This includes all operations in which a subcontractor is performing work for DiFiore Construction Inc. We will do our utmost to protect people form injury and prevent accidents. To do this, we need and expect your wholehearted cooperation in carrying out this Accident Prevention Plan in its entirety. Each Subcontractor is responsible for enforcing the safety and loss prevention program on his/her work. This document is provided to assist in fulfilling that responsibility. No subcontractor will be allowed to perform work unless this signed document is on file with us. Your responsibility as subcontractor for DiFiore Construction Inc., is as follows: 1. Read and understand the enclosed subcontractor safety policy. 2. Ask questions and clarify any misunderstanding regarding this safety policy as it relates to your work. 3. Appoint an on site representative to enforce this policy for your employees. 4. Work with DiFiore Construction Inc., to ensure that all work is done in a safe manner consistent with the safety policy. I fully understand that adherence to this plan is a term and condition of my subcontract agreement with DiFiore Construction Inc. Serious violations of this policy on the part of any employee of subcontractor can be cause for termination of the subcontract agreement in its entirety. ACCIDENT PREVENTION POLICY Subcontractor s to Keep The injured employee and his or her family suffer the most from any needless accident. These safety rules are for the benefit and protection of employees and it is insisted that they be followed at all times. A copy of the full safety policy is posted in the field office and is carrier by each Foreperson. General Safety Instruction 1. You are not prepared to do a job until you know how to do it safely. If you do not know how, ask for further instructions. 2. Climb down from an elevation-do not jump down. Use ladder designed for the job. 3. Report all injuries promptly for treatment. Even small cuts or scratches can become infected unless properly cared for. 4. Remove or hammer down protruding nails in old lumber, forms, etc 5. Never work under suspended loads. Stand to one side until the load has cleared your area. 6. Check cables, chains and other lifting equipment for signs of wear or possible breakage. 7. Obey Warning or Danger signs and tags. They are posted to point out hazards. 8. Get help to carry an awkward load when it is necessary. Decide how to put it down when you are deciding how to pick it up. 9. Practical jokes and horseplay are forbidden on the job. The injuries they often produce are no laughing matter. 10. Drinking and or drugs are forbidden on the jobsite. Do not report to work when under the influence. This holds true for drinking at lunch also. 11. Any unsafe condition on the job should be brought to your forepersons attention. All employees are urged to make suggestions that will improve job safety. Individual Safety Instructions 1. Wear hard hats on ALL JOBS. Keep your hard hat available for use on the job, not at home. 2. Work shoes will be worn at all times. Sandals, sneakers or not shoes at all are totally unacceptable. 3. Shirts with sleeves are to be worn at all times. Shorts are not allowed. 4. No personal stereos are to be used on the jobsite. 5. Wear appropriate goggles, shields, hoods, etc. when welding, grinding, using cut-off stays, operating a jackhammer or any operation which may cause eye injury. 6. Earplugs will be provided and should be worn when exposed to sounds for extended length of time. 7. No one is permitted to ride any power equipment except for the operator. 8. Shut off engines or equipment before making repairs or adjustments. 9. Use the proper tool for each job. 10. Treat all electric wires as hot. 11. Stay clear of operations in which you are not involved. 12. Wear safety vest when directly traffic and when working in travel lanes. 13. Always be courteous and professional to the general public. 12. Never use tools or equipment with missing or defective guards. Bring tools in this condition to your foreperson. 2
SUBCONTRACTORS Accident prevention on this job can only be successful through the whole hearted cooperation of all subcontractors, their foreperson, and the worker s. Subcontractors must insist that their foreperson comply with all details of the general safety program. All subcontractors will designate a responsible individual at the jobsite, who will assume direction of their safety activity and can take immediate action on safety problems. In addition they will: 1. Read the attached general and individual safety instructions, and see that all workers adhere to these safety guidelines. This form must be signed by a responsible person prior to the start of any work. 2. Assume the responsibility for the availability and use of personal protective equipment. 3. Be available to insurance engineer and claim insurance personnel when necessary. 4. Assume responsibility for the prompt filing out and routing of accident and claim reports. 3
