectively Manage Your TASC Accounts It is your responsibility to effectively manage your benefits accounts. In order to do this, TASC provides many options to make this an easy process. TASC offers 24-hour account access online, TASC mobile App, and TASC Customer Care options for you to manage your benefits accounts. To manage your 2016 TASC Account, log on to your online 2016 TASC Account at: https://partners.tasconline.com/etfemployee. Enter your username and password you used during open enrollment. If you did not enroll online or have not logged into the New online system for 2016, use the below format to log in. o Username: First initial of first name + full last name + DOB (mmddyy) + last 4 digits of SSN (e.g.: jsmith0315815643) o Password: First initial of first name + full last name + DOB (mmddyy) + last 4 digits of SSN (e.g.: jsmith0315815643 (same as username) Once logged in, you need to change your username and password as it does contain sensitive information. You can do so by clicking on Profile and then Log-in Information. If you need assistance, please contact TASC Customer Care at 1-844-786-3947. Setting Up Your Notification Preferences for Your 2016 TASC Account To ensure that you receive timely notifications from TASC for you to effectively manage your Flex Spending and/or Health Savings Accounts, it is important that you log into your online 2016 TASC Account and check that your notification preference in your profile is correct. Log into https://partners.tasconline.com/etfemployee, click on Statement & Notifications, then click on Update Notification Preferences, and edit your preferred preference information. Important: Work emails have a tendency to get blocked as spam, therefore, it is important that you change your email to a personal email address. Also, it is important to add TASC as a known contact within your personal email to prevent important notifications being identified as junk mail. Note: Your privacy is important to us, therefore TASC does not disclose emails or other private information to third parties. NEW Co-branded TASC Debit Card Everyone enrolled in the 2016 FSA and/or HSA received a NEW co-branded TASC Debit Card. You can use this new TASC Debit Card to access all of your Flex Spending funds. It s perfect if you are enrolled in multiple plans. If you misplaced, or did not receive your TASC Debit Card, simply request a replacement card through your online 2016 TASC Account or by contacting TASC Customer Care for assistance. Would you like a TASC Debit Card for your dependents? These cards feature the dependent s name. You can order the TASC Debit Card online through your 2016 TASC Account.

FOR FLEX SPENDING ACCOUNT (FSA) PARTICIPANTS ONLY Your 2015 TASC Debit Card Health Care FSA and LPFSA TASC Debit Cards were deactivated December 31, 2015. Any 2015 FSA or LPFSA claim reimbursement requests made after December 31, 2015 must be sent through the online TASC website, fax, or mail. You will need to complete a Reimbursement Request Form and submit it to TASC for reimbursement. 2015 MyCash Accounts: If your 2015 TASC Debit Card has a balance, you may continue to use your TASC Debit Card to access these funds after December 31, 2015. Or go online (www.tasconline.com ) to request that your MyCash funds be moved/deposited into your bank account. Note: The MyCash feature is no longer available for the 2016 plan year. Any reimbursements submitted will be refunded by check to the address TASC has on file in your 2016 TASC Account. If you wish to have your reimbursements directly deposited into your personal bank account, please log into your 2016 TASC Account to set up direct deposit by clicking on the banking/cards tab within your profile. Your TASC Debit Card Substantiation Responsibilities It is your responsibility to provide substantiation if a TASC Debit Card transaction requires proof of the medical expense. Although the TASC Debit Card makes purchasing Health Care Flex Spending expenses simple, substantiation may still be required. To view claims requiring substantiation, log into your online 2016 TASC Account at https://partners.tasconline.com/etfemployee Notifications will be sent on the 1st, 15th, and 30th days after a transaction requiring substantiation. If you have an email on file with TASC, these notifications will be received via email. If an email is not on file, the notifications will be mailed. You must submit proper substantiation within 45 days of the 1st notice. If you do not submit proper substantiation, your TASC Debit Card will be de-activated until approved substantiation, offset claims, or repayment is received. Per IRS guidelines your employer reserves the right to deduct funds from your payroll check or impute income for any unsubstantiated claims. Choose one of the following options to submit your documentation to substantiate your transaction: (1) Online 2016 TASC Account: Upload documentation via your online 2016 TASC Account (go to Claims page, click View Requests, then click Upload to attach documentation), or (2) Mobile App: Upload documentation via your 2016 TASC mobile app. (3) Fax or Mail: Submit receipt along with your printed Substantiation Notification via fax to 608-316- 6097 or toll free fax #1-877-231-1287; or mail to TASC, P.O. Box 7511 Madison, WI 53707-7511. Substantiation Supportive Documentation Tips: Documentation must include the following: Provider Name, Date of Service, Cost, Type of service provided, Name of the person receiving the service, Name of the drug and Rx number. Examples of documentation with all required information include your statement, invoice, bill, or

explanation of benefits (EOB). Examples of documentation that do not have all required information include: debit card receipts with no description of service or canceled checks. Health Care Flex Spending Account $500 Carryover Effective January 1, 2015, Health Care Flex Spending Account (FSA) and Health Care Limited Purpose Flex Spending Account (LPFSA) participants can carryover up to $500 into the next plan year. You will be able to file your 2015 health care expenses through the end of the run-out period (March 30, 2016). After the run-out period any unused Health Care FSA funds (up to $500) will automatically carryover to your current 2016 plan year benefits. Carryover funds will be transferred and available in your 2016 Health Care FSA or LPFSA TASC Account mid-april 2016. Any unused funds over the $500 carryover limit are forfeited. If you are an active employee with Health Care FSA or LPFSA carryover funds and did not enroll into a Health Care FSA or LPFSA for the 2016 plan year, TASC will auto-enroll you into a corresponding plan for these funds to carryover into for the 2016 plan year. Participants will be notified and sent a TASC Debit Card. These funds will be available to you until you have either exhausted the funds or terminate employment, whichever comes first. Under the Health Savings Account (HSA) regulations, a participant cannot have both a Health Care FSA and a HSA. Therefore, if you enrolled into a High Deductible Health Plan and HSA for 2016, your funds will be carried over into a Limited Purpose Flexible Spending Account (LPFSA). What if you have up to $500 balance in a 2015 Health Care or Limited FSA Plan but do not re-enroll in 2016? TASC will automatically enroll you in an appropriate FSA and add your 2015 carryover funds up to $500 to your 2016 TASC Account. Once the funds are carried over, you will receive an enrollment confirmation and a 2016 TASC Debit Card will be sent. What if you have carryover funds in your 2015 FSA but enrolled in a Health Savings Account for 2016? TASC will automatically carryover your Health Care FSA funds (up to $500) into a Limited Purpose FSA (LPFSA). Per IRS regulations, if you are enrolled in an HSA, you may be enrolled in the Limited Purpose FSA only, and may use the funds for dental and/or vision expenses. Dependent Day Care Flex Spending Account Unused Funds The Dependent Day Care FSA does not feature a carryover option. Also, the Dependent Day Care FSA does feature a Grace Period, but does feature a Runout Period. That means for dependent daycare expense claims incurred within the 2015 plan year (1/1/2015-12/31/2015), you can file claims to your 2015 Dependent Day Care Flex Spending Account through the end of the run-out period, which is March 30, 2016. Any unused 2015 Dependent Day Care FSA funds are forfeited. For Transit Participants Only Effective January 1, 2016, the Internal Revenue Service increased the Transit limit from $130 a month to $255 a month. If you would like to change your contribution amount to take advantage of this increase, submit a completed Change of Election Form to your payroll/benefit office. Note: TASC encourages you to use your TASC Debit Card for Transit expenses whenever possible.

FOR HEALTH SAVINGS ACCOUNT (HSA) PARTICIPANTS ONLY In order to take advantage of this newly upgraded system, it requires that TASC moves your current 2015 HSA balance to a new 2016 HSA account. To ensure timely transition of your 2015 HSA funds, you must authorize this process as soon as possible. To authorize the transfer, submit the completed TASC HSA Balance Transfer Form to TASC by following the instructions provided on the form. Please allow 5-7 business days for 2015 funds to be available in your 2016 HSA account. If you fax or mail your form to TASC, please allow 10 12 business days for the transfer to complete. A few things to note regarding the transfer of your HSA funds: Wait until you have submitted all your 2015 claims for reimbursement. You will have no access to 2015 HSA funds during the transition period. If you do not request to have your funds transferred, you may continue to access your 2015 HSA funds using your 2015 TASC Debit Card. However, if you enrolled in the 2016 HSA, all 2016 contributions will be deposited into your 2016 HSA account, which will require you to use your new 2016 co-branded TASC Debit Card or your online 2016 TASC Account to access those funds. All forms are located in your online 2016 TASC Account (https://partners.tasconline.com/etfemployee) under the Tools and Support Tab.