SUBCONTRACTORS Purpose: Establish the subcontractors safety responsibilities or duties on the job site. General: Each subcontractor is required to comply with provisions of the Occupational Safety and Health Administration standards, state, local and owner s regulations specific to this project. Responsibility: Permission and special instructions related to the safety and security of the job site must be secured before beginning work in any area on site. Before any work is started, the insurance certificate and or permits that may be required by the public authorities or the client in connection with the performance of their contract shall be on file in the site Field Office. Each subcontractor shall have a written Safety Program in place consistent with the Rochester area Construction Alliance. Listed below are safety rules and regulations as prescribed by Federal, State, and Local agencies. Compliance with these rules is required. This includes but is not limited to the following: 1. Furnish approved personal safety equipment for employee (hard hats, eye protection, safety harnesses, personal fall arrest equipment, etc.) 2. Provide training, instructions, and monitoring for personal protective equipment to work in hazardous locations or perform special projects. 3. Give instructions to all employees as to the nature of the work, hazards of the job, use of protective equipment, safety rules and site rules under which they will work. 4. Report all accidents and injuries immediately. Each subcontractor is responsible for filling out any required reports or records. Transportation of injured employees is the responsibility of the subcontractor. A copy of the subcontractor accident report and NY State C-2 Form must be submitted to owner s representative. 5. Follow the job site housekeeping rules dealing with proper disposal and storage of debris or materials. 6. Keep flammable liquids in approved safety containers and stored in specific areas as assigned with proper fire fighting equipment. 7. Other fire protection and preventive measures are mandatory. This includes fire extinguishers, proper storage of materials and supplies and other 4
procedures to protect flammable or combustible materials or areas on site. The use of job make heaters prohibited. Only approved heating devices are acceptable with no open fires burning. 8. Each subcontractor will provide his/her own fire extinguishers to protect his/her own equipment, materials, buildings, storage and work areas. Fire extinguishers must be inspected and tagged at least annually. 9. All job site equipment such as trucks, motor cranes, fork lifts, boom trucks, etc., must meet all federal, state, local, and site regulations and should be equipped with fire extinguishers. 10. All scaffolds, work platforms, and open sided floors shall be protected with handrails and toe boards. Safety harnesses will be used for work being performed from elevated areas of greater than 6 feet. 11. Any trench, cut, or holes over 5 feet deep will be properly shored, sloped, or otherwise protected. Access to and from such areas must be provided within 25 feet of where employee are working. 12. All trucks, cranes, bulldozers, and other mobile equipment shall be parked in designated areas and locked or otherwise secured after normal working hours. 13. Any work areas, confined or enclosed spaces shall be monitored for oxygen and or hazardous gas or be ventilated before employees are allowed to enter. Such spaces will be identified, marked and proper access maintained with written daily records. Employees will be instructed as to the hazards and proper working procedure before entry into such areas. A Confined Space Permit is required from Owner s representative. 14. Any work area that presents a hazard such as overhead work, open holes, etc., will be blocked off or barricaded. 15. All welding, cutting or hot work being performed will require protection from slag and or sparks in the area under or close to such work. A hot work permit is required from Owner s Representative. A fire watch is required for 30 minutes after completion of hot work. 16. All outside buildings, trailers, tool rooms, fabrication shops or other buildings will be properly erected and fire protection will be provided. 17. Any hazardous materials used in the performance of the work will be listed with a Hazardous Material Data Sheet from the manufacturer and filed in the Field Office. Subcontractors will instruct their employees in the proper use and or disposal of hazardous materials. 18. All tools and equipment brought into the site for subcontractor use are subject to inspection and approval by the Owner s Representative. 5
19. Loaning of tools and equipment to other subcontractors is not recommended due to the possibility of faulty equipment and liability that follows the piece of equipment. 20. Subcontractors must inspect the work site on a daily basis, inspecting tools and equipment, housekeeping, fire hazards, fall hazards and other exposures to hazardous conditions. 21. All of the above procedures are mandatory for all contractors as well as their subcontractors, in any. These are only minimum requirements and should be used as a guide to a safe site. 22. Any disciplinary actions as prescribed by site procedures will be strictly enforced. 6
